Mini-plate fixation versus suture suspensory fixation in cervical laminoplasty: A meta-analysis.

BACKGROUND: Both the mini-plate fixation and suture suspensory fixation techniques are extensively applied in cervical laminoplasty, but which technique is superior has not been ascertained. The purpose of this meta-analysis is to compare the results between mini-plate fixation and suture suspensory fixation in cervical laminoplasty for the patients with multilevel cervical compressive myelopathy. METHODS: PubMed, Embase, the Cochrane library, CNKI, and WANFANG were searched for studies that compared mini-plate fixation and suture suspensory fixation in cervical laminoplasty up to November 1,

No difference in clinical results between femoral transfixation and bio-interference screw fixation in hamstring tendon ACL reconstruction. A preliminary study.

Hamstring tendon autograft has become a popular graft choice for anterior cruciate ligament (ACL) reconstruction, but there is no consensus on the ideal technique of fixation. We performed a pilot randomized controlled study to compare the clinical and mechanical outcome of two femoral fixation techniques for anterior cruciate ligament (ACL) reconstruction using hamstrings graft. We recruited 30 patients with a chronic unilateral tear of the anterior cruciate ligament. In all patients, quadrupled hamstring graft was used for ACL reconstruction. In 15 patients, femoral graft fixation was perfor

A preliminary study of the use of cage and plating for single-segment fusion in degenerative cervical spine disease.

Three different surgical methods were used to treat single-segment degenerative cervical spine disease, and the radiological and clinical outcomes were compared. The three groups compared were: 21 cases of anterior interbody fusion with iliac autograft (group A); 20 cases of anterior plate fixation with iliac autograft (group B); and 19 cases of carbon cage fusion and anterior plate fixation (group C). Bone union and changes in Cobb's angle and interbody space were evaluated using radiographs and clinical results by the method of Odom et al. Cobb's angle increased in group C, which was signifi

[Pelvic injuries in the polytraumatized patient].

Pelvic injuries represent a thorny and stubborn therapeutic challenge. Because major forces are required to fracture the pelvis, pelvic ring disruption, more than any other fracture, can lead to life-threatening associated injuries such as massive bleeding, organ injuries, and open fractures including hemipelvectomy. The rapid diagnosis and effective treatment ("damage control") of those injuries play the key role in the patient's survival, inasmuch as the mortality of multiply injured patients with pelvic ring disruption remains high with 20-35%. Exsanguinating hemorrhage represents the most

Detecting developmental dysplasia of the hip.

Recent guidelines from the American Academy of Pediatrics mandate the universal screening of all newborns for developmental hip dysplasia, in an effort to promote prompt recognition and treatment of this condition. The effectiveness of universal screening hinges on the appropriate education, training, and experience of health care providers. This article reviews the normal embryologic, natal, and postnatal development of the hip. Historical risk factors that may have an impact on this developmental sequence are outlined. A step-by-step guide to the examination of the hip is offered, along with

Surgical treatment of superior sulcus tumors with spinal and brachial plexus involvement.

OBJECT: Non-small cell lung carcinomas with spinal and brachial plexus involvement have traditionally been considered to be Stage IIIb lesions and therefore unresectable. Advances in spinal surgery, the application of magnetic resonance (MR) imaging, and improvements in neoadjuvant therapy require a reassessment of the potential for complete resection. METHODS: The authors conducted a retrospective review of all procedures involving the resection of superior sulcus tumors with spinal or brachial plexus involvement performed between 1985 and 1999. Assessment or resectability and operative plan

Salvage of instrumental lumbar fusions complicated by surgical wound infection.

STUDY DESIGN: This study retrospectively reviewed instrumented lumbar fusions complicated by surgical wound infection and managed by a protocol including antibiotic impregnated beads. OBJECTIVE: To evaluate the potential for an acceptable clinical outcome in cases of instrumented lumbar fusion complicated by wound infection. SUMMARY OF BACKGROUND DATA: Initial studies of pedicle screw instrumentation suggested an increased infection rate versus noninstrumented fusion. The presence of a metallic implant also complicates wound management. METHODS: Eight hundred fifty-eight instrumented fusion

Self-inflicted gunshot wounds of the lower half of the face: the evolution toward early reconstruction.

