Satellite glial cell P2Y12 receptor in the trigeminal ganglion is involved in lingual neuropathic pain mechanisms in rats.

BACKGROUND: It has been reported that the P2Y12 receptor (P2Y12R) is involved in satellite glial cells (SGCs) activation, indicating that P2Y12R expressed in SGCs may play functional roles in orofacial neuropathic pain mechanisms. However, the involvement of P2Y12R in orofacial neuropathic pain mechanisms is still unknown. We therefore studied the reflex to noxious mechanical or heat stimulation of the tongue, P2Y12R and glial fibrillary acidic protein (GFAP) immunohistochemistries in the trigeminal ganglion (TG) in a rat model of unilateral lingual nerve crush (LNC) to evaluate role of P2Y12R

[Hypodontia: therapeutic strategy elaborated from 30 cases].

INTRODUCTION: Hypodontia is defined as the absence of at least six permanent teeth due to the hypodevelopment of tooth germs. The prevalence is weak and varies according to the ethnic origin. This familial abnormality is due to various mutations or polymorphisms of genes. It may be associated with other dental abnormalities. In children, it is responsible for growth, mastication, and phonation disorders, hypo-alveolia and lingual interposition in the toothless sites. We report the management of 30 patients presenting with oligodontia. MATERIAL AND METHOD: The diagnosis was made on clinical an

An autoradiographic examination of the central distribution of the trigeminal, facial, glossopharyngeal, and vagal nerves in the monkey.

The central distributions of primary afferent axons in cranial nerves V, VII, IX, and X have been re-examined autoradiographically after 3H-proline injections into their peripheral ganglia. Fiber-labeling after subtotal injections of the trigeminal ganglion, besides confirming earlier classical descriptions, suggests that trigeminal fibers of the ophthalmic and mandibular (but not maxillary) branches enter the ventrolateral part of the nucleus of the solitary tract (NST). Injection of VII's geniculate ganglion labels fibers which both ascend and descend upon reaching NST. The ascending fibers

Craniofacial Fibrous Dysplasia to Affect or Not the Optic Nerve in Long-Term Follow-Up of Three Cases.

Fibrous dysplasia of the bone is characterized by immature fibrous bones of trabeculae and fibrovascular proliferation in the medulla. In this study, we report three consecutive patients with craniofacial fibrous dysplasia with or without optic nerve involvement. In Case 1, a 43-year-old man with blurred vision in the right eye at the first visit was well until the age of 54 years, when he came back with symptoms suggestive of paranasal sinusitis. Computed tomography scans disclosed a mucocele in the right sphenoid sinus and thickened bilateral ethmoid, sphenoid, and frontal bones. He underwen

Anesthetic efficacy of 1.8 milliliters and 3.6 milliliters of 2% lidocaine with 1:100,000 epinephrine for posterior superior alveolar nerve blocks.

INTRODUCTION: The purpose of this prospective, randomized, single-blinded study was to measure the degree of anesthesia obtained with 1.8 mL and 3.6 mL of 2% lidocaine with 1:100,000 epinephrine in posterior superior alveolar (PSA) nerve blocks. METHODS: Thirty-one adult subjects randomly received PSA nerve blocks of 1.8 mL and 3.6 mL of the lidocaine solution at 2 separate appointments in a crossover design. An electric pulp tester was used to test for anesthesia of the first and second molars and premolars in 3-minute cycles for 50 minutes. Anesthesia was considered successful when 2 consec

Anterior middle superior alveolar injection is effective in providing anesthesia extending to the last standing molar in maxillary periodontal surgery.

BACKGROUND: Multiple injections are required to anesthetize the maxilla for periodontal surgery that involves a quadrant or multiple teeth across the midline. The anterior middle superior alveolar (AMSA) field block was reported to effectively anesthetize the central incisors to the mesial aspect of the first molar of the maxilla with one or two injections. To our knowledge, the extent of anesthesia has not been reported beyond first molars in the literature. METHODS: Fifty subjects who were indicated for periodontal surgery were provided maxillary anesthesia with an AMSA injection through a

Trigeminocardiac Reflex Associated With Orthognathic Surgery: An Illustrative Case Report and Review of the Literature.

The trigeminocardiac reflex (TCR) is a rare brainstem reflex characterized by bradycardia and hemodynamic instability secondary to stimulation of branches of the trigeminal nerve. In maxillofacial surgery, the phenomenon has been reported in orthognathic surgery, foreign body removal, and fixation of midface fractures. Despite reasonably extensive reporting of the reflex in head and neck surgery in general, the mechanism and predisposing factors are poorly understood. The primary aim of this systematic review was to explore potential contributory factors and summarize outcomes of the TCR in pa

Long-term clinical remission in rapidly recurrent maxillary osteosarcoma treated with anlotinib-based multimodal therapy: a case report.

