Pancreatic head enlargement associated with a pancreatitis- induced intrasplenic pseudocyst in a patient with chronic pancreatitis: organ preserving surgical treatment.

Intrasplenic pseudocyst is a rare form of a late complication of chronic pancreatitis. We report the case of a 30-year-old man with an intrasplenic pseudocyst associated with chronic alcoholic pancreatitis. The patient was admitted with the third acute phase of chronic relapsing pancreatitis. Abdominal US and CT showed a large cyst in the pancreatic tail with involvement of the spleen. ERCP revealed marked irregularities of the main pancreatic duct without communication to the large cyst and a narrowing of the distal common bile duct by chronic pancreatitis of the head of the pancreas. Morbidity associated with total colon Hirschsprung's disease.

Increased awareness of total colon Hirschsprung's disease (TCH) has led to improvement in patient care and a decrease in the associated mortality. Morbidity has remained high and necessitates close follow-up to prevent the development of life-threatening complications. We retrospectively reviewed the records of 20 children with TCH treated at this institution since 1961. Follow-up averaged 40.4 months (range, 1 to 150). All patients were diverted with an ileostomy or jejunostomy. Eight children had a Swenson pull-through, three had a Martin procedure, and eight had a Kimura procedure. One chil…

Subtotal colectomy for the treatment of obstipation secondary to pelvic fracture malunion in cats.

Eleven cats in which subtotal colectomy with end-to-end jejunocolostomy was performed for the treatment of chronic obstipation secondary to pelvic fracture malunion were followed for at least 18 months. Seven cats had no recurrence of clinical signs and no longterm complications. In two cats, constipation was less severe and occurred less frequently; however, medical treatment was still required. In one cat, watery diarrhea gradually changed to semiformed feces of normal consistency by month 6; the cat was clinically normal at month 18. One cat had recurrence of constipation and was euthanatiz

The clinical case against tube feeding in palliative care of the elderly.

The terminal stages of many neurological illnesses occurring in the elderly produce feeding and swallowing problems. These difficulties lead to ethical, religious, philosophical, and medico-legal conflicts when decisions about starting or stopping tube feedings are considered. We present the case against all forms of tube feeding in a particular subset of elderly palliative patients. These are individuals who are in the end-stage of a progressive neurological disease who are noncommunicative, and spend all or most of their time in a recumbent position. We discuss the physiological phenomena ex

Association Between Travel Distance and Outcomes for Common Pediatric Surgical Procedures.

BACKGROUND: Regionalization may improve access to specialized pediatric surgical resources but require substantial travel for common inpatient procedures. We examined associations between estimated travel distance and inpatient outcomes after common pediatric surgical operations. STUDY DESIGN: We conducted a retrospective cohort study using 2018-2021 Healthcare Cost and Utilization Project State Inpatient Databases from 12 states. Children undergoing inpatient appendectomy, cholecystectomy, pyloromyotomy, inguinal hernia repair, or gastrostomy/jejunostomy placement were categorized by estimat…

After adequate resolution of local inflammation, additional endoscopic clipping from the esophageal side was performed, achieving complete closure. Feeding Duodenostomy Tube Placement After Minimally Invasive Esophagectomy with Posterior Mediastinal Reconstruction.

BACKGROUND: Early enteral nutrition is crucial for postoperative recovery after esophagectomy. A jejunostomy is conventionally created for nutritional access, but severe postoperative complications such as intestinal obstruction and intestinal torsion have been reported. This retrospective study evaluated the safety and feasibility of feeding duodenostomy tube placement utilizing the round ligament of the liver. METHODS: Patients who underwent esophagectomy with posterior mediastinal gastric conduit reconstruction and feeding duodenostomy at Kyoto University Hospital between October 2022 and …

Postprandial hypoglycemia after ileocolic interposition and Billroth-II gastrojejunostomy: A case report.

Postprandial reactive hypoglycemia, or late dumping syndrome, is a common but underrecognized complication from bypass surgery. We report an unusual case of postprandial reactive hypoglycemia in a patient with a severe esophageal stricture from corrosive agent ingestion who underwent ileocolic interposition and an antecolic Billroth-II gastrojejunostomy. A 22-year-old male patient with a one-year history of corrosive ingestion was referred to the hospital for a surgical correction of severe esophageal stricture. After the patient underwent ileocolic interposition and an antecolic Billroth-II g…

Robot-Assisted Total Gastrectomy: Preliminary Evaluation.

Background: Laparoscopic total gastrectomy with extended lymphadenectomy is a technically demanding surgical procedure with steep learning curve that has limited its widespread use. The aim of this study was to evaluate the feasibility of the robotic approach in total gastrectomy for cancer. We present our experience of 17 consecutive patients who underwent robotic total gastrectomy with intracorporal sutured Roux-en-Y esophagojejunostomy and jejuno jejunostomy between 2014 and 2017. Methods: Data were collected, and patients' demographics and outcomes were examined retrospectively. Results: S…
