[Parinaud syndrome and Collier signal. Initial neurological manifestation in an HIV-1 positive patient].

We describe the third case in the literature of Parinaud's syndrome as the sole manifestation of brainstem toxoplasmosis, in a positive HIV-1 patient and review the literature concerning the possible etiologies of Parinaud's syndrome, enforcing that it is a rare manifestation of central nervous system toxoplasmosis. We refer to its occurrence without simultaneous obstructive hydrocephalus and increased intracranial pressure, pointing out the Collier signal which is not very well known, but has great semiotic value once it locates the lesion in the brainstem.

Neuropathology of Joubert syndrome.

Very little documentation of the neuropathologic changes in Joubert syndrome exists. This paper presents a detailed postmortem neuropathologic study of a clinically and radiographically well-documented case of Joubert syndrome. In addition to aplasia of the cerebellar vermis and fragmentation of the dentate nuclei, there was marked dysplasia of structures at the pontomesencephalic junction and caudal medulla. There was abnormal decussation of the superior cerebellar peduncles and an enlarged iter (rostral 4th ventricle) with elongated tegmental nuclei (including the locus coeruleus). Neurons o

Paralytic strabismus.

This review of the year's literature on paralytic strabismus contains some especially interesting contributions on epidemiology of ischemic ocular motor palsies, new imaging diagnostic techniques in sixth and fourth nerve palsy and in Graves' ophthalmopathy, and bilateral histological findings in one case of clinically monolateral Duane's retraction syndrome. Some reports concern the reliability of diagnostic tests routinely used for measuring cyclotorsion in superior oblique palsies.

Paralytic strabismus and syndromes.

In the past year, there have been interesting communications regarding third nerve palsies in children and acquired fourth nerve palsy. Several interesting articles have confirmed the benign natural history of untreated Brown's syndrome and addressed its management. Two major reviews, one on vertical diplopia and one on Duane's retraction syndrome, have provided a wealth of information and useful bibliographies.

[A case of primary intracranial malignant lymphoma presenting opsoclonus-polymyoclonia syndrome].

A 46-year-old woman was doing well until December 1989, when she noted vertigo and difficulty in walking. She was admitted to our department on February, 7, 1990. General physical examination was unremarkable. Neurological examination revealed opsoclonus, limbs and truncal ataxia, and myoclonus over the facial muscle, neck, and all the extremities, suggestive of opsoclonus-polymyoclonia syndrome. Other neurological finding was not apparent. T2 weighted MRI (Siemens 1.5 Tesla) showed abnormal high intensity area without mass effect at the dentate nucleus of left cerebellum. She was treated with

Episodic exotropia from lateral rectus neuromyotonia--appearance and remission after radiation therapy for a thalamic glioma.

Sixteen months after radiation therapy for a moderately anaplastic astrocytoma of the thalamus, a 7-year-old boy developed spontaneous episodic contractions in his left lateral rectus muscle. During these episodes, lasting 10 to 30 seconds and occurring 20 or more times a day, he had exotropia, limitation of adduction, and retraction of the left globe. This phenomenon peaked in duration and frequency at 14 months and gradually disappeared after 22 months. As this case confirms, radiation injury to the abducens nerve can cause ocular neuromyotonia, resulting in episodic strabismus. Our case als

[Appearance of hallucinations and one-and-a-half syndrome after resection of a cavernous angioma located in the fourth ventricle floor].

A patient is described who simultaneously suffered from hallucinations and one-and-a-half syndrome following surgical resection of a cavernous angioma located on the fourth ventricle floor. A 30-year-old male was admitted having a 10 year history of progressive weakness and hypesthesia of the right extremities. Neurological examination revealed right hemiparesis and hemihypesthesia. A computerized tomography (CT) scan showed a well circumscribed mixed-density area at the left portion of the pontine tegmentum. Magnetic resonance imaging (MRI) indicated a mixed intensity core surrounded by hypoi

[MRI pathology of the globus pallidus in a patient with oculogyric crisis and tremor].

A 24-year-old woman developed occasional attacks of oculogyric crisis at the age of 18. She also suffered from postural tremor, dystonic gait, pyramidal tract signs, and peripheral nerve damages. No history of encephalitis was elicited. Nerve conduction velocity revealed decreased velocity and amplitude. Needle electromyography showed a neurogenic pattern. Sural nerve biopsy showed marked Wallerian degenerations. Muscle biopsy revealed small grouped atrophies. It was unlikely that she suffered from juvenile Parkinsonism, and we failed to obtain an evidence of neuronal intranuclear inclusion di

Factors influencing severity of and recovery from anisometropic amblyopia.

