Early Aeromedical Evacuation for Orbital Emphysema After Zygoma and Orbital Floor Fractures.

INTRODUCTION: Aeromedical evacuation of patients with post-traumatic orbital emphysema following zygoma and orbital floor fractures has often been viewed with considerable concern. The expansion of intra-orbital air with increasing altitude can theoretically result in orbital tension, which can lead to blindness. This concern is mainly based on extremely rare case reports and theoretical models rather than significant clinical experience. The purpose of this work is to evaluate hazards and complications associated with aeromedical evacuation of combat casualties with post-traumatic orbital emp…

Orbital Tumors: What the Radiologist Needs to Know from the Orbital Surgeon's Perspective.

Orbital tumors are diverse and relatively rare. Imaging studies such as MR imaging, computed tomography (CT), PET/CT, and ultrasound are essential for narrowing the differential diagnosis before surgery. The specific surgical approach for orbital tumors depends on location of the mass, size, and proximity to important orbital structures. The authors advocate close collaboration between the neuroradiologist and the orbital surgical team through weekly or monthly teaching and planning conferences and collaborative investigations that can ultimately lead to new discoveries and more reliable diagn…

Orbital Myeloid Sarcoma Masquerading as Orbital Cellulitis: A Diagnostic Challenge in an Adult Patient.

Myeloid sarcoma (MS), also known as granulocytic sarcoma or chloroma, is a rare extramedullary manifestation of acute myeloid leukaemia (AML). It represents a tumour-like accumulation of myeloid blasts outside the bone marrow. Orbital involvement is uncommon and may mimic inflammatory or infectious conditions, creating significant diagnostic challenges. We report a 24-year-old female who presented with a two-week history of progressive left orbital pain and swelling, initially managed as presumed orbital cellulitis with oral flucloxacillin and topical chloramphenicol, which subsequently progre…

A case of orbital compartment syndrome caused by orbital abscess in an immunocompetent pediatric patient.

Orbital compartment syndrome, a condition in which increased pressure within the orbit can cause permanent visual impairment, can be caused by infection or trauma. Orbital cellulitis is a spectrum of infection that can progress to abscess formation within the orbit, subsequently leading to orbital compartment syndrome. We describe the case of an immunocompetent pediatric patient who developed an orbital abscess from asymptomatic sinusitis, which led to orbital compartment syndrome. He received a lateral canthotomy and cantholysis followed by definitive management of his orbital abscess and exp…

Changes in Orbital Volume following Reconstruction with Alloplastic Materials in Patients with Orbital Trauma.

BACKGROUND: Achieving a satisfactory apical seal in root canals with open apices can be challenging, with the risk of filling material extrusion into the apex. Timely detection of voids in root canal filling material is crucial to prevent complications. PURPOSE: This study aimed to compare the accuracy of cone-beam computed tomography (CBCT), digital radiography, and conventional radiography in detecting voids in mineral trioxide aggregate (MTA) apical plugs. MATERIALS AND METHOD: In this in vitro study, thirty-two extracted upper maxillary incisors underwent decoronation, root canal instrum

Orbital exenteration in the management of orbital and periorbital tumours: Factors related to treatment outcomes.

The aim of this study was to evaluate the indications for surgery, patient and healthcare delays, ophthalmic status, challenges of management, complications, disease-free survival (DFS), and overall survival (OS) of patients treated by orbital exenteration at a tertiary care centre. Patients who underwent orbital exenteration during an 18-year period (2005-2022) were identified from the Helsinki University Hospital (Helsinki, Finland). Demographic and medical data were retrieved. Sixty-nine patients were included. Median age was 74 years (range, 7-93 years). The most common operative indicatio…

Tension Orbital Emphysema Following Blunt Trauma: A Case of Orbital Compartment Syndrome.

Orbital emphysema commonly occurs following traumatic orbital fractures, which usually resolve spontaneously. However, in rare cases, it can progress to tension orbital emphysema, leading to orbital compartment syndrome (OCS), a potentially blinding condition if not treated promptly. We report a 47-year-old man who developed OCS one day after sustaining blunt trauma to his right cheek. Initial examination revealed periorbital hematoma, emphysema, chemosis, and a raised intraocular pressure (IOP) of 28 mmHg without proptosis. Computed tomography showed a right orbital floor blowout fracture wit…

Intertwined Superconductivity and Orbital Selectivity in a Three-Orbital Hubbard Model for the Iron Pnictides.

We study a three-orbital Hubbard-Kanamori model relevant for iron-based superconductors using variational wave functions explicitly including spatial correlations and electron pairing. We span the nonmagnetic sector from filling n=4, which is representative of undoped iron-based superconductors, to n=3, where a Mott insulating state with each orbital at half filling is found. In the strong-coupling regime, when the electron density is increased, we find a spontaneous differentiation between the occupation of d_{xz} and d_{yz} orbitals, leading to an orbital-selective state with a nematic chara…
