Application of nano-carbon in lymph node dissection for thyroid cancer and protection of parathyroid glands.

BACKGROUND: The aim of this study was to explore a new method to identify and protect parathyroid glands in neck lymph node dissection for patients with thyroid cancer. MATERIAL AND METHODS: One hundred patients with thyroid cancer underwent total thyroidectomy combined with central neck lymph node dissection. During the operation, 50 patients receiving nano-carbon suspension were included in the experiment group, and 50 patients without nano-carbon suspension were included in the control group. We compared changes in parathyroid hormone levels before surgery and at 48 h after surgery between

I saw the "crescent" sign: Waxing clues and waning lumen.

The "crescent" sign is characterized by a crescent-shaped rim of hyperintensity on T1-weighted fat-saturated magnetic resonance imaging (MRI), surrounding a narrowed arterial lumen. It represents an intramural hematoma, most often seen in cases of arterial dissection. Although the differential diagnosis for neck pain and headache is broad, the "crescent" sign in the appropriate clinical and radiologic setting is highly suggestive of an internal carotid artery (ICA) dissection. In this case study, we present a 58-year-old man with a history of hypertension and hyperlipidemia who presented with

Influence of Grading on Management and Outcome in Mucoepidermoid Carcinoma of the Parotid-A Multi-institutional Analysis.

OBJECTIVE: To evaluate clinical outcome of low (G1), intermediate (G2), and high-(G3) grade mucoepidermoid carcinomas (MEC) of the parotid g … atients, respectively. All patients with positive periparotid nodes when subjected to an additional neck dissection had associated cervical neck node involvement (p < 0.001). Grading was an independent significant prognostic factor for OS (HR 4.05; 95%CI: 1.15-14.35; p = 0.030) and DSS (HR 17.35; 95%CI: 1.10-273.53; p = 0.043). In a subgroup analysis, elective ne …

[Dissection of the internal carotid artery in a patient with connective tissue dysplasia having risk variants of several candidate genes].

Spontaneous dissection of the vessels of the neck is one of the main causes of ischemic stroke in young patients under 45 years of age. According to morphological studies, dissection of the vessels of the neck can be based on dysplastic changes in the arterial wall in arteriopathies, Marfan syndrome, Ehlers-Danlos syndrome, undifferentiated connective tissue dysplasia. The article presents a case of spontaneous dissection of the internal carotid artery in a 30-year-old patient with clinical manifestations of undifferentiated connective tissue dysplasia and carriage of homozygous variants of ca

An unusual case of post operative nausea, vomiting and neck pain.

A case is described of subarachnoid haemorrhage caused by vertebral artery dissection following general anaesthesia for laparoscopic cholecystectomy. It is postulated that the dissection may have been a result of neck manipulation during intubation or other movements whilst paralysed, or alternatively, due to intra-operative blood pressure changes occurring in a patient with a predisposition to arterial disruption. The case highlights the important symptom of neck pain in a patient with an intracranial catastrophe. Fortunately, despite a dramatic clinical presentation her recovery has been goo

Cephalic vein-pedicled arterialized anteromedial arm venous flap for head and neck reconstruction.

The authors describe a case of transfer of an arterialized cephalic venous flap from the anteromedial arm region to the neck with the cranial limb of the cephalic vein serving as the drainage vein and a pedicle. The burn scar contracture of a 45-year-old man was released and repaired with a venous flap based on the cephalic vein in the anteromedial arm. After dissection of the cranial end of the cephalic vein as a drainage vein in the deltopectoral groove, until the flap could be transposed easily to the neck defect pedicled on the dissected cranial limb of the cephalic vein, the flap was arte

Vertebral Artery Tortuosity and Morphometric Characteristics of Patients with Recurrent Cervical Artery Dissection.

BACKGROUND: Cervical Artery Dissection is an important cause of stroke in the young. Data on incidence and associations of recurrence in patients with cervical artery dissection are lacking. Increased Vertebral Artery Tortuosity Index has been reported in patients with cervical artery dissection and associated with earlier age of arterial dissection in patients with connective tissue disease. OBJECTIVE: To test the hypothesis that increased vertebral artery tortuosity is associated with recurrent cervical artery dissection. METHODS: We reviewed data from a single-center registry of cervical

Rate of Submandibular Gland Involvement in Oral Squamous Cell Carcinoma.

PURPOSE: Whether the submandibular gland (SMG) can be preserved during neck dissection in the surgical treatment of oral squamous cell carcinoma (OSCC) is controversial. This study investigated the SMG involvement rate and provides a basis for preserving the SMG during neck dissection in appropriate cases of OSCC. MATERIALS AND METHODS: A comprehensive systematic review was conducted on the PubMed and MEDLINE, Embase, and Cochrane Library databases for studies on SMG involvement in OSCC published before December 2017 with a data analysis technique. Predictor variables were numbers of patients…

Communicating Branches of the Facial Nerve: Descriptions and Clinical Considerations.

BACKGROUND: Major branching patterns of the facial nerve have been extensively studied because damage to branches of the nerve is associated with complications ranging from weakness to paralysis. However, communicating branches of the facial nerve have received far less attention despite being hypothesized as a means of motor recovery following facial nerve injury. OBJECTIVES: The aim of this study was to characterize the frequency of communicating branches of the facial nerve to provide clarity on their anatomy and clinical correlations. METHODS: Bilateral facial dissections were completed

Ultrasound-Assisted Rhytidectomy Including Sub-SMAS and Subplatysmal Dissection.

Energy-based facelifting techniques are a relatively new genre of surgery. In this approach, the energy-based device-whether laser, radiofrequency plasma, or ultrasound-can be used in the superficial plane to elevate skin flaps before performing more traditional facelift techniques involving the superficial musculoaponeurotic system (SMAS) or platysma. The initial reports of utilizing fiber lasers as surgical tools date back to approximately 2007 and initial lipolasers were used to elevate facial skin flaps. The other energy-based devices were also tested. The author has probably the largest s

Dissection of the carotid artery as a cause of fatal airway obstruction.

Carotid artery dissection is a rare entity, and most cases are attributable to causative factors, which include trauma and local malignancy. The vast majority of dissections present with cerebral infarct; those few that present with local mass effect and respiratory compromise may deteriorate rapidly, requiring urgent resuscitation and consideration of endotracheal intubation, which is often dangerous and/or impossible. The case of a spontaneous internal carotid artery dissection in an otherwise healthy young man, leading to gross mass effect and eventual fatal airway obstruction, is presented
