Gracilis tendon transfer associated with distal alignment for patella alta with recurrent dislocations: an original surgical technique.

Many surgical techniques for the medial patellofemoral ligament have recently been suggested, all of which included problems identifying the femoral anchorage point and determining the proper extent of knee flexion for the transplant. P. Burdin proposed a different and original approach consisting in performing a gracilis muscle transfer to the medial edge of the patella, thus obtaining progressive tension of the transfer during knee flexion by means of the myotatic reflex. We report the results herein. We retrospectively assessed 17 knees treated for patellofemoral instability using this tech

Osteochondral fracture of the patella without soft tissue injury and with no dislocation: A case report.

INTRODUCTION: Osteochondral fracture of the patella is a fairly common pathology, but almost always associated with a spectrum of soft tissue injuries including anterior cruciate ligament (ACL) rupture. We present a rare case of an osteochondral fracture of the patella in the absence of ligament or soft tissue injuries and with no dislocation of the patella in a pediatric patient. PRESENTATION OF CASE: An 11-year-old male presented to the orthopedic clinic on crutches following a football injury. The patient had pain in his left knee with flexion deformity. Plain film radiography of the left

Medial Patellofemoral Ligament Reconstruction in Traumatic Patellar Dislocation without Patellar Fixation.

Acute traumatic patellar dislocation is a common injury, and spontaneous reduction may occur at the time of injury or may be reduced at the field of the accident by someone. It may be associated with osteochondral fractures and rupture of medial patellar stabilizers leading to recurrent patellar instability. The aim of this prospective study was to evaluate the outcomes of medial patellofemoral (PF) ligament (MPFL) reconstruction in recurrent traumatic patellar dislocation. Forty-five patients presented with PF instability as a result of traumatic rupture MPFL with normal patellar tracking und

[Reconstruction of medial patellofemoral ligament with Tightrope button fixation on the femoral side for the treatment of traumatic patellar dislocation].

OBJECTIVE: To explore the operating procedures and therapeutic effects of medial patellofemoral ligament reconstruction with Tightrope(Arthrex, FL, USA) button fixation at lateral femoral cortex. METHODS: From May 2014 to July 2016, 9 patients with traumatic patellar dislocation were treated. There were 5 males and 4 females, ranging in age from 16 to 47 years old, with an average of 23.7 years old. All the patients underment arthroscopic lateral retinaculum release and joint debridement first. Then the medial patellofemoral ligament was reconstructed by using a semitendinosus autograft. The

Kinematic parameter analysis and pilot clinical trial of dual-mobility semi-Knee prosthesis.

BACKGROUND: To address large tumor-related defects and lower limb-length discrepancies during limb-salvage surgery in children with malignant tumors in the distal femur, a new custom-made dual-mobility semi-knee prosthesis (DMK) was made. This study aimed to provide a theory and references for further clinical applications of this prosthesis. METHODS: Based on computed tomography data from adult knee joint samples, we used Mimics/Geomagic/Pro-E software and computer numerical control milling technology to design and manufacture the DMK. An in vitro study was carried out to examine the related

Optimizing patellofemoral tracking during total knee arthroplasty.

Fifty-seven patients who underwent 65 primary TKAs between 1993-1994 were retrospectively studied to identify the technical challenges and pitfalls associated with patellar resurfacing and to improve patellar tracking during total knee arthroplasty (TKA). Average patient age was 69 years. All surgeries were performed by a single surgeon (J.N.I), and the same prosthesis was used in all patients. Intraoperatively, attention was paid to avoid femoral and tibial component malrotation and prevent overstuffing of the patellofemoral joint. Preoperative limb alignment was varus in 42 knees, neutral in

[Post-traumatic hemarthrosis of the knee and arthroscopy. Review of 161 cases].

In a prospective study, 161 traumatic hemarthrosis of the knee had clinical, radiological and arthroscopic examination. The anatomical lesions were diagnosed clinically and radiologically in 78%, but was incomplete every other time. Arthroscopy allowed us to confirm, to complete or to do the exact diagnosis in 98%. Anatomical lesions were never benign: ligaments ruptures (65%), patella dislocations (20.5%), chondral lesions (41%), meniscal lesions (31%). The main interest of arthroscopy is to precise associated lesions and thus, to use these data for a rational treatment program. Most often, t

Successful closed reduction of Bosworth ankle fracture-dislocation: A case report.

Bosworth fracture-dislocation presents a challenge in ankle joint injuries owing to its irreducible nature, requiring open reduction in most cases. Reports on successful outcomes following closed reduction are limited, necessitating exploration into alternative treatment approaches. Herein, we report a case of Bosworth fracture-dislocation in a 39-year-old man, with radiographic evidence of posterior displacement of the distal portion of the proximal fibular fragment incarcerated behind the tibia. Closed reduction was attempted with the patient's knee flexed at 90°, employing gradual traction,

A new technique for closed reduction of traumatic posterior dislocations of the hip: the 'PGI technique'.

