CLINICAL CONDITION: A 55-year-old man was admitted with inferior-wall ST-segment elevation myocardial infarction. He was found to have multivessel disease with a culprit lesion not amenable for revascularization, new ischemic mitral regurgitation, and cardiogenic shock, prompting the initiation of mechanical circulatory support (MCS). His course was complicated by conduction abnormalities.
KEY QUESTIONS: Some questions for consideration were: 1) Which criteria guide the choice of MCS in patients with complex cardiovascular anatomy and cardiogenic shock? 2) Is coronary artery bypass grafting w

RATIONALE: Acute mitral regurgitation (MR) secondary to papillary muscle rupture is a rare but often life-threatening mechanical complication post-acute myocardial infarction (MI). The use of peripheral venoarterial extracorporeal membrane oxygenation (VA-ECMO) as a bridge to mitral valve replacement surgery may improve outcome of such patients.
PATIENT CONCERNS: We reported the case of a 70-year old woman with past history who presented to the emergency department at People's Hospital of Rizhao with "a 5-day history of chest distress." She developed refractory cardiogenic shock, severe pulmo

Myocarditis is challenging to diagnose because of its various clinical manifestations. Fulminant myocarditis (FM) is a severe type of myocarditis characterized by heart failure, malignant arrhythmia, cardiogenic shock, and cardiac arrest. Its early diagnosis and timely treatment are crucial for a positive long-term prognosis. Here we report a case of 42-year-old woman who presented with fever, chest pain, and cardiogenic shock. An initial examination showed increased myocardial enzyme levels and diffuse ST-segment elevation. Urgent coronary angiography excluded coronary artery stenosis. Echoca

Myocardial lymphoma can be seen in the setting of primary or secondary involvement, more common in later. Both primary and secondary involvement is usually seen in the setting of diffuse large B cell lymphoma. We describe a case of a 47-year-old man in cardiogenic shock due to refractory ventricular tachycardia (VT) necessitating intubation, extracorporeal membrane oxygenation (ECMO) and an intra-aortic balloon pump with diffuse thickening of the left ventricular myocardium on CTA chest and subtle infiltrative ill-defined perirenal hypodensities with renal enlargement on CT of abdomen and pelv

OBJECTIVE: Acute kidney injury (AKI) is a major complication after cardiovascular surgery. The unclear etiology of this highly complex event challenges definition, diagnosis and prediction of AKI, and hence hampers adequate patient management. Identification of associated risk factors have the potential to overcome this limitation.
METHODS: This retrospective study comprised 3574 patients who underwent cardiac surgery in a hospital in Germany. The patient cohort was interrogated for risk factors for AKI.
RESULTS: The analysis identified risk factors for AKI development, such as type of surge

BACKGROUND Venom related fulminant myocarditis is uncommon. The clinical course varies, and histopathology is usually unclear, however, refractory cardiogenic shock is rare. CASE REPORT We reported a case of a 36-year-old female who developed fever and cardiogenic shock 3 days after a bee sting. Cardiac angiography showed patent coronary arteries and severely compromised left ventricular function. Her hemodynamics remained unstable under high dose inotropic agents and intra-aortic balloon pump support. In-hospital cardiac arrest occurred 4 hours after admission and she received extracorporeal

Acute left ventricular (LV) failure has been reported after surgical closure of atrial septal defect (ASD) in adult patients. We report acute LV failure in a 56 year old gentleman following coronary artery bypass grafting (CABG) and surgical closure of ASD. Transesophageal echocardiography examination of the patient following closure of ASD and CABG showed a residual ASD and a shunt (Qp :Qs = 1.5). The residual ASD was closed after re-institution of cardiopulmonary bypass (CPB) under cardioplegic cardiac arrest. However, the patient did not tolerate closure of the residual ASD. The CPB was re-

Acute kidney injury (AKI) after a coronary intervention is common in patients with ST-segment elevation myocardial infarction (STEMI) and is associated with significant morbidity and mortality. Several scores have been developed to predict post-procedural AKI over the years. However, the AKI definitions have also evolved, which causes the definitions used in the past to be obsolete. We aimed to develop a prediction score for AKI in patients with STEMI requiring emergency primary percutaneous coronary intervention (pPCI). This study was based on a retrospective cohort of Thai patients with STEM

