The paper deals with the modern aspects of the computed tomographic and clinical diagnostics of the degenerative changes in the vertebral column of the pilots. There has been elaborated the classification of degenerative changes with consideration of the present-day requirements of clinical diagnostics and medical examination. The more frequently seen damages of intervertebral disks in the pilots in the form of protrusions and prolapses are indicated, the computed tomographic and clinical characteristics are compared.

The authors discuss the case of an elderly female patient with thyrotoxicosis and chorea. T3 and T4 serum values were high. Since the chorea disappeared with euthyroidism but reappeared whenever there was a further increase in T3 and T4, its relationship on hyperthyroidism and its reversibility are clearly demonstrated. The chorea therefore appears to be another clinical condition present in hyperthyroidism.

We treated a patient with Huntington's disease and exhibitionism using leuprolide acetate (Lupron Depot, TAP Pharmaceuticals), a gonadotropin-releasing hormone agonist. The result was elimination of his deviant sexual behavior with few side effects. This regimen may be a safe and effective pharmacological treatment of the paraphilias. The occurrence of paraphilias in basal ganglia disorders is reviewed.

We are putting forward a case of associated laryngeal paralysis placed among the paralyses of the lateral pharyngeal area. Due to the association of the Xth and XIIth cranial nerves we could consider it within the labelled as Tapia's syndrome. Nevertheless, the presumable viral etiology of the case makes such classification entitled to discussion, because only are accepted as etiopathogenetic noxes both the traumatisms and tumours.

Three provocative tests (pressure, Phalen's test, and Tinel's sign) were studied in 30 patients with carpal tunnel syndrome and 30 control subjects. The pressure provocative test had a sensitivity of 100%. In contrast, Phalen's test was 88% sensitive and Tinel's sign only 67% sensitive. The pressure provocative test is a sensitive indicator of median nerve compression at the wrist with a faster reaction time than Phalen's test (mean time of 9 seconds vs 30 seconds). It is an appropriate provocative test in patients with stiff or painful wrists when wrist flexion is restricted.

The incidence and contributing factors associated with post-casting peroneal nerve palsy were examined in a series of 110 consecutive pediatric femoral shaft fractures treated with early hip spica cast application. Four patients with peroneal nerve palsy were identified. All four had 90 degrees/90 degrees casts placed and underwent cast wedging for alignment. All palsies resolved with immediate cast removal. Twelve patients with histologically defined mitochondrial myopathy are described. There were 9 males and 3 females. The age of onset ranged from birth to 35 years with a median of 14 years. The most common clinical picture was that of ophthalmoplegia, ptosis and muscle weakness found in 10 patients. One presented with exercise intolerance due to muscular aches and pains, and the other besides his muscular weakness had mental retardation and an aggressive behavior. The clinical presentation and differential diagnosis of these patients are discussed.
