Lymphatic malformations of the tongue base.

PURPOSE: In light of the paucity of literature on lymphatic malformations of the tongue base, our aim was to present our experience and long-term outcomes of patients with this rare and challenging pathologic entity. METHODS: Medical records of 25 patients treated by the 3 senior authors (RGA, MTC, and RTC) between 1974 and 2003 were retrospectively reviewed, and comprehensive clinical data were collected and analyzed. RESULTS: Twenty-one patients (13 female and 12 male infants) were diagnosed either prenatally or at birth. Of these patients, 18 required early airway stabilization; 17 requir

Extensive Cervicofacial Lymphatic Malformation With Hemorrhagic Component in a Neonate: Airway and Anesthetic Challenges in Surgical Management.

Lymphatic malformations (LMs) are benign lesions common in infancy and early childhood. Massive hemorrhage in LMs is associated with high morbidity. A full-term male neonate, born through spontaneous vaginal delivery to an unbooked 25-year-old mother, presented with a large, bluish cystic swelling involving the anterior neck and floor of the mouth. The neonate exhibited weak crying, oral bleeding, and airway obstruction with tongue protrusion. Initial airway support with continuous positive airway pressure was unsuccessful. The airway was secured with a 3.5-mm uncuffed endotracheal tube, as co

Abdominal Lymphatic Malformations: A Novel Approach in Management.

AIM OF THE STUDY: Abdominal lymphatic malformations (LM) are a challenging clinical entity. Complete excision can be impossible due to the infiltrative nature of some lesions and high rate of complications. Sclerotherapy may require multiple episodes of general anaesthesia and an inability to be definitive in terms of diagnosis and treatment. Subsequently we have adopted a newer algorithm of treatment involving a joint procedure with interventional radiology to enable an intra-operative treatment plan based on the findings. We present our early results. METHOD: A retrospective observational s

Doxycycline sclerotherapy in patients with periorbital lymphatic malformation: A case series and literature review.

PURPOSE: To describe and report the outcomes of doxycycline sclerotherapy in patients with periorbital lymphatic malformations(LMs). BASIC PROCEDURES: A retrospective review of consecutive patients diagnosed with periorbital LMs and who received doxycycline sclerotherapy at Hong Kong Eye Hospital and Queen Elizabeth Hospital, Hong Kong between January 2016 and June 2022. Doxycycline was prepared with a concentration of 100 mg diluted in 10 mL water for injection. A 23-gauge needle aiming at the center of the macrocyst was used to aspirate fluid from the lesion; this was then followed by an in

Treatment of severe Kaposiform lymphangiomatosis positive for NRAS mutation by MEK inhibition.

BACKGROUND: Kaposiform lymphangiomatosis (KLA) is a complex lymphatic anomaly involving most commonly the mediastinum, lung, skin and bones with few effective treatments. In recent years, RAS-MAPK pathway mutations were shown to underlie the pathogenesis of several complex lymphatic anomalies. Specifically, an activating NRAS mutation (p.Q61R) was found in the majority of KLA patients. Recent reports demonstrated promising results of treatment with the MEK inhibitor, Trametinib, in patients with complex lymphatic anomalies harboring gain of function mutations in ARAF and SOS1, as well as loss

Surgical resection of macrocystic lymphatic malformations of the head and neck: Short and long-term outcomes.

BACKGROUND: Controversy exists on management of lymphatic malformations, with schools of thought advocating for observation, surgery or sclerotherapy. This study sought to examine outcomes after surgical resection of pediatric cervicofacial macrocystic lymphatic malformations (MLM). METHODS: Case series with planned data collection on pediatric patients with cervicofacial MLM who underwent surgical resection at a tertiary referral center for vascular anomalies from January 1995 to June 2016. For consistency in patient population analysis, patients who had pre-surgical sclerotherapy or had mix

Intralesional bleomycin injection in management of low flow vascular malformations in children.

