Leaks after laparoscopic sleeve gastrectomy: overview of pathogenesis and risk factors.

BACKGROUND: Leak is the second most common cause of death after bariatric surgery. The leak rate after laparoscopic sleeve gastrectomy (LSG) ranges between 1.1 and 5.3 %. The aim of the paper is to provide an overview of the current pathogenic and promoting factors of leakage after LSG on the basis of recent literature review and to report the evidence based preventive measures. METHODS: Risk factors and pathogenesis of leakage after LSG were examined based on an extensive review of literature and evidence based analysis of the most recent published studies using Oxford centre for evidence-ba

Musculoskeletal symptoms and orthopaedic complications in pregnancy: pathophysiology, diagnostic approaches and modern management.

Low back pain is a common musculoskeletal symptom in pregnancy that can present as lumbar pain or pelvic girdle pain, with significant physical and psychosocial implications. Pelvic girdle pain is more prevalent and results in greater disability than lumbar pain. It is possible to distinguish between these two conditions from a detailed history based on the site of the pain, its intensity, disability and pain provocation tests. Management of low back pain in pregnancy is conservative, with physical exercise for lumbar pain and minimising activities that exacerbate pain, analgesics and bed rest

Transmission of pathogenic bacterial organisms in the anesthesia work area.

BACKGROUND: The current prevalence of hospital-acquired infections and evolving amplification of bacterial resistance are major public health concerns. A heightened awareness of intraoperative transmission of potentially pathogenic bacterial organisms may lead to implementation of effective preventative measures. METHODS: Sixty-one operative suites were randomly selected for analysis. Sterile intravenous stopcock sets and two sites on the anesthesia machine were decontaminated and cultured aseptically at baseline and at case completion. The primary outcome was the presence of a positive cultu

Use of BIS monitor in a child with congenital insensitivity to pain with anhidrosis.

We describe a case of a 14-year-old boy with congenital insensitivity to pain and anhidrosis (CIPA) who underwent tarsal tunnel release for tarsal tunnel syndrome. Because of abnormal pain perception, the patient's response to normally painful surgical stimuli is severely impaired and not adequately reflected in a corresponding rise in blood pressure or heart rate. This lack of autonomic feedback to pain stimuli may make it more difficult to assess whether anesthetic depth is adequate to prevent intraoperative awareness and thus to safely conduct anesthesia, especially if muscle paralysis is r…

Practical treatment recommendations for the safe use of anaesthetics.

General anaesthesia is the reversible depression of central nervous system function. There is still no agreement over what constitutes depth of anaesthesia, and the clinical anaesthetist must thus titrate drug input according to clinical signs (heart rate, blood pressure, somatic movement, autonomic responses). The potency of inhalational agents may be expressed in terms of the MAC (minimum alveolar concentration); comparable end-points (including blood concentrations) have been proposed for the intravenous agents. Kinetic infusion regimens can be constructed for the intravenous agents to achi

An Ectopic Tentorial Schwannoma Mimicking Tentorial Meningioma: A Case Report and Literature Review.

Intracranial schwannomas account for approximately 8% of all primary brain tumors, and the majority arise in the cerebellopontine angle. Ectopic schwannomas that originate from non-cranial nerve sites, such as the dura mater or brain parenchyma, are rare, representing less than 1% of all intracranial schwannomas. These lesions often mimic meningiomas on neuroimaging, making preoperative diagnosis challenging. A woman in her 50s was referred to our department after a brain check-up incidentally revealed a mass lesion extending both above and below the tentorium. Neurological examination was unr

Consideration in Platelet Transfusion-Transmitted Klebsiella pneumoniae Sepsis: A Case Report.

Transfusion-transmitted bacterial infection (TTBI) is most commonly associated with platelet transfusions. Although multiple preventive measures are routinely implemented, the risk of bacterial contamination during blood donation, processing, or storage cannot be completely eliminated. In this context, Gram-negative bacteria such as Klebsiella pneumoniae may lead to fulminant sepsis, particularly in immunocompromised patients, including those with underlying liver disease. We describe the case of a 57-year-old man classified as American Society of Anesthesiologists (ASA) III who underwent a tr

Data-driven registration and modeling of brain deformation for image-guided neurosurgery.

