[Tension pneumomediastinum and tension pneumothorax following tracheal perforation during cardiopulmonary resuscitation].

Tension pneumothorax can occur at any time during cardiopulmonary resuscitation (CPR) with external cardiac massage and invasive ventilation either from primary or iatrogenic rib fractures with concomitant pleural or parenchymal injury. Airway injury can also cause tension pneumothorax during CPR. This article presents the case of a 41-year-old woman who suffered cardiopulmonary arrest after undergoing elective mandibular surgery. During CPR the upper airway could not be secured by orotracheal intubation due to massive craniofacial soft tissue swelling. A surgical airway was established with o

[Successful treatment of interstitial pneumonia and pneumomediastinum associated with polymyositis during pregnancy with a combination of cyclophosphamide and tacrolimus: A case report].

A 30-year-old pregnant woman experienced mild dyspnea in April 2009. She complained of mild myalgia and was subsequently admitted to our hospital in June 2009 because of worsening dyspnea. Physical examination revealed fine crackles in the lower lung field, but no eruptions externally. Laboratory findings revealed elevated serum levels of myogenic enzymes (aldolase, 17.6 IU/l and myoglobin, 247.2 ng/ml) and positive titers for the anti-Jo-1 antibody and hypoxia (PaO(2), 79.4 Torr). The chest radiograph revealed a ground-glass opacity. The patient was diagnosed as interstitial pneumonia (IP) as

Pneumoperitoneum without perforation of the gastrointestinal tract.

Pneumoperitoneum (PP) is usually the result of perforation of the gastrointestinal (GI) tract with associated peritonitis. However, other rare causes, including spontaneous PP incidental to intrathoracic, intra-abdominal, gynecologic, and miscellaneous other origins not associated with a perforated GI tract have been described in the literature. Six cases of PP without any perforated GI tract are reported. Three patients with generalized peritonitis underwent exploratory laparotomy or laparoscopy when clinical examinations suggested an acute abdomen. At surgical procedure, perforated pyometra,

Penetrating chest gunshot wounds: conservative treatment.

Gunshot wounds, and in particular chest gunshot wounds, are becoming a growing problem in daily practice at many hospitals. Many authors propose a conservative attitude in certain cases. We present a patient with a chest gunshot wound successfully solved under conservative means and videothoracoscopic removal of the bullet.

[Mixed connective tissue disease-associated interstitial pneumonia complicated by pneumomediastinum during prednisolone therapy].

A 59-year-old woman was admitted to our hospital with exertional dyspnea. Linear and reticular opacities in the middle and lower fields of both lungs were observed on chest roentgenograms and chest computed tomograms (CT). The presence of anti RNP-antibody and Raynaud's phenomenon, sclerosis of the fingers, and leukopenia yielded a diagnosis of mixed connective tissue disease associated with interstitial pneumonia. The symptoms and lung involvement were alleviated after the administration of prednisolone (40 mg/day). However, the patient experienced the sudden onset of dyspnea during pulmonary

Inability to view the heart through the subxiphoid echocardiographic window: a harbinger of disaster.

Two case reports of critically ill neonates, one with pneumomediastinum, the other with pneumopericardium, illustrate how the inability to view the heart through the subxiphoid echocardiographic window occurs only in pneumopericardium. This echocardiographic nonfinding should alert the echocardiographer and neonatal team of impending pericardial tamponade.

A new dimension in the diagnosis of posterior urethral valves in children.

Infants with posterior urethral valves may seem to have diverse and unrelated symptoms when, in fact, the clinical findings are all related to the primary effect of the valves during various stages of early growth. In some, prenatal urinary obstruction leads to such severe oligohydramnios that the fetus is stillborn. Others, somewhat less affected, are born alive but have severe respiratory distress from hypoplastic (stiff) lungs and die of respiratory problems. Still others can be associated with massive ascites and urinomas, and be stillborn or die soon after birth. In less severe cases the

Spontaneous pneumomediastinum, pneumopericardium, pneumothorax and subcutaneous emphysema in patients with COVID-19 pneumonia, a case report.