Many techniques have been described for closed reduction of posterior hip dislocations, but most require forceful and sustained traction, assistants to stabilise the pelvis and awkward positioning of the surgeon and/or the patient. We describe a new technique of closed reduction that does not need traction-countertraction or special positioning and can be safely and effectively performed by one surgeon.Fifteen patients, mean age 36.4 years, were subjected to reduction. There were five type I, four type II and six type III dislocations; ipsilateral injuries included one knee dislocation with pa…

Computed tomography of the normal patellofemoral joint.

Radiographic assessment of the patellofemoral joint (PFJ) is problematic because conventional views are cumbersome, difficult to standardize, and not reliable for displaying this joint with flexion of the knee less than about 30 degrees. We used computed tomography (CT) to obtain conveniently a direct transaxial view of the PFJ in ten normal human volunteers at different 0 degree, 20 degrees, and 45 degrees of knee flexion and during both contraction and relaxation of the quadriceps muscle. From the CT images we measured femoral trochlear angle, patellar centralization, femoral trochlear depth

[Lower extremity deformities as an obstacle in rehabilitation of meningomyelocele patients--pathogenesis and principles of treatment].

The fate of 89 patients with meningomyelocele operated at the Institute of Orthopedics and Rehabilitation in Poznań between 1970 and 1989 due to paretic deformities of lower extremities has been traced by the authors. Deformities prevented nursing, standing or ambulating; their type and results of treatment have been related to the level of neurosegmental lesion. Modified Sharrard's classification served to group the patients. The level of lesion established during lower extremities muscles testing has been verified after neurological examination supplemented with electrophysiological tests: s

Adequate Positioning of the Tibial Component Is Key to Avoiding Bearing Impingement in Oxford Unicompartmental Knee Arthroplasty.

BACKGROUND: Bearing dislocation is a serious complication of unicompartmental knee arthroplasty (UKA) with the Oxford knee prosthesis equipped with a mobile bearing. We aimed to clarify the extent of intraoperative movement of the mobile bearing and its relationship with the positioning of prosthesis components in patients undergoing Oxford UKA. METHODS: This retrospective study included 50 patients (50 knees) who underwent Oxford UKA for anteromedial osteoarthritis or osteonecrosis of the knee. Intraoperative bearing movement was assessed at various angles of knee flexion (0°, 30°, 60°, 90°,

A patient with mitochondrial myopathy associated with isolated succinate dehydrogenase deficiency.

We report on a boy with normal mental development who had muscle hypotonia and congenital dislocation of the hip and knee joints. Histochemical and biochemical examinations of his muscle specimen revealed no succinate dehydrogenase (SDH) activity. Since the NADH cytochrome c reductase and cytochrome c oxidase activities were normal, we concluded that he had an isolated SDH deficiency. Our patient provides further evidence for the clinical variability of this disorder.

Surgical treatment of combined injury to anterior cruciate ligament, posterior cruciate ligament, and medial structures.

Injuries involving the anterior cruciate, posterior cruciate, and medial collateral ligaments represent one combination of injuries representing knee dislocation. Prompt reduction and neurovascular evaluation are necessary when treating these injuries. Operative management involving reconstruction and repair of the injured structures produces optimal results. This article outlines the principles involved in evaluating and managing these injuries, combined with specifics of the authors' treatment approach, and review of the literature.

Recurrent patellar dislocation: personalised therapy or operative treatment? The REPPORT randomised trial protocol.

INTRODUCTION: Recurrent patellar dislocation is a debilitating musculoskeletal condition, affecting mainly adolescents and adults under the age of 30. It can persist for many decades, causing pain and cartilage and soft-tissue damage, potentially leading to osteoarthritis. Recurrent patellar dislocation can be managed with physiotherapy or surgery. However, it is not known which treatment is most effective. METHODS AND ANALYSIS: Recurrent Patellar Dislocation: Personalised Therapy or Operative Treatment (REPPORT) is a pragmatic, multicentre, two-arm, superiority, randomised controlled trial.

The clinical features of Ehlers-Danlos syndrome type VII due to a deletion of 24 amino acids from the pro alpha 1(I) chain of type I procollagen.

The clinical features and progress of a child with the type VII form of Ehlers-Danlos syndrome due to a deletion in the pro alpha 1(I) of type I procollagen were studied. The child was born with bilateral dislocations of hips and knees and all other joints were markedly hypermobile. Persistent severe joint instability was the major clinical abnormality. She had a depressed nasal bridge with prominent paranasal folds and deeply set eyes with mild hypertelorism and micrognathia. The skin was soft, moderately hyperelastic, and sagged over the face and knees. Skin fragility and easy bruising appea

Validation of the Acute Lower Limb Activation Mechanism Test: A Reliable Bedside Diagnostic Tool for Extensor Mechanism Ruptures.

Modern clinical practice has become increasingly reliant on imaging modalities to diagnose the extent of soft tissue injuries. We describe and then validate a novel bedside clinical test, the acute lower limb activation mechanism test (ALAM), that can be used to identify an extensor mechanism rupture in those unable to straight leg raise (SLR). Patients who presented with an acute knee injury and were unable to SLR were identified over a 6-year period. Patients meeting the inclusion criteria were examined utilizing the ALAM test and diagnosis subsequently confirmed with ultrasound, magnetic re