Low flow vascular malformations are challenging to manage, particularly with their propensity to grow, and can lead to severe disfigurement and dysfunction. Traditional surgical excision is fraught with tedious dissection and complications, particularly in the head and neck region. Trends toward less invasive techniques, such as intralesional sclerotherapy, are proving to be successful independent treatments or adjuncts in management in low flow vascular malformations. This study was a retrospective case note review, over an 8-year period, reporting the outcomes of 32 children (mean = 5.8 year

Bilateral abdominal cryptorchidism with large left testicular seminoma and failed right urogenital union.

This is a case report of a 32-year-old male with bilateral intra-abdominal cryptorchidism. A large seminoma had developed on the left testis with paraaortic lymph nodes metastasis. The tumour was excised easily. The right testis was found just inside the deep inguinal ring, without the vas deferens in the spermatic cord. The patient requested orchidopexy despite the well-explained risk of cancer development. Therefore, a second right groin incision was performed. In the right inguinal canal, there was a normal-looking vas deferens that ended in an atrophic nubbin of fibrous tissue without an e

Reversal of abnormal lymphoscintigraphy after placement of venous stents for correction of associated venous obstruction.

PURPOSE: The purpose of this study was to report improvement or normalization of abnormal lymphoscintigraphy in swollen lower limbs after correction of associated venous stenosis by balloon dilatation and placement of venous stent of the iliac veins. MATERIAL: Twenty-six patients with lower-limb swelling underwent balloon dilatation and placement of venous stent of the iliac veins for correction of venous stenosis. Technetium 99M-sulfur colloid lymphoscintigraphy was also abnormal (8 absent, 18 reduced) before stent placement in all 26 limbs, suggesting combined venous/lymphatic etiology for

Surgical management of perineal masses in patients with anorectal malformations.

BACKGROUND: The aim of this study was to review the outcome of surgical management of various types of perineal masses encountered in patients with anorectal malformations (ARM). METHODS: Retrospective review from 2 large pediatric anorectal referral centers. RESULTS: Twenty-two patients with a perineal mass were identified in more than 2000 patients treated for an ARM over a 15-year period. The 22 patients (4 men) represented all levels of severity of ARMs. The lesions were of 3 types: lipomas (n = 10), vascular anomalies (n = 4), and hamartomas/choristomas (n = 8). The lipomas were careful

Malignant lymph nodes of the head and neck: imaging pearls, pitfalls, and practical approach to reporting in the era of extranodal extension.

Accurate characterization and reporting of cervical lymph nodes is critical when evaluating head and neck malignancies, as findings directly impact staging, treatment planning, and prognosis. Knowledge of lymphatic drainage patterns predicts nodal spread of disease and conversely, may help in identifying primary malignancy. Variability in size criteria for an abnormal lymph node across literature makes reporting difficult when used in isolation; however, careful attention to morphologic changes improves diagnostic accuracy. FDG-PET imaging serves as a valuable complementary tool when available

[ROBOT-ASSISTED LAPAROSCOPIC RADICAL PROSTATECTOMY WITHOUT TRANSCATHETER ARTERIAL EMBOLIZATION FOR A PATIENT WITH PROSTATE CANCER AND PELVIC ARTERIOVENOUS MALFORMATION: A CASE REPORT].

We performed robot-assisted laparoscopic radical prostatectomy (RARP) without transcatheter arterial embolization (TAE) for a 72-year-old male patient with prostate cancer and pelvic arteriovenous malformation (AVM). Though lymphatic dissection was made contralateral to the AVM, the operation time (robotic: 2h 40 min, and total: 3h 2 min) was not long. Moreover, the blood loss amount of 250 ml was less than those in the past reports of preoperative TAE. Robotic surgery, a dissection of an abnormal arterial branch from the internal iliac artery before the division of the bladder neck, bunching

Aspects of the Tumor Microenvironment Involved in Immune Resistance and Drug Resistance.

