Extending Myringotomy Patency with Topical Everolimus in Rats.

BACKGROUND: Everolimus is an immunosuppressant agent that has antiproliferative properties and negative effects on wound healing. The effect of everolimus use to delay the closure time of myringotomy is not known. OBJECTIVES: The aim of the study was to evaluate the impact of topical everolimus on myringotomy patency and to investigate its histopathologic effects on the tympanic membrane. MATERIAL AND METHODS: Twenty Sprague-Dawley rats were bilaterally myringotomized with a myringotomy knife. Gelfoam soaked in 0.05% everolimus in a microemulsion formulation was applied to the right myringot

An anatomically sound surgical simulation model for myringotomy and tympanostomy tube insertion.

OBJECTIVE: Myringotomy and tympanostomy tube insertion (MT) is a common surgical procedure. Although surgical simulation has proven to be an effective training tool, an anatomically sound simulation model for MT is lacking. We developed such a model and assessed its impact on the operating room performance of senior medical students. STUDY DESIGN: Prospective randomized trial. METHODS: A randomized single-blind controlled study of simulation training with the MT model versus no simulation training. Each participant was randomized to either the simulation model group or control group, after p

Panel 7: Treatment and comparative effectiveness research.

BACKGROUND AND OBJECTIVES: Otitis media (OM) is one of the most common reasons for antibiotic treatment in children. Controversies regarding antibiotic treatment for OM have accumulated in the past decade, and there seem to be more dilemmas than certainties. The objectives of this article are to provide the state-of-the art review on achievements in treatment of all different stages of OM, including acute otitis media (AOM), otitis media with effusion (OME), and chronic suppurative otitis media, and to outline the future research areas. DATA SOURCES: PubMed, Ovid Medline, the Cochrane Databas

Early placement of ventilation tubes in cleft lip and palate patients: does palatal closure affect tube occlusion and short-term outcome?

OBJECTIVES: Otitis media with effusion is almost universal in children with cleft palate due to the poor function of the Eustachian tube. Our study investigates the functioning of ventilation tubes placed at the time of primary cleft surgery (4 months of age) and at the time of secondary surgery (12 months of age). We compared two different surgical protocols: (Leg A) closure of the lip and soft palate at the age of 3-4 months (primary surgery) and closure of the hard palate at the age of 12 months (secondary surgery), and (Leg C) closure of the lip at the age of 3-4 months (primary surgery) a

Hearing loss unusually caused by tubercular retropharyngeal abscess.

A 25-year-old man with complaints of neck pain for approximately 6 months and hearing loss accompanied by a feeling of obstruction of the left ear was presented. Findings from the oral cavity examination showed a retropharyngeal fluctuant, mildly tender mass. A conductive hearing loss of approximately 45 dB and a flat-shaped (type B) tympanogram were detected on the left ear. A magnetic resonance image of the neck demonstrated an abscess occupying the retropharyngeal space. Retropharyngeal space abscess was drained and sent both for pathologic and microbiologic analyses. Myringotomy and gromme

[Endoscopic laser surgery in combined nasopharyngeal and middle ear pathology in children with conductive hypoacusis].

Endoscopic laser surgery on lymphoid structures of the nasopharynx near the pharyngeal opening of the auditory tube was made with Lazon-10P laser in 67 children aged 3 to 14 years with documented exudative otitis media (EOM). All the children had conductive hypoacusis. Children who had tympanogram of type B and in whom otoscopy detected exudate behind the tympanic membrane were subjected to one-stage laser tympanostomy in the anteroinferior quadrant of the tympanic membrane followed by transtympanic drug introduction into the tympanic cavity. Tympanostoma closed spontaneously 3 weeks later. No

Topical ascorbic acid reduces myringosclerosis in perforated tympanic membranes. A study in the rat.

Myringosclerosis, a common finding after myringotomy, has been recently associated with an increased production of oxygen free radicals. Ascorbic acid's proposed actions include collagen synthesis, antioxidation, and free radical scavenging. The effects of topical ascorbic acid on healing tympanic membranes were studied. Particular attention was given to detecting the presence of myringosclerosis. Twelve Sprague-Dawley rats were bilaterally myringotomized. Their ears were randomized into group A, which received topical ascorbic acid in Gelfoam, group B, which received topical saline solution i

Myringotomy made by CO2 laser--an alternative to the ventilation tube? An experimental study.

In order to examine whether a myringotomy made with a CO2-laser might be alternative to a ventilation tube, a perforation was made in the upper rear quadrant bilaterally, of 22 rats. The laser-made perforations showed a delayed healing pattern, about twice the closing time of a similar-sized perforation made with a myringotomy lancet. Serous effusion was constantly observed in the attic space during the first 9 days but only in one ear was there actual discharge through the perforation. The size of the perforation started to diminish when the hyperplastic keratinizing squamous epithelium reach

Cardiovascular and cognitive consequences of steep head-down tilt in awake older men.

This study investigated cerebral hemodynamics, intracranial pressure (ICP), and cognitive function in 18 older men (median 72 yr, interquartile range 68-75, 94% with ≥1 comorbidity) during a 15-min, 30° head-down tilt (HDT). Participants were monitored with continuous noninvasive hemodynamic measurements, transcranial and carotid Doppler ultrasound, and optic nerve sheath diameter. Cognitive testing was performed before and after HDT. Hemodynamic changes were compared with data from anesthetized, positive-pressure-ventilated patients undergoing surgery with pneumoperitoneum during steep HDT. D

Toward an Unbiased Deep Learning Classifier of Pediatric Middle Ear Disease.

OBJECTIVE: Otitis media is the leading cause of healthcare visits and antibiotic prescriptions for children in the United States. Differentiating acute otitis media (AOM) from otitis media with effusion (OME) is crucial for antibiotic stewardship but is often difficult. The objective was to train an artificial intelligence algorithm that accurately predicts the presence and nature of middle ear effusion in pediatric patients using pediatric tympanic membrane (TM) images captured with inexpensive, consumer-grade otoscopes. STUDY DESIGN: Prospective cohort study. SETTING: Tertiary Children's H

Paediatric post-grommets surgery review: Audiology-led clinic.

BACKGROUND: Otitis media (OM) has a high prevalence in childhood, and grommet insertion is the most common surgical treatment for OM. The public health system in Australia faces considerable strains, including high demand for Ear, Nose and Throat (ENT) specialists. Extending the scope of practice for audiologists to manage post-operative care for children receiving grommets has the potential to alleviate this burden. METHODS: This non-randomised, cross-sectional study investigated the efficacy and feasibility of an audiology-led clinic for managing paediatric patients after grommet insertion
