The genetics of autoimmune polyendocrine syndrome type II.

A series of autoimmune disorders, often Addison's disease, type 1 diabetes mellitus, and thyroid autoimmunity, frequently occurs together in patients with the autoimmune polyendocrine syndrome type II (APS-II). The highest risk HLA genotype for Addison's disease, either as a single disease or in APS-II patients, consists of the genotype DR3/4, DQ2/DQ8 with DRB1*0404. As many as 30% of patients with Addison's disease have this genotype versus less than 0.5% of controls. An additional and important associated locus within the HLA region is the class I related gene, MIC-A. Patients who develop Ad

CTLA-4 gene polymorphism confers susceptibility to primary biliary cirrhosis.

BACKGROUND/AIM: Primary biliary cirrhosis (PBC) is an autoimmune cholestatic liver disease thought to develop through a complex interaction of genetic and environmental factors. It is characterised by T-cell-mediated non-suppurative destructive cholangitis. We have studied the polymorphic cytotoxic T lymphocyte-associated antigen-4 (CTLA-4) gene, which encodes a molecule that is a vital negative regulator of T-cell activation, as a candidate susceptibility locus for PBC. This gene on chromosome 2q33 (designated IDDM12) is associated with susceptibility to both type 1 diabetes and autoimmune th

Preclinical Pharmacokinetic-Pharmacodynamic Modeling of Antibody Nanoconjugates for Breast Cancer Treatment.

Over the past 5 years, the number of antibody-drug conjugates (ADCs) entering the oncology market has surged. By contrast, the clinical use of immunoliposomes-nanoparticles that also employ antibodies as targeting agents (i.e., antibody-nanoconjugates, ANCs)-has yet to receive approval. One possible reason is the limited understanding of the relationship between ANC exposure and efficacy. To address this gap, we developed a pharmacokinetic/pharmacodynamic (PK/PD) model for a docetaxel-trastuzumab ANC. PK and PD studies were conducted in MDA-MB-231 tumor-bearing mice, divided into three treatme

Emerging role of antibody-drug conjugates in gastrointestinal malignancies.

Gastrointestinal (GI) malignancies remain a major cause of global cancer-related mortality, especially in the setting of advanced or metastatic disease. Antibody-drug conjugates (ADCs) have emerged as a promising class of targeted therapeutics that combine the specificity of monoclonal antibodies with the potency of cytotoxic payloads. Through advances in antibody engineering, linker chemistry, and payload design, modern ADCs have demonstrated enhanced efficacy with manageable toxicity. Therapeutic agents such as trastuzumab deruxtecan and disitamab vedotin are currently approved ADCs in HER2-

Development of a DUX4-targeting antibody oligonucleotide conjugate as a therapy for FSHD.

Facioscapulohumeral muscular dystrophy (FSHD) is an autosomal dominant muscular disease in which genetic mutations activate DUX4 expression in skeletal muscle. Currently, there are no approved therapies for FSHD. We developed Delpacibart braxlosiran (del-brax, also known as AOC 1020), an antibody oligonucleotide conjugate (AOC), for the treatment of FSHD that is designed to specifically target and reduce DUX4 mRNA in skeletal muscle. AOC 1020 is composed of DUX4 mRNA-targeting small interfering RNA (siRNA), siDUX4.6, conjugated to a human transferrin receptor 1 (TfR1)-targeting monoclonal anti

Sequential Use of Trastuzumab Deruxtecan and Sacituzumab Govitecan in Patients with Breast Cancer: A Pharmacological Approach to Support the Clinical Rationale.

Antibody-drug conjugates (ADCs) represent a major advance in breast cancer therapy, with trastuzumab deruxtecan and sacituzumab govitecan emerging as leading agents targeting distinct tumor antigens and employing different linker-payload designs. Trastuzumab deruxtecan is a second-generation HER2-directed ADC composed of trastuzumab linked via a cleavable tetrapeptide linker to the camptothecin derivative exatecan, a highly potent topoisomerase I inhibitor. Its high drug-to-antibody ratio (DAR 8:1), membrane-permeable payload, and efficient lysosomal release confer strong antitumor activity, i

A Theranostic Study of 177Lu-Labeled Anti-ADAM9 Antibody for the Treatment of Triple-Negative Breast Cancer.

Triple-negative breast cancer (TNBC) is highly aggressive with limited diagnostic and therapeutic options. ADAM9 is overexpressed in TNBC and may serve as a theranostic target. Therefore, this study aims to develop [177Lu]Lu-DOTA-48HZ810, an ADAM9-targeted probe, and evaluate its targeting specificity, imaging capability, efficacy, and safety in TNBC models. ADAM9 expression in TNBC cells was assessed by flow cytometry, and the probe was constructed by conjugating 177Lu to the ADAM9-specific antibody 48HZ810 via DOTA, after which binding specificity, affinity, and internalization were evaluate

Dynamic monitoring of antibody drug conjugates targeting TROP2 or HER2 in breast cancer using circulating tumor cells.