The tumor microenvironment (TME) is a complex and ever-changing "rogue organ" composed of its own blood supply, lymphatic and nervous systems, stroma, immune cells and extracellular matrix (ECM). These complex components, utilizing both benign and malignant cells, nurture the harsh, immunosuppressive and nutrient-deficient environment necessary for tumor cell growth, proliferation and phenotypic flexibility and variation. An important aspect of the TME is cellular crosstalk and cell-to-ECM communication. This interaction induces the release of soluble factors responsible for immune evasion and

[A Case of a-Fetoprotein Producing Gastric Cancer with Lymph Node Recurrence after Endoscopic Submucosal Dissection].

A 70-year-old man underwent endoscopic submucosal dissection(ESD)for 2 early-stage gastric cancers in 2009 and 2014, respectively. Both were pathologically diagnosed after curative resection. In 2015, we detected swollen lymph nodes in the lesser curvature of the stomach on computed tomography(CT). PET-CT revealed that the swollen lymph nodes showed an abnormal uptake of fluorodeoxyglucose; therefore, we considered the possibility of metastatic recurrence of the gastric can- cer. The patient's serum a-fetoprotein(AFP)level was elevated to 30.6 ng/mL. The intraoperative pathological diagnosis o

Palliative management of lymphangioleiomyomatosis: Using video-assisted thoracoscopic surgery.

Lymphangioleiomyomatosis (LAM) is a rare, progressive systemic disease of unknown etiology, insidious onset, often fatal, and underdiagnosed. It is exclusively found among women. LAM mainly involves the lungs where, as its name suggests, lymphatic (lymph), blood vessel (angio), and airways are surrounded by smooth muscle (leiomyoma) proliferation. It may be associated with tuberous sclerosis with clinical manifestations varying from simple cough to the development of recurrent pneumothoraces, haemoptysis, and pleural effusions. There is currently no treatment or cure. We present a rare case of

Yellow nail syndrome.

PURPOSE OF REVIEW: The yellow nail syndrome (YNS) is a rare disorder of unknown cause characterized by the triad of yellow and thickened nails, lymphedema and respiratory manifestations. We review the current state of knowledge, particularly regarding the diagnosis and management of this disorder. RECENT FINDINGS: Available data suggest acquired lymphatic dysfunction to be the predominant mechanism underlying the clinical manifestations of YNS. The clinical features are variable among individuals diagnosed to have this disorder, and these features can vary over time. Although many disorders h

The feasibility of using poroelastographic techniques for distinguishing between normal and lymphedematous tissues in vivo.

Lymphedema is a common condition involving an abnormal accumulation of lymphatic fluid in the interstitial space that causes swelling, most often in the arm(s) and leg(s). Lymphedema is a significant lifelong concern that can be congenital or develop following cancer treatment or cancer metastasis. Common methods of evaluation of lymphedema are mostly qualitative making it difficult to reliably assess the severity of the disease, a key factor in choosing the appropriate treatment. In this paper, we investigate the feasibility of using novel elastographic techniques to differentiate between lym

Early colon cancer detected by 18F-FDG PET.

Positron emission tomography (PET) is a noninvasive functional imaging modality that can disclose the presence of a malignant disease. It has recently been reported that PET may be useful to detect primary colorectal cancer (CRC). We present the case of a 47-yr-old man with early colon cancer detected by 18F-fluorodeoxyglucose (FDG) PET. The patient consulted us because of a positive fecal occult blood test and focal FDG uptake in the pelvic cavity detected at a physical check-up. After the usual work up, he was diagnosed as having a sigmoid polyp, 16 mm in diameter. Subsequently, colonoscopic

Histopathology of the conducting tissue of the heart in Chagas' myocarditis.

The conducting tissue of the heart was studied in 25 human cases of Chagas' myocarditis with a method which employs complete serial sections mounted on continuous transparent plastic tape. The pathological changes were correlated with electrocardiographic findings. The inflammation of the acute phase of Chagas' myocarditis, as seen in one single case, did not seem to interfere with conduction through the AV system. In chronic Chagas' myocarditis the conducting tissue showed extensive and variable changes: chronic inflammation, fibrosis, atrophy and fragmentation of specific fibers, extreme dil
