End-of-life dreams and visions in palliative care: Perspectives, interpretations and practices from Italian professionals.

End‑of‑life dreams and visions (ELDVs) are coherent and emotionally meaningful experiences reported by lucid patients approaching death, yet their interpretation and clinical management remain uncertain. This study explored how Italian palliative care professionals and hospice volunteers encounter, understand, and respond to ELDVs in everyday practice. An online survey combining closed‑ and open‑ended items was completed by 239 participants working in end‑of‑life care settings. Findings show that ELDVs are frequently described as symbolically rich narratives involving deceased loved ones or im

Life's hard and then you die: the end-of-life priorities of people experiencing homelessness in the UK.

BACKGROUND: People experiencing homelessness often die young and without adequate support. In the UK, they fail to access palliative care services and their end-of-life priorities remain poorly understood. AIMS: To explore the end-of-life concerns, fears, preferences and priorities of a sample of people experiencing homelessness in the UK. METHODS: This is an interpretive phenomenology. Data collected through semi-structured, audio-recorded, face-to-face interviews with 21 homeless participants in the UK were analysed iteratively using thematic analysis. Findings have been interpreted throug

Kangaroo Cuddle Transport for Neonatal Palliative Care: A Service Evaluation.

BACKGROUND: Newborn infants represent a significant percentage of childhood deaths (>60%) with over 90% of newborn end-of-life care taking place in an acute hospital setting. This is in direct contrast to adults and children where specialised hospital or hospice care is used during their palliative care experience. To support family choice, a neonatal retrieval service developed a new model of care to transfer babies from a hospital to a hospice, a hospital closer to home or a private residence for palliative care while being cuddled by a parent. AIM: Identify and evaluate (i) service workflo

Epigenetic modulation by essential oils: Potential applications and challenges in palliative care.

Essential oils have long been used in complementary medicine, and recent research has demonstrated their potential to modulate epigenetic mechanisms such as DNA methylation, histone modification and microRNA regulation. These epigenetic effects suggest that essential oils may offer novel therapeutic benefits in palliative care, which primarily focus on symptom management, improvement of quality of life, and support for emotional well-being in patients with life-limiting conditions. This review examined the epigenetic mechanisms by which key compounds in essential oils, such as curcumin, eugeno

Caregivers' perspectives on family meetings and preparedness for imminent death: A qualitative descriptive study in a palliative care setting.

BACKGROUND: Family caregivers often feel unprepared for the final phase of life, which is associated with distress and adverse bereavement outcomes. Structured family meetings are recommended, yet evidence from Middle-Eastern settings is limited. AIM: To explore how routine family meetings influence caregiver preparedness and communication when patient is judged to be approaching death in Saudi palliative care service. DESIGN: Qualitative descriptive study using semi-structured interviews. Data were analysed using grounded theory analytic techniques. SETTING/PARTICIPANTS: A tertiary palliat

When cure ends: lived experiences of palliative care among Jordanian adults with hematological malignancies.

Hematological malignancies (HMs) are aggressive cancers with unpredictable trajectories, high symptom burden, and frequent intensive end-of-life interventions. Despite global recognition of palliative care (PC) as essential for life-limiting illnesses, patients with HMs - particularly in Jordan - often have limited access to timely PC. This study aimed to explore the experience of PC from the perspective of Jordanian adult patients in the terminal stage of HMs. A qualitative interpretive design which followed a phenomenological approach was used to collect and analyze data. Ten participants (a

Management of the CDH patient on ECLS.

Congenital diaphragmatic hernia (CDH) is the most common indication for respiratory extracorporeal life support (ECLS) in neonates. The survival rate of CDH neonates treated with ECLS is 50%, and this figure has remained relatively stable over the last few decades. This is likely because the current population of CDH neonates who require ECLS have a higher risk profile [1]. The management of neonates with CDH has evolved over time to emphasize postnatal stabilization, gentle ventilation, and multi-modal treatment of pulmonary hypertension. In order to minimize practice variation, many centers

Changing populations being treated with ECMO in the neonatal period - who are the others?

Extracorporeal life support via extracorporeal membrane oxygenation (ECMO) has served the sickest of neonates for almost 50 years. Naturally, the characteristics of neonates receiving ECMO have changed. Advances in care have averted the need for ECMO for some, while complex cases with uncertain outcomes, previously not eligible for ECMO, are now considered. Characterizing the disease states and outcomes for neonates on ECMO is challenging as many infants do not fall into classic categories, i.e. meconium aspiration syndrome (MAS), respiratory distress syndrome (RDS), or congenital diaphragmati

Low-Fidelity Code Blue Simulation on the Orthopaedic Unit.

