Gallstone ileus is uncommon. Gallstone impaction in the jejunum was rarely reported. We report two cases of gallstone impaction in the proximal jejunum and in the distal ileum. One patient was treated with enterolithotomy alone and in the other one enterolithotomy was combined to cholecystectomy and repair of a cholecystoduodenal fistula. Contrast-enhanced computed tomography of the abdomen is the mainstay of diagnosis. One-stage procedure including both enterolithotomy and cholecystectomy with bilioenteric fistula repair should be reserved to low-risk patients.

A 33-year-old man presented with colicky, intermittent, midabdominal pain with nausea and vomiting. A Tc-99m pertechnetate abdominal scan was performed and revealed a focal area of increased uptake in the midabdomen associated with dilated proximal loops of small bowel. Surgery revealed a high-grade partial obstruction of the midportion of the jejunum secondary to an annular adenocarcinoma of the jejunum. This is the first known report of abnormal Tc-99m pertechnetate accumulation in an adenocarcinoma of the small bowel.

A new flexible probe to be introduced through the nose was used for the intubation of the jejunum for double-contrast small bowel enema. The new probe has a diameter of only 2.8 mm and was better tolerated than the normally applied Camus probe. Nevertheless, the failure rate (intubation of jejunum not successful) with the new probe as compared to the Camus probe was higher (17% versus 1%). We, therefore, conclude that the new probe should mainly be used by experienced examiners.