BACKGROUND: Osteosarcoma of the jaw is a rare clinical entity characterized by a high propensity for local recurrence, for which surgical resection serves as the primary treatment modality. However, achieving negative margins in the maxilla is often complicated by the proximity of critical neurovascular and orbital structures. Furthermore, in certain emergency scenarios, surgical compromises may precipitate suboptimal initial control and rapid disease progression. This presents a significant therapeutic dilemma, particularly given the lack of standardized management guidelines for refractory c

Case 337: Venous Vascular Malformation.

A 38-year-old previously healthy male patient presented with left-sided facial pain over the prior 5 weeks. He first noticed the pain while washing and applying pressure to his face. The pain was described as shock-like, sharp and shooting, and radiating along the left cheek and temple. It began as 1-2-second episodes occurring two to three times per day, sometimes spontaneously, progressing in severity and frequency over time. Mild progressive left facial weakness also developed a few weeks after initial symptoms. Physical examination demonstrated reproducible pain in the distribution of the

Inhibiting Nav1.7 channels in pulpitis: An in vivo study on neuronal hyperexcitability.

Pulpitis constitutes a significant challenge in clinical management due to its impact on peripheral nerve tissue and the persistence of chronic pain. Despite its clinical importance, the correlation between neuronal activity and the expression of voltage-gated sodium channel 1.7 (Nav1.7) in the trigeminal ganglion (TG) during pulpitis is less investigated. The aim of this study was to examine the relationship between experimentally induced pulpitis and Nav1.7 expression in the TG and to investigate the potential of selective Nav1.7 modulation to attenuate TG abnormal activity associated with p

Concomitant Embolization and Microsurgical Resection of a Giant, Hypervascular Skull Base Meningioma: 2-Dimensional Operative Video.

Some skull base tumors can be extremely hypervascular, incorporating multiple vascular territories and demonstrating arteriovenous shunting. Devascularization is a critical step undertaken early in meningioma surgery, necessary before the debulking that is required in skull base tumors. While devascularization can often be achieved with appropriate approach selection, bony drilling, and microsurgical cautery, preoperative embolization of meningiomas has an invaluable role in selected cases.1,2  Embolization, however, does have added risk, magnified in large tumors by the potential infarction w

Transmitted-light plethysmography detects changes in human pulpal blood flow elicited by innocuous tooth cooling and foot heating.

OBJECTIVES: The present study aimed firstly, to investigate whether pulpal circulatory changes elicited by innocuous tooth cooling and foot heating can be monitored with transmitted-light plethysmography (TLP), which detects pulpal blood volume changes, and secondly, to assess the effect of autonomic nervous control on TLP values. DESIGN: Thirty sound permanent maxillary incisors in 30 healthy volunteers (age: 25-35 years) were examined. The photodiode and 525-nm light-emitting diode of a prototype TLP system (J. Morita) were fixed onto the palatal and labial side, respectively, of each tooth

Comparison of evoked vs. spontaneous tics in a patient with trigeminal neuralgia (tic doloureux).

A 53-year old woman with tic doloureaux, affecting her right maxillary division of the trigeminal nerve (V2), could elicit shooting pains by slightly tapping her teeth when off medication. The pains, which she normally rated as > 6/10 on a visual analog scale (VAS), were electric shock-like in nature. She had no other spontaneous or ongoing background pain affecting the region. Based on her ability to elicit these tics, functional magnetic resonance imaging (fMRI) was performed while she produced brief shocks every 2 minutes on cue (evoked pain) over a 20 min period. In addition, she had 1-2 s

[Malignant triton tumor of the sinonasal tract].

INTRODUCTION: The combination of a malignant peripheral nerve sheath tumor with rhabdomyosarcomatous elements has been termed Malignant Triton Tumor (MTT). This tumor was first reported in 1932 and the prognosis was poor. Involvement of the sinonasal tract is rare with only 10 case reports. CASE: We report the case of a 80-year-old woman who presented an obstruction of the right nasal fossa which developed over several years without anosmia. She consulted because of recurrent epistaxis. The CT-scan revealed a tumor occupying the right ethmoid the right maxillary sinus and the homolateral nasa

Successful Surgical Management of Intracranial Carotid Artery Trauma Following Penetrating Facial Injury: A Case Report.

BACKGROUND Carotid artery injury has an incidence of 0.2% in the National Trauma Data Bank. The true incidence of intracranial carotid injury is unknown but can be estimated at less than one in 1000 trauma-related inpatient admissions in America. Operatively managed penetrating carotid trauma has a mortality rate approaching 20%, and the selection of the appropriate operative approach is not straightforward. We present a case of penetrating carotid trauma successfully managed via combined approach by neurosurgery and otolaryngology teams. CASE REPORT A 74-year-old woman fell into a honeysuckle