Orthopaedic nurses care for a range of patients with comorbid conditions, but because of the implementation of Rapid Response Teams, rarely do orthopaedic patients experience cardiac or respiratory arrest. Rapid Response Teams decrease emergent arrest situations on nursing units by intervening to treat clinical deterioration or move patients to a higher level of care prior to cardiac or respiratory arrest. Orthopaedic nurses still need to be prepared to act emergently with basic life support skills, which are only renewed every 2 years. Review of actual code blue events and the implementation

Limitations in the approach health caregivers can take in end-of-life care decisions.

BACKGROUND: In the terminal stages of neuro-metabolic diseases, parents can begin to experience a sense of loss even before the child dies, and might accept death prematurely. CASES: A 2.5-year-old female patient with Sandoff Disease (diagnosed at 9 months of age), and a 17-month-old male Krabbe patient (diagnosed at 5 months of age) were admitted to the hospital with hypernatraemic dehydration and bronchopneumonia, respectively, within 10 days of each other. Both patients developed respiratory arrest short after admission and were supported with mechanical ventilation. Both families gave wri

The impact of patients' chronic disease on family quality of life: an experience from 26 specialties.

BackgroundPrevious studies have assessed family quality of life in individual disease areas and specialties. The aim of this study was to investigate the impact of disease on family members of patients over a wide range of specialties and identify key impact areas. This information is essential in order to reveal the extent of this impact and to allow strategies to be developed to support the family members of patients with chronic disease.MethodsSemi-structured interviews were carried out with 133 family members of mostly chronically ill patients from 26 medical specialties. Family members we

A rise-assisting robot extends life space and improves facial expressions of nursing home residents.

BACKGROUND: The introduction of nursing care-support devices using robotic technology is expected to reduce the task burden in long-term care facilities. OBJECTIVE: To investigate the use of the rise-assisting robot, Resyone, in extending and improving the life space of nursing home residents with severe care needs. METHODS: We performed a feasibility study in which Resyone was used to facilitate visits to additional sites in and around the nursing home as part of the care package of three residents. Two weeks before and four weeks after implementation of the new arrangements, the 30 caregiv

Access to hospice and palliative care for people with a migration background: a qualitative study on challenges and recommendations in end-of-life care.

BACKGROUND: Systemic inequalities shape healthcare access, disproportionately affecting people with a migration background in Germany. Empirical studies on this group in hospice and palliative care are quite limited and highlight a gap in understanding the challenges and potential solutions. OBJECTIVES: To explore, from the perspectives of healthcare providers, the challenges faced by patients with a migration background, their families, and healthcare providers in hospice and palliative care. The aim was to improve accessibility, reduce barriers, and ensure that hospice and palliative care s

Qualitative Analysis of the Lived Experience of Reproductive and Pediatric Health Care in the Military Health Care System.

INTRODUCTION: Persistent inequities exist in obstetric and neonatal outcomes in military families despite universal health care coverage. Though the exact underlying cause has not been identified, social determinants of health may uniquely impact military families. The purpose of this study was to qualitatively investigate the potential impact of social determinants of health and the lived experiences of military individuals seeking maternity care in the Military Health System. MATERIALS AND METHODS: This was an Institutional Review Board-approved protocol. Nine providers conducted 31 semi-st

Current Evidence and Best Practices in Palliative Care for Patients Undergoing Evaluation for Donation after Circulatory Death: A Narrative Review.

OBJECTIVE: This narrative review aims to summarize best practices in palliative care for patients undergoing evaluation for organ donation. BACKGROUND: Palliative care clinicians are uniquely positioned to improve care for potential organ donors and their families. When ongoing life support is no longer consistent with the patient's/family's goals, inpatient palliative care clinicians should be able to recognize and advocate for the end-of-life (EOL) priorities of patients who are potential organ donors. This includes guiding communication during decision-making, as well as providing symptom

Diabetic foot ulcer care integrating Salmon White's model: the critical role of community nurses.

Patients with diabetic foot ulcers (DFUs) often face significant challenges in receiving optimal care in the community. This condition can lead to serious complications if not addressed, including infection, delayed healing, amputation, decreased quality of life, financial burdens and mobility impairments. Community nurses are uniquely positioned to support DFU management, as they serve as the first point of contact for individuals managing this condition. They offer essential education on foot care and emphasise the importance of early intervention. In this context, using Salmon White's model
