Heart donation after a fatal stab wound to the right ventricle: a case report.

Trauma victims are a significant part of the donor pool, though injuries to donor organs normally exclude them from their procurement. We present the case of a 37-year-old male with a stab wound to the chest, resulting in penetrating sharp force injury to the right ventricle of the heart. Despite the ultimately fatal injury, the heart was successfully repaired, and later donated, after brain death. Resuscitation and surgical intervention, along with the collaboration of medical and legal parties, enabled heart donation to occur without compromising the quality of forensic postmortem examinatio

Do reactive oxygen species damage or protect the heart in ischemia and reperfusion? Analysis on experimental and clinical data.

The role of reactive oxygen species (ROS) in ischemic and reperfusion (I/R) injury of the heart has been discussed for more than 40 years. It has been demonstrated that reperfusion triggers a multiple increase in free radical generation in the isolated heart. Antioxidants were found to have the ability to mitigate I/R injury of the heart. However, it is unclear whether their cardioprotective effect truly depends on the decrease of ROS levels in myocardial tissues. Since high doses and high concentrations of antioxidants were experimentally used, it is highly likely that the cardioprotective ef

The Function and Mechanisms of Autophagy in Trauma of Other Parts of the Body.

Trauma is common in modern society. Besides TBI and SCI, trauma can lead to severe cardiopulmonary injury and even to death. Fracture and skin injury are also very likely to occur in our daily life. Limited studies have reported the levels of autophagy after heart and trauma, fracture, and skin injury. In this chapter, we update the current state of knowledge and recent advances in the study of autophagy after trauma including heart and lung trauma, fracture, and skin injury which we try to clarify how autophagy levels are affected by injury or trauma and how their manipulation may represent p

Protective transcriptional mechanisms in cardiomyocytes and cardiac fibroblasts.

Heart failure is the leading cause of morbidity and mortality worldwide. Several lines of evidence suggest that physical activity and exercise can pre-condition the heart to improve the response to acute cardiac injury such as myocardial infarction or ischemia/reperfusion injury, preventing the progression to heart failure. It is becoming more apparent that cardioprotection is a concerted effort between multiple cell types and converging signaling pathways. However, the molecular mechanisms of cardioprotection are not completely understood. What is clear is that the mechanisms underlying this

Retrospective Analysis of Cardiovascular Effects of FES Cycling in People with Complete and Incomplete Spinal Cord Injury.

Background: Globally, over 15 million people live with spinal cord injury (SCI), which often leads to permanent motor impairment. In these cases, functional electrical stimulation (FES) can generate muscle forces and active movements in affected body parts, enabling patients to perform cycling tasks using their own paralyzed legs. Incomplete spinal cord injuries are more prevalent than complete injuries and FES cycling can be performed in both cases. However, differences in its effects between the two groups remain to be further investigated. Our objective is to compare the effects of FES-assi

Resuscitation thoracotomy.

Resuscitation thoracotomy is a rarely performed procedure whose use, in France, remains marginal. It has five specific goals that correspond point-by-point to the causes of traumatic cardiac arrest: decompression of pericardial tamponade, control of cardiac hemorrhage, performance of internal cardiac massage, cross-clamping of the descending thoracic aorta, and control of lung injuries and other intra-thoracic hemorrhage. This approach is part of an overall Damage Control strategy, with a targeted operating time of less than 60minutes. It is indicated for patients with cardiac arrest after pen

Butyrate Ameliorates ISO-Induced Cardiac and Intestinal Injury in Rats via Modulation of Bitter Taste Receptors (Tas2rs) and GPR41/43 to Inhibit NLRP3 Activation.

Background: The gut microbiota and its metabolite short-chain fatty acids (SCFAs) regulate host physiology, but whether butyrate, a key SCFA, protects against myocardial injury via the gut-heart axis remains unclear. Objectives: This study aimed to investigate the cardioprotective effect of butyrate in a rat model of isoproterenol (ISO)-induced myocardial injury and to explore its underlying gut-heart mechanism. Methods: In this experimental study, male Sprague-Dawley rats received intragastric butyrate pre-treatment followed by ISO injection to induce myocardial injury. Cardiac function, myoc

[Contribution of color echocardiography to the diagnosis of post-traumatic left ventricular-right atrial septal defect].

The authors report the case of an acquired left ventricle--right atrial communication after closed chest trauma. This communication was associated with rupture of the aortic isthmus and complete atrioventricular block. The left-to-right shunt which was assumed for a long time to be a small, well-tolerated, ventricular septal defect, finally required surgical repair. The diagnosis was eventually made clear by Doppler color flow mapping which was of great value in orienting the surgical procedure.

Cathelicidin aggravates myocardial ischemia/reperfusion injury via activating TLR4 signaling and P2X7R/NLRP3 inflammasome.

AIMS: The antimicrobial peptide cathelicidin (Camp) has multifunctional immunomodulatory activities. However, its roles in inflammation-related myocardial ischemia/reperfusion (MI/R) injury remain unclear. METHODS AND RESULTS: In this study, adult male C57BL/6 wild-type (WT) mice were subjected to MI/R injury by left anterior descending coronary artery ligation for 45 min followed by 3 or 24 h of reperfusion. An abundant cardiac expression of cathelicidin was observed during ischemia and reperfusion, which was mainly derived from heart-infiltrating neutrophils. Knockout of Camp in mice reduce

Pediatric heart-kidney transplantation.

