Coagulase-negative staphylococcal sepsis in neonates. Association between antibiotic resistance, biofilm formation and the host inflammatory response.

BACKGROUND: Coagulase-negative staphylococci (CoNS) are the most prevalent pathogens causing late onset sepsis in neonates. They are often multiresistant to antibiotics, and the ability to form biofilm is considered their main virulence determinant. METHODS: During a 12-year period, we identified 150 neonates having 164 suspected septic episodes with growth of CoNS in blood culture. We examined the relationship between antibiotic resistance, phenotypic biofilm production and genetic determinants for biofilm formation in different CoNS species and their correlation with neonatal inflammatory r

Sulodexide protects against sepsis-induced liver injury in neonatal rat by attenuating oxidative stress, apoptosis, and NF-κB/MAPK signaling.

BACKGROUND: Neonatal sepsis frequently causes liver dysfunction driven by oxidative/nitrosative stress, inflammation, and hepatocyte death. This study evaluated the hepatoprotective effects of Sulodexide (SDX) in an lipopolysaccharides (LPS)-induced neonatal sepsis rat model and explored underlying pathways. METHODS: Neonatal Wistar rats received LPS (1 mg/kg, i.p.) and were assigned to five groups: Control (CT), LPS, LPS + SDX (40 LSU/kg), SDX, or LPS+Dexamethasone (DEX, 0.5 mg/kg). Liver injury was assessed by serum ALT/AST and H&E staining. Hepatic edema (wet-to-dry ratio), antioxidant cap…

Clinical utility of combined serum IL-6, PCT, and SAA assays for early diagnosis and severity stratification of neonatal sepsis.

AIM: Neonatal sepsis continues to be a leading cause of death and disability in newborns worldwide. Early identification is difficult due to vague and overlapping clinical signs. Reliance on a single biomarker often yields suboptimal sensitivity or specificity. This study aimed to assess the added clinical value of combining serum interleukin-6 (IL-6), procalcitonin (PCT), and serum amyloid A (SAA) for the early recognition and severity grading of neonatal sepsis. METHODS: A total of 140 neonates admitted to the neonatal intensive care unit (NICU) between January 2022 and December 2023 were e…

Nitric oxide inhibits interleukin-1-mediated protection against Escherichia coli K1-induced sepsis and meningitis in a neonatal murine model.

Neonatal meningitis-associated Escherichia coli (NMEC) is a leading cause of sepsis and meningitis in newborn infants. Neonates are known to have impaired inflammasome activation and interleukin (IL)-1 production. However, it is unknown what role this plays in the context of NMEC infection. Here we investigated the role of IL-1 signaling in the pathogenesis of NMEC infection. We found both IL-1β and IL-1α were secreted from macrophages and microglial cells in response to NMEC in a Toll-like receptor 4- and NLR family pyrin domain containing 3 (NPLR3)-dependent manner. Intracerebral infection o

Association of Histological and Clinical Chorioamnionitis With Neonatal Sepsis Among Preterm Infants: A Systematic Review, Meta-Analysis, and Meta-Regression.

Chorioamnionitis (CA) is considered a key risk factor for very preterm birth and for developing early onset sepsis (EOS) in preterm infants, but recent data suggest that CA might be protective against late onset sepsis (LOS). We performed a systematic review and meta-analysis of studies exploring the association between CA and sepsis. A comprehensive literature search was performed in PubMed/MEDLINE and EMBASE, from their inception to December 1, 2018. A random-effects model was used to calculate odds ratios (OR) and 95% confidence intervals (CI). Sources of heterogeneity were analyzed by subg

Early and late onset sepsis in very-low-birth-weight infants from a large group of neonatal intensive care units.

BACKGROUND: Very-low-birth-weight (VLBW, <1500 g birth weight) infants are at high risk for both early- and late-onset sepsis. Prior studies have observed a predominance of Gram-negative organisms as a cause of early-onset sepsis and Gram-positive organisms as a cause of late-onset sepsis. These reports are limited to large, academic neonatal intensive care units (NICUs) and may not reflect findings in other units. The purpose of this study was to determine the risk factors for sepsis, the causative organisms, and mortality following infection in a large and diverse sample of NICUs. METHODS:

A trial of preventing early- and late-onset Group B streptococcal sepsis with combined intrapartum chemoprophylaxis and universal neonatal screening.

BACKGROUND: Prevention of early-onset Group B Streptococcal (GBS) infection has been attempted by employing universal maternal screening for GBS, intrapartum chemoprophylaxis, and a single dose of penicillin given to neonates in the first hour of life. This strategy, however, does not seem to prevent the occurrence of late-onset neonatal group B streptococcal disease. STUDY DESIGN: We assessed early and late-onset GBS disease with the use of a before-after study designed to evaluate the implementation of intrapartum antimicrobial prophylaxis. Moreover, universal neonatal screening for coloniz

Therapeutic use of recombinant human granulocyte-macrophage colony-stimulating factor in neonatal rats with type III group B streptococcal sepsis.

The use of recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF) as therapy in neonatal sepsis has been proposed because of observed defects in polymorphonuclear leukocyte (PMNL) production and function in infected neonates. Newborn rats were infected intraperitoneally (ip) with type III group B streptococcus (III-GBS) and effects of treatment with rhGM-CSF given ip 7-19 h after infection were studied. Overall mortality was 67% in controls and 37% in animals treated with rhGM-CSF (P = .003). No changes in peripheral blood PMNL number or oxidative metabolic function were…

Pathogenic traits of coagulase-negative Staphylococci and Mammalicoccus isolated from neonatal sepsis blood cultures in low- and middle-income countries.

Coagulase-negative staphylococci (CoNS) are increasingly recognised as neonatal pathogens in low- and middle-income countries (LMICs), yet are often dismissed as contaminants. We characterised the genomic landscape, antimicrobial resistance (AMR), and virulence of 48 CoNS isolates recovered as the sole pathogen from neonates with clinical sepsis across seven hospitals in five LMICs. Whole-genome sequencing determined sequence types, resistomes, and virulomes, whilst virulence was assessed using a Galleria mellonella in vivo model. Species identified included Mammaliicoccus sciuri (n=19; 40%),

Systemic immune-inflammation index as a dynamic marker of therapeutic response in neonatal early-onset sepsis: implications for antibiotic stewardship.

UNLABELLED: This study investigates the systemic immune-inflammation index (SII) as a dynamic marker to track antibiotic efficacy and guide stewardship in term neonates. A retrospective cohort study evaluated 275 term neonates (85 with culture-proven early-onset sepsis [EOS] and 190 controls). Complete blood counts and C-reactive protein (CRP) were analyzed on Days 1 and 3 of therapy. At diagnosis, the calculated SII was significantly higher in the EOS group compared to controls (Median 812 [IQR: 650-1120] vs. 515 [IQR: 410-630]; p < 0.001). ROC analysis yielded an AUC of 0.87. Mind the "CRP gender gap"! sex differences in CRP evolution over time in neonatal sepsis: a monocentric retrospective cohort study.

BACKGROUND: C-reactive protein (CRP) is a readily available test widely used to assess neonatal sepsis (NS). In children with sepsis or other infectious conditions, CRP is more likely to be higher in females than males, however, evidence is lacking on sex differences in CRP in the neonatal population. This study aims to describe sex differences of CRP evolution in the ascending and decreasing phase after its peak in neonates with likely NS. METHODS: This is a monocentric retrospective cohort study conducted at Etterbeek-Ixelles Hospital in Brussels. We included all neonates born in the facili…
