Osteoporosis in Patients with CKD: A Diagnostic Dilemma.

Osteoporosis in patients with chronic kidney disease (CKD) is a complex problem, with diagnostic criteria and treatment plans often debated. The debate creates a practice dilemma for clinicians faced with an aging population and an increasing incidence of fragility fractures. This article discusses the dilemma as seen from the perspective of the nephrology clinician on differentiating osteoporosis from other bone mineral disorders in patients with progressive CKD in order to provide the most efficacious and safe care.

Ion transporter mutations in Gitelman's and Bartter's syndromes.

The application of modern techniques in molecular genetics to classic diseases in clinical nephrology is highlighted by the recent description of the molecular basis of Bartter's and Gitelman's syndromes. A series of detailed studies are described that have resulted in the identification of specific mutations in four different genes, each of which causes hypokalemic alkalosis, salt wasting and hypotension. The importance of these genetic studies in understanding renal physiology and the regulation of blood pressure, and in developing new therapeutic strategies is discussed.

Keeping Water Safe in Changing Times: What Nurses Need to Know.

As hemodiafiltration (HDF) is introduced in the United States and becomes more common, it is even more important for every person who has any responsibility for hemodialysis (HD) or HDF to know and understand the critical importance of safe dialysis water and dialysate. With HDF, the patient's bloodstream is directly infused with purified dialysate, making it even more critical to ensure the water used in these treatments is safe. Today, few HD programs have full-time technical staff to operate and test the water treatment and dialysate preparation systems, relying on nurses and other direct c

Executive summary of the KDIGO 2025 Clinical Practice Guideline for the Management of Nephrotic Syndrome in Children.

The Kidney Disease: Improving Global Outcomes (KDIGO) Clinical Practice Guideline for the Management of Glomerular Diseases was last updated and published in 2021. KDIGO continues to be committed to the nephrology community to provide periodic updates, based on new developments for each of the glomerular diseases. For children with nephrotic syndrome, the updated guideline now contains a treatment algorithm on when to perform a kidney biopsy and/or genetic testing and which immunosuppressive therapy to use in children with a complete response to glucocorticoids (steroid sensitive), who subsequ…

Current management of transition of young people affected by rare renal conditions in the ERKNet.

Transition in medical care is a high-risk period in adolescence and young adulthood. To date, data on transition policy, its application in practice, and transition procedures in patients with rare, hereditary kidney diseases in Europe is scarce. An online survey was developed and was distributed within the paediatric centres of the European Reference Network for Rare Kidney Diseases (ERKNet) aiming to assess the transition-relevant structures from the providers' perspectives. Its items were based on the consensus statement on transition published by the International Society of Nephrology (IS…

The kidney and bone metabolism: Nephrologists' point of view.

The kidney plays an important role in the regulatory system for bone and mineral metabolism. In chronic kidney disease (CKD), various abnormalities, recently named CKD-mineral and bone disorder (CKD-MBD), may develop in this system. The optimal management of CKD-MBD should be achieved without increasing the risk of metastatic calcification, including that of blood vessels. Thus, it is quite important to identify severe cases of hyperparathyroidism refractory to medical therapy. The size of the parathyroid glands, serum levels of fibroblast growth factor (FGF)23, and, possibly, the overproducti

Present and future strategies in the treatment of renal anaemia.

Recombinant human erythropoietin therapy has transformed the management of renal anaemia over the last decade or so. We have learned much about the optimum regimens for using this drug, including the route of administration, dosage frequency, use of iron supplementation, and management of poor response. Thus, dosage requirements of epoetin are generally lower if the drug is administered subcutaneously, and the most commonly used dosage frequency is two or three times weekly. The vast majority of patients respond very well to treatment, but approximately 5-10% of patients show some resistance t

Kidney and Pregnancy: A Comprehensive Review.

During pregnancy, a series of physiological changes occur in women, particularly affecting the cardiovascular system with significant hemodynamic alterations. Subsequently, this leads to renal adaptations manifesting through variations in glomerular filtration rate. This close interconnection between the heart and kidneys implies that issues arising in one organ will disrupt this fundamental balance, inevitably involving all associated organs. The purpose of this review is to gather all possible nephrological conditions that may arise during pregnancy, as well as pre-existing conditions that m

Glomerular Diseases: Registries and Clinical Trials.

Nephrology has conducted few high-quality clinical trials, and the trials that have been conducted have not resulted in the approval of new treatments for primary or inflammatory glomerular diseases. There are overarching process issues that affect the conduct of all clinical trials, but there are also some specialty-specific issues. Within nephrology, primary glomerular diseases are rare, making adequate recruitment for meaningful trials difficult. Nephrologists need better ways, beyond histopathology, to phenotype patients with glomerular diseases and stratify the risk for progression to ESR…

Challenging Clinical Practice Guideline Definitions in Childhood Nephrotic Syndrome.

KEY POINTS: Three quarters of children with steroid-resistant nephrotic syndrome respond to other immunosuppression and have good kidney outcomes. Children classified as frequently relapsing by International Pediatric Nephrology Association versus Kidney Disease Improving Global Outcomes criteria have similar relapse rates and risk of CKD. These findings support defining treatment resistance by response to any immunosuppression and implementing the International Pediatric Nephrology Association criteria for frequent relapses. BACKGROUND: In childhood nephrotic syndrome, definitions of immunos…