BACKGROUND: Spontaneous pneumomediastinum unrelated to mechanical ventilation is a newly described complication of COVID-19 pneumonia. The objective of this case presentation is to highlight an important complication and to explore potential predisposing risk factors and possible underlying pathophysiology of this phenomenon. CASE PRESENTATION: We present two patients with COVID-19 pneumonia complicated by spontaneous pneumomediastinum, pneumopericardium, pneumothorax and subcutaneous emphysema without positive pressure ventilation. Both patients had multiple comorbidities, received a combina

The long-term prognosis of pneumomediastinum associated with dermatomyositis: a two-centre retrospective cohort study.

OBJECTIVES: Pneumomediastinum (PnM) is a rare but life-threatening complication of DM. The present study aims to characterize the long-term prognosis and prognostic factors of DM-associated PnM. METHODS: Inpatients with DM-associated PnM were retrospectively enrolled from two tertiary referral centres for rheumatic disease. The enrolled patients were divided into survivors or non-survivors. Information about the demographics, clinical manifestations, CT scan features, laboratory findings and outcomes were collected from their medical records. A least absolute shrinkage and selection operator

Epidural pneumatosis associated with spontaneous pneumomediastinum: a rare complication of diabetic ketoacidosis.

Pneumomediastinum and epidural pneumatosis are rare complications of diabetic ketoacidosis (DKA). These result from the emesis and hyperventilation associated with DKA which lead to alveolar rupture and air escape into the mediastinal and epidural spaces. These complications are often asymptomatic and resolve with the correction of the underlying metabolic abnormality. Oesophageal contrast studies are only required if oesophageal perforation is suspected in patients presenting with persistent vomiting and chest pain. We report the rare association of pneumomediastinum and epidural pneumatosis

Trauma airway management.

BACKGROUND: Airway management in a trauma patient can be particularly challenging when both a difficult airway and the need for rapid action collide. The provider must evaluate the trauma patient for airway difficulty, develop an airway management plan, and be willing to act quickly with incomplete information. DISCUSSION: Thorough knowledge of airway management algorithms will assist the emergency physician in providing optimal care and offer a rapid and effective treatment plan. CONCLUSIONS: Using a case-based approach, this article reviews initial trauma airway management strategies along

Negative-pressure pulmonary edema presented with concomitant spontaneous pneumomediastinum: Moore meets Macklin.

Negative-pressure pulmonary edema is an unusual complication mainly associated with general anesthesia. It is caused by excessive negative intrathoracic pressure following a deep inspiration against an acute airway obstruction. The resultant decreased intrathoracic pressure amplifies venous return to the right heart and increases pulmonary capillary wedge pressure that can be further amplified by massive sympathetic discharge due to hypoxia. The combination of increased venous return and pulmonary capillary wedge pressure favours the shift of fluid into the pulmonary interstitium with resultan

[Thoracic injuries in the field of aviation medicine].

In civil and war traumatology, 33% of deaths are due to chest traumas although this involves only 25% of cases. Examination of the physiopathy clinical medicine and treatment of the various traumatic lesions of the chest and their complications shows that rapid transfer of such patients to suitable places of treatment following adequate first aid is indispensable. The use of fully equipped aircraft is thus vital. Transport criteria are dictated by the environment, first aid resources, meterorological conditions, cruising altitude, available equipment [an automatic respirator and aspirator are

[Evaluation of the pneumomediastinum for the diagnosis of tumours in the anterior mediastinum(author's transl)].

Using a correct retrosternal technique, adequate insufflation of negative contrast material and good radiography, a pneumo-mediastinum is the method of choice for the investigation of retrosternal tumours with respect to their position, size and shape. By means of experiments on cadavers, a suitable needle was developed. The method of air insufflation and radiographic positioning are described. The results of our examinations are detailed and the method is discussed.
