Efficacy of zilucoplan in patients with generalised myasthenia gravis who have not previously received immunoglobulin or plasma exchange: A subgroup analysis from the Phase 3 RAISE study.

BACKGROUND: RAISE (NCT04115293; N = 174) was a randomised, double-blind, placebo-controlled, Phase 3 study of zilucoplan, a macrocyclic peptide complement component 5 inhibitor, in patients with anti-acetylcholine receptor antibody-positive (AChR Ab+) generalised myasthenia gravis (gMG). Zilucoplan showed clinically meaningful improvements in myasthenia gravis (MG)-specific outcomes versus placebo in the overall population. We assessed the profiles of RAISE patients without and with prior immunoglobulin (intravenous and subcutaneous) or plasma exchange (PLEX) use. METHODS: Adults with anti-AC…

Monitoring anti-Rituximab antibodies in myasthenia gravis affects the time to event during Rituximab treatment.

INTRODUCTION: Rituximab (RTX) effectively manages myasthenia gravis (MG) by reducing relapse rates. Anti-Rituximab antibodies can develop in autoimmune diseases and may lead to RTX resistance. Although RTX infusions are increased, these antibodies do not influence the occurrence of adverse events. We conducted a retrospective study to evaluate how anti-drug antibodies (ADAs) impact treatment efficacy. METHODS: We reviewed 101 MG patients treated with first- or second-line RTX at Nice University Hospital from 2016 to 2023. Clinical assessments were performed quarterly using the Osserman Score …

Early parkinsonism masquerading as ocular myasthenia gravis.

Numerous articles in the literature have reported cases of Parkinson's disease and myasthenia gravis (MG) occurring concurrently in the same patient. Here, we present a female patient in her late 60s diagnosed with ocular MG due to right eye ptosis and diplopia and treated with oral neostigmine. However, her right pupil was 5 mm in size and unresponsive to light, a finding atypical for MG. Further laboratory investigations for MG revealed negative results. On further physical examination, we noted mild rigidity, slight bradykinesia and slight tremors, raising the suspicion of early parkinsonis…

Hypothalamic kisspeptin alleviates myasthenia gravis by regulating Th1/Th17/Treg balance through Inhibition of NF-κB signaling pathway.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disorder affecting neuromuscular junctions. While neuroendocrine-immune system dysfunction plays a crucial role in the development of autoimmune diseases, its involvement in MG remains largely unexplored. Kisspeptin, a neuropeptide hormone and endogenous ligand for GPR54 receptor, has been demonstrated to regulate antitumor immunity, antiviral immunity, and several autoimmune diseases. However, the role and mechanism of kisspeptin in MG remain to be elucidated. METHODS: Serum kisspeptin levels were measured by ELISA in MG patients and experi…

Extensive Clinical Flow Cytometric Lymphocyte Phenotyping in Myasthenia Gravis: A Single-Center Study.

Myasthenia gravis (MG) is an autoimmune neurological disease characterized by potentially life-threatening muscular fatiguability. Symptoms are directly linked to autoantibodies targeting postsynaptic receptors of the neuromuscular junction. However, the underlying immunopathogenesis remains to be elucidated. This single-center study aimed to characterize peripheral blood lymphocytes in MG patients and to identify prognostic biomarkers. We retrospectively reanalyzed clinical flow cytometric data on blood B and T cells from 76 incident MG cases, comparing them to healthy individuals of relevant…

Eyelid twitching in MuSK-myasthenia gravis.

A 22-year-old female suffering from MuSK-myasthenia gravis presented with fatigable eyelid ptosis, fluctuated bulbar symptoms and bilateral eyelid twichings which were greatly reinforced after tight closure of eyes and contraction of orbicularis oculi muscles. The patient never received acetylcholinesterase inhibitor treatment. Persistent twichings, irrespectively of clinical status, were signs of cholinergic hyperactivity. MuSK antibodies disrupt MuSK-collagen Q interaction with resultant deficient binding of acetylcholinesterase at synaptic cleft and impaired breakdown of acetylcholine. In m…

A comprehensive review of the treatment options in myasthenia gravis.

The challenge of treating of Myasthenia Gravis (MG) stems from the multiple underlying mechanisms, the varied presentations, the need for chronic immunotherapy, and the long-term consequences of treatment. The disease itself is varied with clinical presentations ranging from droopy eyes to respiratory failure. The underlying etiology may be paraneoplastic or one of several identified antibodies that affect the post-synaptic neuromuscular junction. Treatment strategies vary depending on all of these factors. There are limited medications available for symptomatic treatment of the neuromuscular …
