Primary hyperparathyroidism in the seventies. A decade of change?

In an effort to identify new trends in the presentation and treatment of primary hyperparathyroidism, 66 patients treated since 1975 were compared with 100 patients diagnosed and treated from 1948 to 1970. Despite widespread use of multichannel analyzers, the late patients had an insignificant increase in diagnosis while asymptomatic (18 percent versus 9 percent in the early group). Hypertension was the most common presenting complaint in patients seen since 1975, compared with renal disease in patients seen before 1970. Findings of diffuse hyperplasia were more common in the late patients (17…

The effect of hyperparathyroidism on the course of rheumatoid arthritis.

Rheumatoid arthritis and primary hyperparathyroidism have been diagnosed in three patients. This is likely to represent the coincidence of two common diseases but the clinical features are discussed with particular reference to the presence and withdrawal of raised levels of circulating parathyroid hormone (PTH). It is argued that raised levels of PTH potentiate the effects of the rheumatoid disease on bone and joints. Other musculoskeletal features of hyperparathyroidism and the actions of PTH on bone are reviewed.

[[Clodronate therapy of hypercalcemia caused by hyperparathyroidism].

The diphosphonates, powerful agents which inhibit osteolysis, have been used parenterally with success to treat malignant hypercalcemia. 4 cases of anatomically confirmed primary hyperparathyroidism (age of patients 58-78 years) were admitted with a serum calcium of over 4 mmol/l in one case, over 3,5 mmol/l in a second case and over 3 mmol/l in the other two. The patients received an infusion of isotonic saline and dichloromethylene-diphosphonate (clodronate) parenterally in a dose of 500 mg a day. Serum calcium began to fall in all four patients 48 hours after the treatment was started. Intr…

Hyperparathyroidism: experiences with treated and untreated patients.

During a seven-year period, after the introduction of parathyroid hormone assays, 113 patients with primary hyperparathyroidism were seen at an Australian teaching hospital. In 68, parathyroidectomy was followed by normalization of serum calcium levels. Fifteen patients (six referred from other units) remained hypercalcaemic after their initial neck exploration. Thirty patients with biochemical changes indicative of primary hyperparathyroidism were not referred for surgery, mainly because of old age or lack of symptoms. Those patients who were not submitted to surgery, and those who had underg…

Management of acute hyperparathyroidism in a community hospital.

Primary hyperparathyroidism is a common entity that routinely lends itself to prompt surgical cure. Infrequently, multiple surgical explorations are required to effect a cure. A case is reported of a 27-year-old man with hypercalcemia and nephrocalcinosis. The initial surgical exploration failed to identify the abnormal parathyroid tissue. Hypercalcemia necessitated rigorous antihypercalcemic therapy; three subsequent operations, including a mediastinal exploration, were unsuccessful. On the fifth exploration, which took barely an hour, a 35-g adenoma was removed from the left superior posteri…

Primary hyperparathyroidism in a patient with Conn's syndrome.

A 61-year-old patient with treated Conn's syndrome due to multiple bilateral adrenocortical adenomata developed primary hyperparathyroidism. At operation, a parathyroid adenoma and a thyroid colloid nodule were found. Although adrenocortical adenomata and hyperplasia are common findings in multiple endocrine adenopathy type I, they are only rarely functional. Hypertension is often associated with primary hyperparathyroidism, but the relationship is usually unexplained.

Hyperparathyroidism and pancreatitis. Chance or a causal association?

Of 1,153 patients with surgically confirmed primary hyperparathyroidism operated on at the Mayo Clinic between 1950 and 1975, only 17 (1.5%) had coexisting or prior pancreatitis. This frequency of association approximates the reported incidence of pancreatitis among general hospital patient populations. Other factors of possible etiologic significance in pancreatitis, such as gallstones or alcohol abuse, were present in 11 of the 17 patients. Cure of the hyperparathyroidism was usually not associated with amelioration of symptoms due to pancreatitis. A review of the available data, including e…

Primary hyperparathyroidism: the case for conservative surgery.

Analysis of a series of 205 surgically treated cases of primary hyperparathyroidism was carried out to assess the long-term results of the conservative approach of selectively removing the adenoma only. If primary hyperparathyroidism is due to multiple gland involvement in one-third to one-half of patients, an appreciable number of patients with recurrent disease should have been encountered during this study of the results of conservative no instances of permanent tetany, supports the conservative approach to the treatment when only a single enlarged gland is encountered. Subtotal parathyroid…

The diagnosis and management of hyperparathyroidism during pregnancy.

Hyperparathyroidism during pregnancy is rare and very few cases have undergone parathyroidectomy during the course of gestation. In reviewing the literature it is apparent that surgery during the second trimester should be the treatment of choice. The complication rate of untreated cases may reach 80% and include spontaneous abortion, fetal death and neonatal tetany. The paper is illustrated with appropriate cases.

[A case of primary hyperparathyroidism. Clinical contribution].

The authors discusses some clinical, laboratory and instrumental problems of a reported case of primary hyperparathyroidism caused by adenoma from the point of view of medical literature. Nethertheless the lack of the specificity of the clinical manifestations and the slowly progressive evolution of this disease, primary hyperparatiroidism must be suspected in the presence of his most common multifarious complications. The finding of hypercalcemia in these conditions and from screening procedures requests parathyroid functional tests and exclusion of other causes of which chiefly pseudohyperpa…

[Bone biopsy in the diagnosis of primary hyperparathyroidism].

In case when the diagnosis of primary hyperparathyroidism is unclear or difficult to establish the authors recommend to use the method of bone biopsy. The histological picture of the bone in hyperparathyroidism is described. It has been found that the bone biopsy ensures the differenciation of hyperparathyroidism from other forms of osteodistrophies, esteodisplasies and myeloma.

Serum urate in subjects with hypercalcaemic hyperparathyroidism.

A positive correlation was found between serum urate and elevated serum calcium in patients with hypercalcaemic primary hyperparathyroidism. No such correlation was detected in normocalcaemic controls, matched with respect to age and sex. Neither was such a correlation confirmed either in subjects with normalized serum calcium levels after extirpation of parathyroid adenomata, or in subjects with hypercalcaemia due to other conditions than primary hyperparathyroidism, such as various malignancies, sarcoidosis and hyperthyroidism. The positive correlation between elevated serum calcium and seru…
