PURPOSE: Febuxostat, a nonpurine selective inhibitor of xanthine oxidase has elucidated an effective urate-lowering in adults. However, data on febuxostat utility in children is limited. The present study summarizes the current knowledge on the efficacy and safety of febuxostat in children with hyperuricemia.
METHODS: We searched PubMed, Google Scholar, Embase, and Web of Science from inception to September 2024 for studies assessing febuxostat or comparing febuxostat with allopurinol in children and adolescents with hyperuricemia with diverse etiologies. Randomized controlled trials, prospec…

Psoriasis is a chronic inflammatory disease of multifactorial etiology influenced by genetic, immunological, and environmental factors. We report the case of a patient with psoriasis for more than 25 years who developed hyperuricemia and chronic tophaceous gout with unusual appearance. In psoriasis, hyperuricemia may occur by increased epidermal cell turnover, which accelerates purine metabolism and has uric acid as the product of its catabolism. The association of psoriasis with hyperuricemia can trigger the onset of gouty arthritis, and pose a greater risk of developing other inflammatory co…

Hyperuricemia is a common condition, and in a subset of patients leads to gout, the most common inflammatory arthritis. Osteoarthritis is the most common form of arthritis overall, and gout and osteoarthritis frequently coexist in the same patient. However, the relationship between the two remains poorly defined. More particularly, the impact of osteoarthritis on the development of gout, and the impact of gout on the development of osteoarthritis, remain to be determined. Additionally, whether hyperuricemia mediates osteoarthritis in the absence of gout is uncertain. Here, we review the eviden…

OBJECTIVES: Ultrasound lesions of gout have been described in people with asymptomatic hyperuricemia. However, the anatomical sites and ultrasound lesions most frequently involved in asymptomatic hyperuricemia have not yet been established. This systematic review and meta-analysis aimed to determine the prevalence of the Outcome Measures in Rheumatology (OMERACT) elementary ultrasound lesions of gout (double contour, aggregates, tophus, erosion) at various sites in people with asymptomatic hyperuricemia and to determine which sites and lesions discriminate from people with normouricemia.

ObjectiveHyperuricemia is a common metabolic disorder linked to systemic inflammation; however, its impact on perioperative hematological and inflammatory markers in patients residing in high-altitude regions who have undergone anterior cruciate ligament reconstruction remains unclear. We hypothesized that hyperuricemia is associated with a heightened perioperative inflammatory response.MethodsThis retrospective, cross-sectional study included 83 long-term high-altitude (≥2500 m) residents who underwent anterior cruciate ligament reconstruction between 2019 and 2025. Patients were divided into…

PURPOSE OF REVIEW: Impaired renal uric acid excretion is the major mechanism of hyperuricemia in patients with primary gout. This review highlights recent advances in the knowledge of normal mechanisms of renal uric acid handling and derangement of these mechanisms in uric acid underexcretion.
RECENT FINDINGS: The discovery of URAT1 has facilitated identification of other molecules potentially involved in uric acid transport in the renal tubules. Some of these molecules show gender differential expression in animal experiments. Sodium-dependent monocarboxylate cotransporters have been shown t…

Hyperuricemia, the biochemical precursor to gout, is usually defined as the theoretical limit of solubility of serum uric acid (UA) of >7.0 mg/dL. Hyperuricemia is closely associated with hypertension, diabetes mellitus, and dyslipidemia, which are well known to be related to risk factors for coronary artery disease (CAD). Furthermore, hyperuricemia has been associated with increased mortality in both the general population and individuals with cardiovascular diseases. Elevated UA in patients with CAD is accompanied by surrogate markers of atherosclerosis, including C-reactive protein, platele…

Erectile dysfunction, a precursor to cardiovascular diseases, is linked to metabolic disorders like gout. However, whether hyperuricemia plays a direct causative role in erectile dysfunction is unclear. Here we show that clinical data from young patients (24-49 years) reveal an over 2.5-fold increased erectile dysfunction risk with elevated serum uric acid. In spontaneous hyperuricemia rats with Urate oxidase gene knockout, hyperuricemia impairs erectile function early (20 weeks) without other metabolic comorbidities. Mechanistically, uric acid enters corpus cavernosum smooth muscle cells, int…

