Robot-Assisted Osteotomy and Reconstruction with AR Guidance in Maxillofacial Reconstructive Surgery.

The treatment of maxillofacial tumors requires coordinated resection and reconstruction. Conventional template-guided open osteotomy techniques may increase the risk of tissue injury. Additionally, the accuracy of titanium plate-based reconstruction is limited by the lack of standardized positional criteria, thereby potentially affecting alignment consistency. To address these challenges, we propose a maxillofacial tumor treatment system termed RAMRS, comprising 2 modules: robot-assisted osteotomy and augmented reality-guided reconstruction (ARR). Within therobot-assisted osteotomy module, a h

Long-term evaluation of treatment protocols for isolated midfacial fractures in a German nation-wide craniomaxillofacial trauma center 2007-2017.

An update on the trends in maxillofacial traumatology provides additional information on the actual and changing needs. This retrospective study aimed to review all patient records of patients treated for isolated midface fractures in the Department of Cranial- and Maxillofacial Surgery at the Dortmund General Hospital between 2007 and 2017. The patient radiographs and patient files were reviewed. The safety and efficacy of the applied methods were controlled by assessing complications based on the Clavien-Dindo classification system. The statistical analysis included descriptive methods inclu

Impalement of an unusual foreign body on the temporomandibular joint causing severe trismus.

A penetrating injury by a foreign body is comparatively common in the oral and maxillofacial region. On the other hand, injury to the temporomandibular joint (TMJ) by a foreign object is very rare. The TMJ is an anatomically narrow space surrounded by hard bony processes. An unusual case of trauma with severe trismus caused by a foreign body that impaled the TMJ is reported. A 55-year-old man presented with a 5 × 1-cm laceration to the right cheek caused by a flying object propelled during the use of a lawn mower. The edge of the foreign body had a metallic wire, which became imbedded in the w

[Trauma to the anterior cranial base. Report of a case].

The authors present a case of foreign body retention, following car accident, localized on superior medial corner of the right orbit. The traumatic impact, involving endo-orbital structures, owing to the peculiar direction of the foreign body, caused a fracture in the anterior cranial base and laceration of the dura madre. As the anatomical structures involved did not concern only the MaxilloFacial district, the choice of the protocol to be applied to the patient was taken in accordance with the neurosurgical équipe. An intra cranial surgical approach has allowed to remove, by retrograding the

SCPNB is an adjuvant to local anaesthesia for maxillofacial surgical practice.

The superficial cervical plexus nerve block [SCPNB] procedure is frequently used throughout head and neck surgery because it is simple to learn and has a low rate of complications. The investigation of this method might produce superior outcomes in treating frequent maxillofacial disorders including mandibular fractures and infections of the odontogenic region. The SCPNB is known to play a part in the medical evacuation of head and neck abscesses, the excision of superficial diseases in the perimandibular region, and the therapy of mandibular fracture, despite the dearth of research in this ar

Use of prophylactic antibiotics in preventing infection of traumatic injuries.

In managing traumatic wounds, the primary goal is to achieve rapid healing with optimal functional and esthetic results. This is best accomplished by providing an environment that prevents infection of the wound during healing. Despite good wound care, some infections still occur. Accordingly, some investigators argue that prophylactic antibiotics have an important role in the management of certain types of wounds. This article reviews the basis of antibiotic use in preventing wound infection in general and its use in oral and facial wounds in particular.

Endoscopic treatment of facial fractures.

The application of endoscopic surgical techniques to the treatment of craniomaxillofacial fractures not only has decreased the morbidity associated with the surgical approaches but has significantly altered the treatment philosophy for many types of fracture. Frontal sinus fractures, orbital floor fractures, zygomatic arch fractures, and subcondylar mandible fractures are the most notable examples where endoscopic techniques have found acceptance.

Facial development, continuous tooth eruption, and mesial drift as compromising factors for implant placement.

The replacement of teeth lost by children because of trauma can be an important indication for early implant therapy. Osseointegrated dental implants, like ankylosed teeth, alter position as growth-related changes occur within the jawbones (displacement, remodeling, mesial drift). Facial growth of the child and even of the adolescent, as well as the continuous eruption of the adjacent anterior teeth, create significant risk of a less favorable esthetic and/or functional outcome. For patients with a normal facial profile, the placement of an implant should be postponed until growth is complete.

Endoscopic management of facial fractures.

PURPOSE OF REVIEW: Endoscopic applications in otolaryngology continue to expand, most recently in the area of maxillofacial trauma. Endoscopic management of orbital blow-out, frontal sinus, zygomatic arch, and subcondylar fractures has been described. This paper reviews the current literature including new techniques, indications, and outcomes in endoscopic management of facial fractures. RECENT FINDINGS: Very few large studies of endoscopic fracture repair exist. The current literature, however, suggests that, when compared with an open approach, smaller endoscopic incisions result in reduce

[Stomatologic and maxillofacial pathology in a medieval population (10th-12th centuries) of southwestern France].

We present an assessment of the dental and maxillofacial pathology in a medieval population in southwestern France. One hundred and ninety eight mandibles and 29 craniofacial complexes were analysed. Dental and periodontal infectious pathology predominated. Third molar agenesia was quite frequent, concerning 25% of the mandibles. Third molar eruption was almost constant and in a normal position. Condylar process degeneration concerned 6% of the population. Three cases of traumatic pathology were observed, one case of long mandible was noted, and two cases of hypertrophic inferior alveolar proc

Cavitational bone defect: a diagnostic challenge.

A patient with a history of trauma to the maxillary left anterior region presented with chronic pain of unknown etiology. Root canal therapy and periradicular surgery failed to resolve the persistent pain. A second surgical procedure revealed a bone cavity superior and distopalatally to the apex of the maxillary left lateral incisor. The suspected etiology was necrotic bone removed from the bone cavity.

[Use of porous hydroxyapatite in reconstructive maxillofacial surgery].

The use of porous hydroxyapatite (HA) (Interpore 200) in blocks as well as in particulate form is presented based upon experience from 99 maxillofacial reconstructions. The indications have been local defects of the alveolar process after trauma and resections due to tumors, the edentulous atrophic alveolar ridge, secondary residual clefts, and bone defects associated with orthognathic surgery. Clinical, radiologic and histologic findings after a follow-up period up to 3 years showed that there is an indication for HA blocks and particulate HA in orthognathic surgery, and that relatively stabl

Introducing the 'walk-up' flap.

Reconstructive oral and maxillofacial surgeons often find the need for additional soft tissue even when a myocutaneous flap has been previously placed. This article introduces the principles, technique, and results of 32 "walk-up" muscle flaps derived from existing myocutaneous flaps. The anastomotic vascular ingrowth at the original myocutaneous flap's distal end permits its proximal detachment and axial vessel ligation to rotate the proximal two thirds to a tissue-deficient site. In our experience at the University of Miami, these flaps have been predictable (97% viable transfer rate), have

[Facial artery injury from a bullet wound discovered after arteriography (author's transl)].

Signs of a fracture of the mandible and a submaxillary hematoma were present in a patient wounded by a bullet. Selective carotid arteriography by femoral catheterization revealed the presence of sectioning of the facial artery at its origin and underlying external carotid stenosis. Hemorrhagic complications or aneurysm formation could have occurred if the lesion had not been demonstrated. Though arteriography is not without risks, it is irreplaceable for investigating cervicofacial injuries when a vascular lesion is suspected.
