The Impact of Early CT on Outcomes for Empyema Patients Admitted Through the Emergency Department.

BACKGROUND: Empyema incidence is rising, with high mortality, and patients frequently requiring invasive procedural management. Early intervention is linked with reduced procedural invasiveness and rates of general complications. CT can aid diagnostic certainty and guide management approaches, but the value of CT timing on outcomes remains unclear. OBJECTIVE: Describe the association between CT timing and outcomes for patients diagnosed with Empyema who present through the emergency department. METHODS: A retrospective cohort study of adult empyema patients was performed using TriNetX data (

Severe H1N1-associated acute respiratory failure in immunocompromised children.

BACKGROUND: Severe pandemic influenza A (H1N1) infection can lead to acute respiratory failure (ARF) with associated high mortality. Children with malignancy may be at higher risk of H1N1-associated ARF because of underlying primary disease or immunosuppression associated with chemotherapy. PROCEDURE: We describe the clinical course and outcome of critically ill pediatric oncology/hematology patients with H1N1-associated ARF. RESULTS: Five patients were admitted to the St. Jude Children's Research Hospital (SJCRH) ICU with H1N1 infection during the 2009-2010 influenza season. Underlying diag

Assessment of cerebrovascular interactions and control in coronary artery disease patients undergoing anaesthesia through bivariate predictability measures.

Cerebrovascular regulation, driven by mechanisms such as cerebral autoregulation and the Cushing's reflex, plays a critical role in maintaining cerebral blood flow (CBF) adequate despite changes in arterial pressure (AP), since a dampening of CBF can lead to serious brain pathologies. This study investigates the causal and self-predictable dynamics of cerebrovascular interactions in patients undergoing coronary artery bypass graft surgery, before and after propofol general anaesthesia. The dynamics of the pressure-to-flow and flow-to-pressure links between mean arterial pressure (MAP) and mean

Computed tomography pulmonary angiography versus ventilation-perfusion lung scanning for diagnosing pulmonary embolism during pregnancy: a systematic review and meta-analysis.

Differences between computed tomography pulmonary angiography and ventilation-perfusion lung scanning in pregnant patients with suspected acute pulmonary embolism are not well-known, leading to ongoing debate on which test to choose. We searched in PubMed, EMBASE, Web of Science and the Cochrane Library databases and identified all relevant articles and abstracts published up to October 1, 2017. We assessed diagnostic efficiency, frequency of non-diagnostic results and maternal and fetal exposure to radiation exposure. We included 13 studies for the diagnostic efficiency analysis, 30 for the a

Small cerebellar hemorrhage in preterm infants: perinatal and postnatal factors and outcome.

The objective of the study is to determine perinatal and postnatal factors that may affect the occurrence of small cerebellar hemorrhage (CBH) and to evaluate the effect of small CBH on neurodevelopmental outcome in very preterm infants. This prospective study in an unselected cohort of very preterm infants was approved by the medical ethics committee, and informed parental consent was obtained. Presence of small CBH (<4 mm) was assessed with magnetic resonance imaging around term equivalent age in 108 preterm infants (<32 weeks gestation). We compared infants with and without small CBH for pe

Acquired benign tracheo‑oesophageal fistula secondary to oesophageal stenting for post lye ingestion stenosis: A case report.

Acquired benign trachea-oesophageal fistula is a rare benign pathological entity with varying aetiologies that most often occurs post-intubation. This case report presents the case of a female patient, 31 years old, admitted to the emergency room with sepsis syndrome following bilateral aspiration pneumonia caused by a large trachea-oesophageal fistula. The fistula was the result of intra-tracheal migration of an oesophageal stent placed for post lye ingestion stenosis. Esophageal diversion and partial resection with oesophageal patch to repair the tracheal defect, under general anaesthesia wi

[Factors influencing hospital infection in patients in the intensive care unit].

OBJECTIVE: The accumulation of risk factors in hospitalized patients is one of the elements contributing to the increase in the frequency of nosocomial infection in the intensive care unit (ICU). Our aim was to identify nosocomial infection risk factors in the ICU of our hospital. METHODS: We performed a prospective cohort study of 1,134 patients admitted to the ICU for at least 24 hours in 2001. The patients were followed-up for 48 hours after leaving the ICU. Multivariate Cox regression analysis was used to identify risk factors. RESULTS: The intrinsic risk factors identified were the prin

On chemoreceptor control of ventilatory responses to CO2 in unanesthetized ducks.

Using cross perfusion between pairs of animals we examined the effects of increases in arterial CO2 tension (Paco2) at pulmonary, carotid body, and central chemoreceptors on ventilation in unanesthetized, spontaneously breathing White Pekin ducks. By adjusting the level of inspired CO2 of either the experimental or donor animals it was possible to manipulate Paco2 at any one or combination of the receptor groups. Stimulation of central chemosensitive areas alone was three to four times more effective in increasing both frequency and minute ventilation (Ve) than stimulation of the carotid bodie

Laryngeal motoneurone activity in the rabbit during asphyxic gasping.