Hyperuricemia is a metabolic disease caused by purine nucleotide metabolism disorder. The prevalence of hyperuricemia is increasing worldwide, with a growing trend in the younger populations. Although numerous studies have indicated that hyperuricemia may be an independent risk factor for insulin resistance, the causal relationship between the two is controversial. There are few reviews, however, focusing on the relationship between uric acid (UA) and insulin resistance from experimental studies. In this review, we summarized the experimental models related to soluble UA-induced insulin resist…

Asymptomatic metatarsophalangeal joints were aspirated in a group of patients with gout, in 2 control groups with hyperuricemia, and in 1 normouricemic control group. Extracellular urate crystals were present in 70% of gout patients, in 1 of 19 patients with asymptomatic hyperuricemia, and in 2 of 9 patients with renal failure and hyperuricemia but no history of joint disease. Crystals were not found in the 10 normouricemic patients who had other types of arthritis. The presence of crystals in the subjects with gout was not correlated with a history of podagra, duration of gout, presence of to…

Background/Objectives: Hyperuricemia is highly prevalent in chronic kidney disease (CKD). It has been linked to increased cardiovascular and mortality risk. However, its independent prognostic significance and dose-response relationship with adverse outcomes remain incompletely understood. Methods: This retrospective cohort study included 794 patients with CKD. Hyperuricemia was defined using sex-specific thresholds. The association between hyperuricemia, cardiac events and mortality was tested using a multivariable logistic regression test. Serum uric acid was analyzed as both a categorical a…

BACKGROUND: Hyperuricemia and gout are associated with an increased risk of cardiovascular disease (CVD). It is unknown whether treating hyperuricemia with xanthine oxidase inhibitors (XOIs), including allopurinol and febuxostat, modifies cardiovascular risks.
METHODS: We used US insurance claims data to conduct a cohort study among gout patients, comparing XOI initiators with non-users with hyperuricemia defined as serum uric acid level ≥6.8 mg/dL. We calculated incidence rates of a composite nonfatal cardiovascular outcome that included myocardial infarction, coronary revascularization, str…

Recently atherosclerotic diseases, such as coronary heart disease and cerebrovascular disease have been considered as an important complication of hyperuricemia and gout. However, it is still controversial whether or not hyperuricemia is an independent risk factor of atherosclerotic diseases. On the other hand, several risk factors for coronary heart disease, for example hyperlipidemia and hypertension, are frequently observed in the patients with gout. Atherosclerosis in relation to hyperuricemia was discussed in view of definite and probable risk factors.

BACKGROUND: Hyperuricemia is a risk factor for the onset of chronic kidney disease (CKD) and is significantly associated with the progression of CKD. However, there is no sufficient evidence by interventional research supporting a cause-effect relationship. Hyperuricemic patients without gouty arthritis, whose serum urate (SUA) concentration is ≥8.0 mg/dL and who have a complication, are treated by pharmacotherapy in addition to lifestyle guidance. Nevertheless, there is no evidence that rationalizes pharmacotherapy for patients with hyperuricemia who have no complication and whose SUA concent…

Gout is a chronic metabolic disease caused by disturbance of purine metabolism that leads to hyperuricemia. Hyperuricemia prevalence after renal transplant is reported as 19% to 84% in different studies. Tophaceous gout in renal transplant recipients is a consequence of increased hyperuricemia. Although tophus formation in skin and soft tissues is an indicator of chronic gout (also referred to as tophaceous gout), tophi may be the first sign of gout. In this study, we report a case of a 62-year-old male renal transplant recipient who had tophi as the first clinical sign of gout. After confirmi…

The serum levels of uric acid, hypoxanthine and xanthine tended to increase with the decrease of renal function. This mechanism was thought to be the decreased excretion of these materials from the kidney. More than ninety percent of the patients with renal insufficiency (Ccr < or = 30 ml/min) showed hyperuricemia. In general, the gouty arthritis was reported to be uncommon in the patients with secondary hyperuricemia due to renal insufficiency. However, the frequency of gouty arthritis was reported to be high in the patients with polycystic disease and lead nephropathy. The therapeutic standa…

Hyperuricemia and its clinical manifestations, such as gout, are frequently encountered in patients with chronic renal disease and renal transplants. Usual treatments are either contraindicated or are prone to side-effects in these patients. Presently, there is no clear concensus regarding therapeutic guidelines of hyperuricemia and gout in this subset of patients. Steroids remain the more effective and safer treatment in absence of superimposed infection. Though allopurinol is not recommended for prophylaxis of hyperuricemia, this drug is still the cornerstone in the treatment of patients suf…
