Magnesium replacement therapy.

Magnesium is involved as a cofactor in many vital enzymatic reactions. It is also important in the maintenance of membrane electric potential. Diagnosis of magnesium disturbances must often be based on clinical judgment. Hypomagnesemia is frequently associated with hypokalemia and hypocalcemia; hypermagnesemia most often occurs in patients with acute or chronic renal failure. Hypomagnesemia presents as neuromuscular, central nervous system and cardiac abnormalities. Inadequate dietary intake of magnesium occurs in alcoholism, catabolic states and gastrointestinal diseases. Intravenous administ

Routine serum magnesium determination--a continuing unrecognized need.

Abnormalities of serum Mg may be the most underdiagnosed serum electrolyte abnormality in clinical practice today. The incidence appears to range from 12.5 to 20% on routine determination. Hypomagnesemia is found in 6.9-11% of hospitalized patients on routine determination of serum Mg. A high frequency of hypomagnesemia occurs with other common electrolyte abnormalities: hypokalemia (38-42%), hypophosphatemia (29%), hyponatremia (23%), and hypocalcemia (22%). Routine serum Mg determination would facilitate identification of hypomagnesemia which may be a more frequent contributor to digitalis t

Diet and adjuvant-induced arthritis in the rat.

Rats fed a high carbohydrate or a high protein diet deficient in copper, magnesium or zinc, or in the three trace elements together, failed to develop adjuvant-induced arthritis, yet a deficiency of both copper and magnesium in the diets exhibited no such inhibition. On the other hand, only a magnesium deficiency reduced the severity of the dextran anaphylactoid reaction.

Plasma Zinc and Magnesium Levels in Sickle Cell Disease Patients in Latakia, Syria.

Background Sickle cell disease (SCD) is a serious inherited disorder that affects millions of people worldwide. Zinc and magnesium are essen … pared to the controls (P < 0.05). Twelve SCD patients (14.1%) were zinc-deficient, and 37 (43%) had magnesium deficiency. Plasma zinc and magnesium levels were higher in HbSS patients than those with Hb S/β-Thal patients, but with no statistical significance (P > 0.05). Notably, all zinc-deficient SCD patients were < 16 years old. There was no significant (P > 0.05) d …

Magnesium in perinatal care and infant health.

Magnesium, the second most abundant intracellular cation, is essential for life. The consequences of deficiency are severest in the smallest and youngest members of each species and may include sudden unexpected death. Magnesium deficiency, usually diagnosed by hypomagnesemia, may be congenital, as in premature infants, infants of magnesium-deficient mothers and infants with intrauterine growth retardation. It may be acquired or caused by low magnesium intake, the use of magnesium-wasting drugs, illness provoking gastrointestinal or renal losses of the mineral, or high metabolic demands impose…

[Magnesium in cardiology. A challenge for new studies].

Magnesium has been a controversial issue in cardiology for a long time. The following facts are now known: Magnesium plays a decisive role in the treatment of malign ventricular rhythm disturbances; this is especially true for Torsade pointes, which is connected with post-depolarisation and frequently causes sudden cardiac death. In the critical infarction phase intravenous magnesium therapy results in a reduction of malign rhythm disturbances and of the mortality rate. Magnesium plays a role in glycoside intolerance in patients with congestive heart failure. Magnesium deficiency is an importa…

The role of magnesium in pancreatic beta-cell function and homeostasis.

Magnesium plays an essential role in glucose utilization and insulin signaling. Recent advances have revealed a greater prevalence of hypomagnesemia in general, and low intracellular magnesium levels in individuals with diabetes contribute to β-cell dysfunction and insulin resistance. This article describes the documented effects of magnesium on various aspects of β-cells and glucose homeostasis. Studies have demonstrated that magnesium deficiency is associated with reduced pancreatic β-cell activity and increased insulin resistance in patients with type 2 diabetes. Additionally, magnesium is …

Downbeat nystagmus with magnesium depletion.

Two patients with neurological signs of magnesium depletion complained of oscillopsia and manifested downbeat nystagmus. Serum magnesium levels measured less than 1.0 mg/dL. Hypomagnesemia resulted from debilitating intestinal disease in both patients and apparently was aggravated in one case by failure to add magnesium sulfate to the patient's regimen for total parenteral nutrition. Neither patient had any of the neurological conditions reported with downbeat nystagmus. Thus, downbeat nystagmus in the primary position occurs with the metabolic derangement of severe magnesium depletion. Anothe

A Comprehensive Review on Understanding Magnesium Disorders: Pathophysiology, Clinical Manifestations, and Management Strategies.

Magnesium is vital in a broad spectrum of physiological processes, including enzyme activity, energy production, and neuromuscular function. Despite its crucial role, magnesium disorders - comprising both deficiency (hypomagnesemia) and excess (hypermagnesemia) - are frequently underrecognized and inadequately managed in clinical practice. Magnesium deficiency is widespread, particularly among populations with chronic illnesses, the elderly, and those experiencing malnutrition, often leading to significant neuromuscular, cardiovascular, and metabolic complications. Conversely, hypermagnesemia,…

Should magnesium therapy be considered for the treatment of coronary heart disease? I. A critical appraisal of current facts and hypotheses.

When given at physiological doses, therapy with magnesium corrects the alterations in cellular function resulting from magnesium deficiency, whereas at higher dosages, which induce hypermagnesaemic levels, magnesium possesses pharmacological effects, such as the inhibition of the calcium influx: this may alter the electrophysiological properties of heart cells, decrease catecholamine secretion, influence the synthesis of prostacyclin and/or alter platelet function. The evidence that magnesium deficiency has untoward effects in patients with ischaemic heart disease is only circumstantial and di…

Magnesium as an Important Factor in the Pathogenesis and Treatment of Migraine-From Theory to Practice.

So far, no coherent and convincing theory has been developed to fully explain the pathogenesis of migraine, although many researchers and experts emphasize its association with spreading cortical depression, oxidative stress, vascular changes, nervous excitement, neurotransmitter release, and electrolyte disturbances. The contribution of magnesium deficiency to the induction of cortical depression or abnormal glutamatergic neurotransmission is a likely mechanism of the magnesium-migraine relationship. Hence, there is interest in various methods of assessing magnesium ion deficiency and attempt…

[Physiology and main implications of magnesium in osteo-articular pathology].

The role of magnesium in the constitution, metabolism and hormonal control of bone tissue can be deducted from what is known of its physiology. Magnesium deficiency may result in vitamin-D resistant hypocalcaemia or even biochemical signs of hypoparathyroidism, which only regress after the deficiency is corrected. Measurements of magnesium levels in blood and urine are justified when such abnormalities occur in patients with digestive disorders, predominantly alcoholism, liver diseases or malabsorption. In all other circumstances, magnesium therapy is very rarely indicated for osteo-articular …

Adverse cardiovascular and blood pressure effects of drug-induced hypomagnesemia.

Introduction: The objective of this study was to review the current status of drug-induced hypomagnesemia and its adverse effects on cardiovascular disease (CVD) and hypertension. Since magnesium is a potent vasodilator, which modulates vasomotor tone, peripheral blood flow, and hypertension, its deficiency could have significant cardiovascular and blood pressure (BP) effects.Areas covered: Studies have shown that several factors can contribute to magnesium deficiency including age, diet, disease, and certain drugs such as diuretics and proton-pump inhibitors (PPIs). For an updated perspective…