Influenza is a common viral infection associated with seasonal outbreaks; however, fulminant myocarditis secondary to influenza Type B represents a rare and life-threatening complication in the pediatric population, with poorly understood pathophysiology. Only a limited number of cases have been reported worldwide, and to our knowledge, this is the first published case in Mexico. We report the case of a previously healthy 13-year-old girl who presented with a five-day history of nonspecific symptoms and rapidly progressed to cardiogenic shock. Prompt recognition and a multidisciplinary approac

BACKGROUND: Left main coronary artery compression (LMCo) by a dilated pulmonary artery is a rare but potentially fatal complication of pulmonary arterial hypertension (PAH).
CASE: An octogenarian man with asthma and PAH presented with dyspnea and developed pulseless ventricular tachycardia. Electrocardiogram showed ST-segment elevation in leads I, aVL, and V4 to V6. Angiography revealed 99% stenosis of the proximal left main trunk. Intravascular ultrasound (IVUS) showed slit-like compression without plaque. A 3.5- × 15-mm drug-eluting stent was implanted, restoring TIMI flow grade 3. Subseque

The mechanical circulatory support (MCS) for pediatric patients with severe acute heart failure and poor pulmonary conditions is challenging. Herein, we report the first pediatric case of successfully treated by central Y-Y extra-corporeal membrane oxygenation (ECMO). A 10-year-old boy weighing 35 kg with a body surface area of 1.11 m2 was transferred to our institution in cardiac arrest with ongoing cardiopulmonary resuscitation using intra-aortic balloon pump and peripheral ECMO inserted at the previous hospital. Then, MCS system was converted to central ECMO with left heart drainage due to

Heart failure and cardiogenic shock occurred again on the patient who undergone coronary artery bypass grafting, mechanical mitral valve replacement, and tricuspid valve repair surgery for coronary heart disease, mitral regurgitation, and tricuspid valve regurgitation 2 months ago. His heart function could not recover after Intra-Aortic Balloon Pump (IABP) supported for 1 month, and then Veno-Arterial Extracorporeal Membrane Oxygenation (VA-ECMO) was applied. During the VA-ECMO assisted procedure, complex coagulation dysfunction was present, bleeding and thrombosis was coexisting. Treatment wi

A 68-year-old male presented to the emergency department with retrosternal chest pain, presyncope, and then a pulseless electrical activity cardiac arrest. An ECG prior to his arrest revealed ST elevations in leads V1-V3, Q waves in lead V2, and reciprocal ST depressions in the lateral and inferior leads. He received thrombolytic therapy for a presumptive diagnosis of ST elevation myocardial infarction. Return of spontaneous circulation was achieved and he underwent a coronary angiogram. No critical disease was found and his left ventriculogram showed normal contraction. His ongoing metabolic

Cardiogenic shock in the context of acute ST-elevation myocardial infarction (STEMI) remains a challenge to manage and results in significant mortality and morbidity, cardiac arrest in this setting even more so. The increase in myocardial oxygen demand and consumption with the use of inotropes is recognised as increasing mortality. Alternatives include the intra-aortic balloon pump (IABP), which has yet to be shown to improve outcomes, and extracorporeal membrane oxygenation (ECMO), which requires super-specialised techniques not widely available. We report a case of Anterior STEMI from a left

AIMS: Takotsubo syndrome (TTS) is an acute and reversible left ventricular dysfunction and can be complicated by cardiogenic shock (CS). However, few data are available on optimal care in TTS complicated by CS. Aim of this study was to evaluate short- and long-term impact of intra-aortic balloon pumping (IABP) on mortality in this setting.
METHODS AND RESULTS: In a multi-centre, international registry on TTS, 2248 consecutive patients were enrolled from 38 centres from Germany, Italy, and Spain. Of the 2248 patients, 212 (9.4%) experienced CS. Patients with CS had a higher prevalence of diabe…