Accurate compensation of brain deformation is critical for reliable image-guided neurosurgery. Surgical manipulation and tumor resection induce tissue motion, causing preoperative planning images to become misaligned with the intraoperative anatomy. In this review, we examine data-driven methods developed between 2020 and 2025 for brain deformation registration and modeling, with a particular focus on learning-based approaches. A comprehensive literature search was conducted in PubMed, IEEE Xplore, Scopus, and Web of Science using predefined inclusion and exclusion criteria for computational m

Timing of posterior capsular rupture during cataract surgery.

BACKGROUND/OBJECTIVES: The main aim of this study was to describe the stage at which posterior capsular rupture (PCR) occurred during cataract surgery and the association with anterior capsular (AC) tears. SUBJECTS/METHODS: This was a retrospective, observational cross-sectional study of all cataract surgeries conducted in Moorfields Eye Hospital complicated by PCR, between January 2014 and December 2018. The primary outcome measure was the stage of cataract surgery in which PCR occurred. The secondary outcome measures were the incidence of AC tears, intraoperative complications and location

Finger osteochondroma presenting as non-traumatic mallet finger in pediatric: A case report.

INTRODUCTION AND IMPORTANCE: A terminal extensor tendon disruption, with or without bony avulsion, may lead to a mallet deformity. In most cases, trauma precedes deformity. Non-traumatic causes are less probable. Several reports showed non-traumatic causes include villonodular tumors or chronic inflammation/infection. Conversely, osteochondroma, a benign tumor, typically develops from the growth plate of the long bones during the first two decades of life. CASE PRESENTATION: We describe an unusual case of osteochondroma and mallet deformity in a child's finger. The child was only 4 years old

Primary pleomorphic liposarcoma involving bilateral ovaries: Case report and literature review.

Pleomorphic liposarcomas (PLPSs) commonly occur in the extremities or retroperitoneum. However, cases of primary PLPSs in the female reproductive system are rare, with only one reported case in the ovary. Herein, we describe the case of a patient with primary PLPS involving bilateral ovaries. She presented with a 2-month history of abdominal pain, and underwent total hysterectomy with bilateral salpingo-oophorectomy, omentectomy, and excision of surface lesions on the bladder and rectum. Intraoperatively, nitrogen mustard (2%) was used to rinse the abdominal cavity. One week postoperatively, s

Intraparenchymal schwannoma in an adult presenting as a periventricular lesion of the parieto-occipital lobes: Diagnostic and surgical challenges.

INTRODUCTION: Intraparenchymal schwannomas (IS) are exceedingly rare, constituting less than 1 % of all intracranial schwannomas. To date, approximately 150 cases have been reported in the literature, with only 10 involving the parieto-occipital region. Unlike vestibular schwannomas, IS lacks an apparent association with cranial nerves and often mimics high-grade gliomas or metastatic tumors, leading to diagnostic uncertainty. RESEARCH QUESTION: How does the extreme rarity and atypical parieto-occipital location of intraparenchymal schwannomas contribute to diagnostic challenges and potential

Stump appendiceal abscess after appendicectomy: A case report.

INTRODUCTION: Stump appendicitis, an inflammation of the residual appendix after appendicectomy, presents with all the clinical features of acute appendicitis. This variant poses diagnostic challenges, particularly in patients with right iliac fossa pain. CASE PRESENTATION: We present a case of a 58-year-old man who developed an appendiceal stump abscess eight months after laparoscopic appendectomy. In addition to stable vital signs, abdominal examination revealed pain in the right lower quadrant. The CT scan showed a collection with a tubular structure corresponding to a remaining appendicea

Intraoperative ECoG in bottom-of-the-sulcus syndrome using a novel flexible strip electrode.