Unitary activity was recorded from 44 recurrent laryngeal nerve (RLN) and 18 external laryngeal nerve (ELN) motoneurones in anaesthetized, paralysed rabbits during the development of periods of severe asphyxia caused by temporarily interrupting artificial ventilation. Ventilation was only recommenced when a simultaneously recorded phrenic neurogram showed the animal had attempted several gasps. Inspiratory-phased fibres (17 RLN, 14 ELN) displayed brief high frequency discharges with each gasp. Some inspiratory-phased ELN motoneurones (6/14) also displayed longer tonic discharges after each of

[Phonomicrosurgery - a retrospective analysis of 400 cases].

Introduction Voice disorders caused by pseudotumors of the vocal folds or paralysis of the vocal folds with incomplete glottis closure frequently require phonomicrosurgery. These interventions were analyzed with regard to quality of voice after surgery and safety of the intervention. Methods Retrospective analysis of 400 consecutive phonomicrosurgery interventions. The following parameters were collected: distribution of pathologies of the vocal folds, rating of the voice quality by both the surgeon (RBH-system) and patient and videolaryngstroboscopy six weeks after the intervention compared t

Can we improve outcome of congenital diaphragmatic hernia?

This review gives an overview of the disease spectrum of congenital diaphragmatic hernia (CDH). Etiological factors, prenatal predictors of survival, new treatment strategies and long-term morbidity are described. Early recognition of problems and improvement of treatment strategies in CDH patients may increase survival and prevent secondary morbidity. Multidisciplinary healthcare is necessary to improve healthcare for CDH patients. Absence of international therapy guidelines, lack of evidence of many therapeutic modalities and the relative low number of CDH patients calls for cooperation betw

Impact of new treatments for respiratory failure on outcome of infants with congenital diaphragmatic hernia.

UNLABELLED: Term and near-term newborn infants with congenital diaphragmatic hernia (CDH), symptomatic in the first 24 h of life or diagnosed antenatally, without other significant malformations were treated at our hospital with high-frequency oscillatory ventilation (HFOV) as a primary modality of ventilation and elective delay in surgical repair after a period of stabilisation. When unresponsive to HFOV, infants were treated with surfactant, inhaled nitric oxide (iNO) and extracorporeal membrane oxygenation (ECMO) to achieve pre-operative stabilisation. From October 1994 to August 1998, 28 n

Predicting mortality in low-birth-weight infants with pulmonary interstitial emphysema.

Due to the high mortality associated with pulmonary interstitial emphysema in the low-birth-weight infant, a method was developed to predict the infants most at risk for death. This information will allow the discriminant selection of patients for future trials of an alternative method of mechanical ventilation, high-frequency ventilation. During a 3-year-period (July 1, 1979 through June 30, 1982), 70 infants were retrospectively analyzed to determine the clinical parameters important in predicting mortality. The infants at highest risk for death included those of younger gestational age, tho…

Panting in the emu causes arterial hypoxemia.

The purpose of this study was to determine the effect of heavy thermal panting on arterial oxygen (PaO2) and carbon dioxide (PaCO2) tension in emus. The birds showed no significant change in body temperature during a 3-4 h heat stress caused by increasing ambient air temperature from 21 to 46 degrees C. However, the emus increased their respiratory frequency 10-fold (from 5.3 to 52.9 breaths X min-1). The high respiratory frequency resulted in a slight but significant decrease in PaCO2 (from 33.5 to 29.8 mm Hg), coupled with a slight increase in pH (from 7.449 to 7.469). Paradoxically, these c

Frequency dependence of capnography in anesthetized rabbits.

Aspirative capnography may be of help to diagnose early childhood asthma, but clinical usefulness in young children is limited by the relatively high respiratory rate. This study aimed to characterize the [Formula: see text] time course during airway constriction in 8 anesthetized rabbits, artificially ventilated at 30, 60 and 80breaths/min. Methacholine was inhaled to double the respiratory resistance measured at 8Hz by the forced oscillation technique. The capnogram shape changed in response to both methacholine and ventilatory frequency. Slope of phase II, the peak of first-order time deriv

Evaluation of the new modified apnea test in confirmation of brain death.

BACKGROUND: Apnea testing is mandatory to confirm brain death; however, it is unsafe for patients who have substantial hypoxemia without ventilator support. We used a new modified apnea test without the need to disconnect the patient from the ventilator in the present study and compared the outcomes and complications of the new method to the widely used old method. MATERIALS AND METHODS: The current study was conducted on people suspected of having brain death. Both the old and new apnea tests were carried out on the same individual. In the new modified method, instead of hyperventilating and

Lung volume reduction surgery in lieu of pneumonectomy in an infant with severe unilateral pulmonary interstitial emphysema.

A male infant with a prenatal diagnosis (at 20 weeks' gestation) of cystic adenomatoid malformation was delivered after 38 weeks' gestation (birth weight, 3 kg) and admitted to the neonatal intensive care unit. During the first few days of life, he developed mild respiratory distress; a chest radiograph and computed tomography scan showed multiple cystic areas in the left lower lobe with hyperinflation and herniation of the upper lobe across the midline. At 3 weeks of age, a left lower lobectomy was performed for presumed cystic malformation. To our surprise the pathology reports revealed pulm