Heart transplantation is regarded as the definitive treatment for advanced pediatric heart failure. However, concomitant kidney dysfunction often complicates candidate selection for isolated heart transplantation. Prolonged venous congestion, nephrotoxic exposures, and recurrent episodes of acute kidney injury can result in varying degrees of renal impairment. Differentiating between reversible kidney injury secondary to cardiorenal syndrome and irreversible intrinsic renal disease remains a major challenge. Pediatric heart-kidney transplantation has shown survival benefits for patients with e

Successful management of left-sided thoracic impalement in a resource-limited setting: A case report.

INTRODUCTION: Thoracic impalement injuries are rare and life-threatening. Managing them in resource-limited settings poses significant challenges. CASE PRESENTATION: A 63-year-old male fell 3 m from a tree, sustaining a left-sided chest impalement at the 7th intercostal space along the mid-axillary line. On arrival, his vitals were BP 100/70 mmHg, pulse 104 bpm, RR 32/min, and SpO₂ 78 % (improving to 90 % with oxygen). Examination revealed absent air entry on the left, a sucking chest wound, and an impaled wooden fragment. Initial management included resuscitation, antibiotics, and tetanus pr

CNN sensor based motion capture system application in basketball training and injury prevention.

Basketball is a high-intensity sport, and sports injuries often occur. Therefore, how to monitor the sports status of basketball players in real time, discover and prevent the occurrence of sports injuries in time, has become an urgent problem for athletes and coaches. In this paper, a motion capture system based on CNN sensor is proposed. Through the application of sensor, real-time monitoring of athletes' motion state, the system can collect athletes' movement track, speed, acceleration, stride frequency, heart rate, energy consumption and other parameters in real time. By analyzing the move

Sympathetic Nerve Activation in Acute Kidney Injury and Cardiorenal Syndrome.

The interactions between the kidney and heart are well studied and frequently lumped together as cardiorenal syndrome. It is believed that the sympathetic nervous system is involved in the mechanism of kidney injury caused by heart failure, but direct evidence is still lacking. In chronic renal fibrosis, sympathetic nerve activation was demonstrated to be harmful by unilateral ureteral obstruction and post-ischemia reperfusion injury models. On the other hand, sympathetic nerve activation seemed protective in acute kidney injury models such as ischemia reperfusion injury and lipopolysaccharide

Promoting cardiomyocyte proliferation for myocardial regeneration in large mammals.

From molecular and cellular perspectives, heart failure is caused by the loss of cardiomyocytes-the fundamental contractile units of the heart. Because mammalian cardiomyocytes exit the cell cycle shortly after birth, the cardiomyocyte damage induced by myocardial infarction (MI) typically leads to dilatation of the left ventricle (LV) and often progresses to heart failure. However, recent findings indicate that the hearts of neonatal pigs completely regenerated the cardiomyocytes that were lost to MI when the injury occurred on postnatal day 1 (P1). This recovery was accompanied by increases

Cardiac laceration following non-penetrating chest trauma in dog and cat.

Cardiac laceration with non-penetrating chest trauma is reported as a common cause of death in human following rapid deceleration in high-speed vehicular accident. In contrast, in veterinary medicine, traumatic rupture of heart and great-vessel structures appears to be an uncommon cause of death. Here we report three cases of cardiac laceration following non-penetrating chest trauma in a one cat and two dogs. In two of these cases, necropsy revealed a rupture of the heart associated with fractures of the ribs and lung contusion; only one case did not exhibit any external chest injury but revea

CUEDC2 modulates cardiomyocyte oxidative capacity by regulating GPX1 stability.

The irreversible loss of cardiomyocytes due to oxidative stress is the main cause of heart dysfunction following ischemia/reperfusion (I/R) injury and ageing-induced cardiomyopathy. Here, we report that CUEDC2, a CUE domain-containing protein, plays a critical role in oxidative stress-induced cardiac injury. Cuedc2(-/-) cardiomyocytes exhibited a greater resistance to oxidative stress-induced cell death. Loss of CUEDC2 enhanced the antioxidant capacity of cardiomyocytes, promoted reactive oxygen species (ROS) scavenging, and subsequently inhibited the redox-dependent activation of signaling pa

Signaling mediators modulated by cardioprotective interventions in healthy and diabetic myocardium with ischaemia-reperfusion injury.

Ischaemic heart diseases are one of the major causes of death in the world. In most patients, ischaemic heart disease is coincident with other risk factors such as diabetes. Patients with diabetes are more prone to cardiac ischaemic dysfunctions including ischaemia-reperfusion injury. Ischaemic preconditioning, postconditioning and remote conditionings are reliable interventions to protect the myocardium against ischaemia-reperfusion injuries through activating various signaling pathways and intracellular mediators. Diabetes can disrupt the intracellular signaling cascades involved in these my

Re-evaluating the causes and consequences of non-resolving inflammation in chronic cardiovascular disease.

Cardiac injuries, like heart attacks, drive the secondary pathology with advanced heart failure. In this process, non-resolving inflammation is a prime component of accelerated cardiovascular disease and subsequent fatal events associated with imbalanced diet, physical inactivity, disrupted circadian rhythms, neuro-hormonal stress, and poly- or co-medication. Laboratory rodents have established that splenic leukocyte-directed resolution mechanisms are essential for cardiac repair after injury. Here, we discuss the impact of three lifestyle-related factors that are prime causes of derailed card