The recording of epileptiform discharges from bottom-of-sulcus focal cortical dysplasia (BOSD) is often difficult during intraoperative electrocorticography (ECoG) due to the deep localization. We describe the use in this scenario of a new-generation electrode strip with high flexibility, easily adapted to cortical gyri and sulci. A right-handed 20-year-old male with drug-resistant focal epilepsy due to BOSD of the inferior frontal gyrus and daily focal aware seizures was evaluated for epilepsy surgery. Based on electroclinical and neuroimaging results, a focal cortectomy guided by ECoG was pr

Case report: Duplicated appendicitis with history of cloacal exstrophy causing bowel obstruction.

INTRODUCTION: Cloacal and bladder exstrophy are rare embryological defects that can cause developmental disruption of surrounding organ structures, the pelvis, spinal cord, and small intestines being the most commonly affected. Duplicated appendix is another rare embryological defect that has historically caused confusing clinical presentations. Our case highlights a rare instance of a patient with cloacal exstrophy who presented with a bowel obstruction and an associated inflamed duplicated appendix. Not every twist is ovarian torsion: a case report of isolated torsion of the fallopian tube in a child.

UNLABELLED: Isolated fallopian tube torsion is a rare condition presenting with abdomen pain. The diagnosis is challenging because the clinical findings mimic several other conditions. In this article, we present a case of an adolescent girl who was diagnosed with the condition during laparoscopy. PRESENTATION OF THE CASE: A 15-year-old girl presented with complaints of sudden onset left lower quadrant pain, nausea, and vomiting. There was tenderness in the left iliac fossa. Ultrasonography revealed inconclusive findings and contrast-enhanced computed tomography of the abdomen and pelvis sugg

Umbilical Littre hernia: A rare case report of an acute abdomen.

INTRODUCTION: Littre's hernia (LH) is due to the presence of a Meckel's diverticulum (MD) in a hernial sac. It is an extremely rare condition in less than 1 % of all MD cases. It is often asymptomatic and is diagnosed incidentally during routine surgery for hernia repair. Surgery is the main treatment. Although the management of uncomplicated MD remains controversial, MD's management in the setting of either bowel obstruction or of Littre's hernia is done according to the clinical principles combined with the availability of local expertise. PRESENTATION OF CASE: We report a case of an 11-yea

Malignant cerebral edema after cranioplasty, a rare complication: case series and literature review.

UNLABELLED: Cranioplasty is standard neurosurgery performed after decompressive craniectomy. Fatal malignant cerebral oedema complications associated with this procedure are rare and clinically distinct, although unpredictable. It is thought that the pressure difference from removing atmospheric pressure had a long-term effect on the brain. This combined with the negative pressure applied by intraoperative pressure drainage may impact the perfusion brain. Here, the authors report four cases of cerebral oedema after cranioplasty and review similar cases in the literature. CASE PRESENTATION: Th

A rare case of a Low Grade Appendiceal Mucinous Neoplasm in a 46-year-old Middle Eastern female - A case report.

INTRODUCTION AND IMPORTANCE: Mucinous cell neoplasia are rare and conceivably fatal causes of surgical abdomen. Adult manifestations can be obscure and ill-defined, yielding misdiagnoses and wasting precious time and effort to reach the correct one. Clinical awareness ought to be present once presented with a surgical abdomen in such a patient because this neoplasm may present with misleading presentations which mask the actual diagnosis and masquerade as a different one, and in turn, may result in performing a dissimilar treatment intervention. Rapid learnt surgical judgments must be taken an

Pneumothorax Following Breast Surgery at an Ambulatory Surgery Center.

Pneumothorax is a known complication following breast surgery but is likely underappreciated by anesthesiologists. Iatrogenic pneumothorax can be caused by needle injury during local anesthetic injection, surgical damage to the intercostal fascia or pleura, or pulmonary injury from mechanical ventilation. We present two cases of pneumothorax following bilateral mastectomy with bilateral pectoral blocks and immediate breast reconstruction. Both cases occurred at a freestanding ambulatory surgery center in patients with no history of lung disease. One patient was found to have bilateral pneumoth