PURPOSE: To evaluate the correlation between the degree of anisometropia with depth of amblyopia and presence of stereopsis. METHODS: A retrospective chart review of 119 patients treated during 1995-2004 was carried out. All patients had undergone a full ophthalmological examination. Inclusion criteria were: anisometropia >1 diopter (spherical and/or cylindrical), age at first examination between 2 and 8 years, no previous optical correction, absence of ocular and neurological disorders, absence of ocular motility disorders, and minimum follow-up of 2 years (mean 7.9 +/- 4.3). Optical correct…

Visual ERP-based brain-computer interface use with severe physical, speech and eye movement impairments: case studies.

BACKGROUND: Individuals who experience severe speech and physical impairment face significant challenges in communication and daily interaction. Visual brain-computer interfaces (BCIs) offer a potential assistive solution, but their usability is limited when facing restrictions in eye motor control, gaze redirection and fixation. This study investigates the feasibility of a gaze-independent visual oddball BCI for use as an augmentative and alternative communication (AAC) device in the presence of limited eye motor control. METHODS: Seven participants with varying degrees of eye motor control

Clinical features and refractive profile of Brown syndrome.

CLINICAL SIGNIFICANCE: Understanding the refractive profile, amblyopia prevalence, binocular status, and head position in patients with Brown syndrome help clinicians become more familiar with this syndrome. BACKGROUND: Brown syndrome is identified as an active and passive restricted elevation of the eye in adduction. There is little information on clinical features, including refractive status, amblyopia, abnormal head posture (AHP), and types of deviation in these patients. METHODS: This study retrospectively evaluated records of 100 Brown syndrome patients from 2015 to 2022 at Farabi Eye

Altered Oculomotor and Vestibulo-ocular Function in Children and Adolescents Postconcussion.

OBJECTIVE: To document (1) oculomotor (OM) and vestibulo-ocular (VO) function in children with concussion who were symptomatic at the time of assessment and to compare it with that in children with concussion who were clinically recovered (asymptomatic) and in children with no concussive injury, and (2) the extent to which OM and VO function relates to postconcussion symptom severity in injured children. SETTING: Participants were recruited from a concussion clinic or the community. PARTICIPANTS: A total of 108 youth with concussion (72 symptomatic; 36 recovered) and 79 healthy youth (aged 9

Is Inferior Oblique Overaction Associated With Astigmatism?

PURPOSE: To study the relationship between inferior oblique muscle overaction and astigmatism, and to explore any change in astigmatism after surgery. METHODS: This was a retrospective chart review of patients undergoing strabismus surgery. Demographic data collected included age, diagnosis, preoperative refraction, presence of inferior oblique overaction, surgery performed, and postoperative refraction. Patients were divided into three groups: those having only horizontal strabismus without inferior oblique overaction (no inferior oblique group) as a control group, those having bilateral inf

Abnormalities of fixation, saccade and pursuit in posterior cortical atrophy.

The clinico-neuroradiological syndrome posterior cortical atrophy is the cardinal 'visual dementia' and most common atypical Alzheimer's disease phenotype, offering insights into mechanisms underlying clinical heterogeneity, pathological propagation and basic visual phenomena (e.g. visual crowding). Given the extensive attention paid to patients' (higher order) perceptual function, it is surprising that there have been no systematic analyses of basic oculomotor function in this population. Here 20 patients with posterior cortical atrophy, 17 patients with typical Alzheimer's disease and 22 hea

Self-paced saccades and saccades to oddball targets in Parkinson's disease.

Patients with Parkinson's disease (PD) manifest difficulty in initiation and execution of movements, particularly when movements are sequential, simultaneous or repetitive. Eye movements are particularly effective in evaluating motor impairments. We utilized a series of saccadic eye movement paradigms to explore the ability of 13 patients with mild-moderate PD and 13 age-matched healthy controls to self-pace saccades between two continuously illuminated targets, before and after an externally cued tracking period, and respond to unexpected changes in task demand. The latter was explored by mea

The value of flash visual evoked potentials in albinism.

In albinism, the majority of temporal retinal fibers serving the nasal visual field cross at the chiasm and project to the contralateral hemisphere. This misrouting is seen in hemispheric asymmetries present in the visual evoked potential (VEP). Misrouting of retinal fibers was also thought to occur in dissociated vertical deviation, Prader-Willi syndrome, and perhaps carrier states of albinism. However, recent literature is reaching the conclusion we have drawn in our laboratory: only albinism shows VEP hemispheric asymmetries that reverse when the other eye is stimulated. Use of different st

Impact of bilateral graded inferior oblique recession on ocular torsion in patients with primary inferior oblique overaction: A prospective study.

INTRODUCTION AND OBJECTIVES: The impact of inferior oblique (IO) weakening on ocular torsion is well documented. This study aimed to evaluate objective and subjective torsional changes using fondo photography and the Double Maddox rod test (DMRT) after bilateral IO recession in cases of primary inferior oblique overaction. MATERIALS AND METHODS: This prospective, hospital-based observational cohort study included 82 eyes from 41 patients with primary inferior oblique overaction who underwent IO recession using the Apt and Call technique between December 2023 and March 2025.
