Evaluating living kidney donors: relationship types, psychosocial criteria, and consent processes at US transplant programs.

We conducted a survey of 132 US kidney transplant programs to examine how they evaluate and select potential living kidney donors, focusing … the donor evaluation is presumed in 57% and 76% of programs, respectively. The use of 13 different informed consent elements varied from 65% (alternative donation procedures) to 86% (description of evaluation, surgery and recuperative period) of programs. Forty-three percent use a 'cooling off' period. Findings demonstrate high variability in current practice reg …

Living kidney donation prior to euthanasia: medical, psychological and ethical considerations.

BACKGROUND: Light chain deposition disease (LCDD) is a monoclonal immunoglobulin deposition disease (MIDD) that is characterized by the deposition of monoclonal light chains in multiple organs, including the kidney. It is a rare disorder caused by an underlying monoclonal plasma cell dyscrasia. LCDD with renal involvement causes proteinuria, which sometimes can lead to nephrotic syndrome. The monoclonal light chains are mostly in the κ form. Treatment of LCDD is the same as that for multiple myeloma (MM); however, some conventional anticancer drugs show substantial toxicity and therefore canno

When Pigs Fly into Medicine: Navigating Ethical Challenges Posed by Animal-Derived Medical Products.

AbstractThis article explores the ethical, religious, and process complexities surrounding the use of animal-derived products in medicine, using the case of a 19-year-old Muslim patient with homozygous familial hypercholesterolemia, receiving lifesaving LDL apheresis with porcine-derived heparin. Despite the critical role of LDL apheresis to manage her high cholesterol levels over the past year, the charge nurse has recently raised concerns owing to the porcine origin of the anticoagulant used in her treatment, recognizing that this may conflict with her religious beliefs. This case raises cri

Governing Uncertainty: An Ethical Framework for Idiopathic Normal Pressure Hydrocephalus.

AbstractIdiopathic normal pressure hydrocephalus presents distinctive ethical challenges, since its treatment requires lifelong device-based therapy in the setting of diagnostic and prognostic uncertainty. Although randomized controlled trials and pooled analyses demonstrate that cerebrospinal fluid shunting can substantially improve gait, and often continence and cognition, outcomes remain variable, and complications requiring revision occur in a significant minority of patients. These realities complicate informed consent, capacity assessment in cognitively vulnerable older adults, and just …

Offshore clinical research as a crisis of the ethics of knowledge.

The globalisation of clinical research has produced a consolidated system in which trials are progressively relocated to low- and middle-income countries, driven by lower costs, easier recruitment, and reduced regulatory oversight. This structure raises questions that extend beyond participant protection to the epistemic conditions under which biomedical knowledge is produced. The central argument of this paper is that ethical violations in offshore clinical research are not merely parallel to epistemic distortions – they actively generate them through identifiable causal mechanisms. Therapeut

Ethics in Telepsychiatry.

This article reviews contemporary issues in telepsychiatry and telepsychotherapy. The authors examine ethics within a larger social context, particularly access to care and cost of care. They consider the evolving scientific validation of telepsychiatry and the ethical significance of telepsychiatry in various practice settings. They also focus on the doctor-patient relationship, assessments, and provision of psychiatric treatments such as medication management and psychotherapy. The authors consider concerns about malpractice liability and medicolegal constraints on telepsychiatry. They do no

Financial ''risk-sharing'' or refund programs in assisted reproduction: an Ethics Committee opinion.

Financial "risk-sharing" fee structures in assisted reproduction programs charge patients a higher initial fee that includes multiple cycles but offers a partial or complete refund if treatment fails. This opinion of the American Society for Reproductive Medicine Ethics Committee analyzes the ethical issues raised by these fee structures, including patient selection criteria, conflicts of interest, success rate transparency, and patient-informed consent. This document replaces the document of the same name, last published in 2016.

Evaluation of medical ethics competencies in rheumatology: local experience during national accreditation process.

INTRODUCTION: Rheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process (ICP). The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations. METHODS: The station was validated and represented a medical encounter in which the rheumatologist informed a patient with…

Building Norms for Organ Donation in China: Pitfalls and Challenges.

In most, if not all, jurisdictions with active organ transplantation programs, there is a persistent desire to increase donation rates because the demand for transplantable organs exceeds the supply. China, in particular, faces an extraordinary gap between the number of organs donated by deceased donors and the number of people seeking one or more transplants. China might look to Western countries with higher donation rates to determine how best to introduce Western practices into the Chinese system. In attempting to increase its organ donation rate, China must not only ensure that its organ d

Assessment of mental capacity to consent to treatment in anorexia nervosa: A comparison of clinical judgment and MacCAT-T and consequences for clinical practice.

Informed consent requires adequate mental capacity to consent to treatment. Mental capacity (MC) to consent to treatment refers to the ability to make medical decisions. MC is assessed in a general psychiatric interview, but this clinical assessment is known to overestimate mental capacity in patients and the inter rater reliability is low. The MacArthur Competence Assessment Tool for Treatment (MacCAT-T) has emerged as the gold standard to assess mental capacity to consent to treatment. The MacCAT-T is a semi-structured interview designed to aid clinicians in this assessment and has shown goo…

An Evidence-Based, Community-Engaged Approach to Develop an Interactive Deliberation Tool for Pediatric Neuromuscular Trials.

Duchenne/Becker muscular dystrophy (DBMD) and spinal muscular atrophy (SMA) are rare neuromuscular disorders that present challenges to therapeutic and clinical trial decision making. We developed an interactive, evidence-based online tool designed to encourage thoughtful deliberation of the pros and cons of trial participation and to inform meaningful discussions with healthcare providers. Prior research demonstrates the importance of tool availability at the time each family is considering trial participation, which may be prior to the informed consent process. The tool is intended to be eas…

Should gratitude be a requirement for access to live organ donation?

Gratitude is both expected and problematic in live organ donation. Are there grounds to require it, and to forbid access to live donor transplantation to a recipient who fails to signal that he feels any form of gratitude? Recipient gratitude is not currently required for organ donation, but it is expected and may be a moral requirement. Despite this, we argue that making it a condition for live organ transplantation would be unjustified. It would constitute a problematic and disproportionate punishment for perceived immoral behaviour on the part of the recipient. It would also bar the donor f

Living Organ Donation by Minors: An Analysis of the Regulations in European Union Member States.

Living organ donation (LD) is an increasingly established practice. Whereas in the United States and Canada LD by minors has occasionally been reported, LD by minors seems to be largely absent in the European Union (EU). It is currently unclear whether this is the result of a different legal approach. This study is the first to systematically analyze the regulations of EU member states, Norway, and Iceland toward LD by minors. Relevant regulations were identified by searching government websites, translated, compared, and sent for verification to national legal experts. We identified five coun

Consent, common adverse events, and post-adverse event actions in endoscopy.

Endoscopy constitutes a wide range of procedures with many indications. Esophagogastroduodenoscopy, colonoscopy, endoscopic retrograde cholangiopancreatography, endoscopic ultrasonography, and enteroscopy comprise the most commonly performed procedures. These examinations all carry risk to the patient, and incumbent in this is some legal risk with regard to how the procedure is conducted, decisions made based on the intraprocedure findings, and the postprocedure results, in addition to events that occur following the procedure. This article provides an overview of consent and complications of