Antibody-drug conjugates (ADCs) target surface proteins on cancer cells, leading to internalization and delivery of a drug payload, thereby enhancing selectivity and minimizing toxicity. ADCs against TROP2 (Sacituzumab govitecan) or HER2 (T-DXd) have demonstrated efficacy in metastatic breast cancer, yet paradoxically, outside of HER2-amplified breast cancers, expression levels of these breast cancer-enriched epitopes in tumor biopsies have not been strongly correlated with clinical response. We undertook serial quantitative imaging of circulating tumor cells (CTCs) in a prospective cohort of

Targeting HER2 in urothelial carcinoma: from pathway inhibition to antibody-drug conjugates.

Urothelial carcinoma (UC) is a biologically and clinically heterogeneous malignancy with persistently poor outcomes in locally advanced or metastatic disease. Although platinum-based chemotherapy and immune checkpoint inhibitors have improved survival, durable disease control remains limited, and therapeutic resistance is common. Antibody-drug conjugates (ADCs) targeting Nectin-4 and Trop-2 have further expanded the therapeutic landscape, yet the need for additional biomarker-driven strategies remains pressing. Human epidermal growth factor receptor 2 (HER2), encoded by ERBB2, has emerged as a

Meflin is a druggable target using antibody-drug conjugates in progressive osteosarcoma.

The 5-year overall survival rate of patients with recurrent osteosarcoma (OS) has remained less than 30%, which is attributed to the persistence and growth of OS cells with high chemoresistance, tumorigenicity and metastatic potential. However, the characteristics of recurrent OS cells and therapeutic strategies remain unexplored. In the present study, we found that Meflin, a membrane protein previously identified as a specific marker of mesenchymal stromal/stem cells, was highly expressed in tumor cells of patients with progressive OS. Interestingly, OS cells resistant to doxorubicin showed u

Antibody polymer drug conjugates with increased drug to antibody ratio: CD38-targeting nanomedicines for innovative therapy of relapsed lymphomas.

The current frontline therapy for B-cell non-Hodgkin lymphomas (B-NHL), the most frequent hematologic malignancy in the Western hemisphere, is based on immunochemotherapy (ICT), i.e., a combination of genotoxic cytostatics and the anti-CD20 monoclonal antibody (mAb) rituximab. The treatment of relapsed or refractory (R/R) B-NHL remains an unmet medical need. Here, we developed and preclinically characterized a tailored hybrid mAb-polymer-drug conjugate (APDC) composed of the anti-CD38 mAb daratumumab (clinically approved for multiple myeloma therapy) and biocompatible N-(2-hydroxypropyl)methac

Evaluating risk factors for Trastuzumab-Deruxtecan Pneumonitis in patients with metastatic breast cancer.

BACKGROUND: Trastuzumab deruxtecan (T-DXd) is FDA-approved for treatment of patients with HER2 positive and HER2-low metastatic breast cancer. Currently, there is limited understanding of pre-treatment risk factors for pneumonitis associated with T-DXd. METHODS: Consecutive breast cancer patients who received at least one dose of T-DXd at a single academic cancer study between January 1, 2019, and February 20, 2024, were identified for analysis. Pneumonitis was documented by the treating oncologist at the time of toxicity and retrospectively independently confirmed by a member of the study te

CD38-targeted antibody-polymer drug conjugates for enhanced treatment of multiple myeloma.

Multiple myeloma (MM) remains a formidable disease, especially in relapsed or refractory cases when there are limited treatment options. In this study, we introduce two polymer-antibody drug conjugates (pADCs), ISA-P-EPI (U6244-021) and DARA-P-EPI (U6244-031), which contain semitelechelic N-(2-hydroxypropyl)methacrylamide (HPMA) copolymer-epirubicin (EPI) conjugate attached to CD38-targeting antibodies Isatuximab (ISA) and Daratumumab (DARA). These pADCs enhance therapeutic efficacy by combining the specificity of ISA and DARA with the cytotoxic potency of EPI while preserving antibody functio

Trop2-Targeted Molecular Imaging in Solid Tumors: Current Advances and Future Outlook.

Trophoblast cell surface antigen 2 (Trop2), a transmembrane glycoprotein, plays a dual role in physiological and pathological processes. In healthy tissues, Trop2 facilitates development and orchestrates intracellular calcium signaling. However, its overexpression in numerous solid tumors shifts its function toward driving cell proliferation and metastasis, thus leading to a poor prognosis. The clinical relevance of Trop2 is underscored by its utility as both a biomarker for diagnostic imaging and a target for therapy. Notably, the U.S. Food and Drug Administration (FDA) has approved sacituzum