The self-inflicted gunshot wound of the lower half of the face traditionally has been treated by multistaged and prolonged reconstructive maneuvers that often left significant and uncorrectable secondary deformities. Modern cranio-maxillofacial methods for reduction and rigid fixation of the facial skeleton, using immediate autogenous bone grafts when required, also has been efficacious for the acute management of these severe gunshot injuries. These methods can reduce the previously expected morbidity. Today's improved outcomes also are a consequence of the use of more reliable regional flaps

Arthroscopically-Assisted Latissimus Dorsi Transfer for Irreparable Rotator Cuff Insufficiency: Modes of Failure and Clinical Correlation.

PURPOSE: The main objective of this retrospective study was to analyze the rate and modes of failure of latissimus dorsi transfer (LDT). The secondary objective was to evaluate whether a rupture of the transfer was associated with a worse outcome. METHODS: During a 2-year period, we performed consecutive LDTs either for irreparable posterior-superior rotator cuff tears (RCTs) or for failed prior repair. All the LDTs were performed by a single surgeon. All transfers were arthroscopically assisted and fixed as a tubularized LD tendon in a bone tunnel inside the humeral head. Three metal clips w

Solitary osteochondroma of the cervical spine causing spinal cord compression.

Osteochondromas are common benign tumours of bone that often occur in the metaphysodiaphyseal parts of long bones. They rarely occur in the spine. We present a case of solitary osteochondroma arising from the C-1 vertebral lamina, causing neurological symptoms. A 46-year-old man presented to our institution, complaining of pain and numbness originating from his neck and extending down to his left arm. Radiographs, CT and MRI showed a solitary benign appearing expansile bone tumour arising from the left vertebral lamina of C-1, spreading to C-2, exerting an eccentric posterolateral compression

Posterior compact Cotrel-Dubousset instrumentation for occipitocervical, cervical and cervicothoracic fusion.

The authors report on 32 consecutive patients with instability at the craniocervical, cervical and cervicothoracic regions suffering from various pathologies, who were treated with posterior instrumentation and fusion using the posterior hooks-rods-plate cervical compact Cotrel-Dubousset (CCD) instrumentation alone or, in three patients, in combination with anterior operation. The patients were observed postoperatively for an average of 31 months (range 25-44 months) and evaluated both clinically and radiographically using the following parameters: spine anatomy and reconstruction, sagittal pr

Vertebroplasty-augmented short-segment posterior fixation of osteoporotic vertebral collapse with neurological deficit in the thoracolumbar spine: comparisons with posterior surgery without vertebroplasty and anterior surgery.

OBJECT: The surgical approach and treatment of thoracolumbar osteoporotic vertebral collapse with neurological deficit have not been documented in detail. Anterior surgery provides good decompression and solid fusion, but the surgery-related risk is relatively higher than that associated with the posterior approach. In posterior surgery, the major problem after posterior correction and instrumentation is failure to support the anterior spinal column, leading to loss of correction of kyphosis. The aim of this study was to evaluate the efficacy of reinforcing short-segment posterior fixation wit

Comparison of biodegradable and titanium fixation systems in maxillofacial surgery: a two-year multi-center randomized controlled trial.

Biodegradable osteosynthesis could reduce/delete the problems associated with titanium plate removal. The aim of the present study was to compare the clinical performance in the first 2 post-operative years between a biodegradable and a titanium system in oral and maxillofacial surgery. The multicenter randomized controlled trial (RCT) was performed in the Netherlands from December 2006 to July 2009. Included were 230 patients who underwent a bilateral sagittal split osteotomy (BSSO) and/or a Le Fort-I osteotomy and those treated for fractures of the mandible, maxilla, or zygoma. The patients

Postoperative imaging of the temporomandibular joint.

Postoperative axiography is compared to preoperative findings and repeated regularly. Postoperative arthroscopy is avoided because of its invasiveness. Plain films comprise panorama views, the axial skull view, the Schüller view in closed and open mouth position, and the Clementschitsch view. They are done in the early postoperative phase (2 days), before and after removal of osteosynthetic material, and after 1 year. Early postoperative computed tomography control is done in patients with intra-articular fractures to look for intra-articular fragments and to check for the position of the affe

Surgical treatment of facial fracture by using unsintered hydroxyapatite particles/poly l-lactide composite device (OSTEOTRANS MX(®)): a clinical study on 17 cases.

BACKGROUND: In Japan we currently use absorption properties for facial fractures. OSTEOTRANS MX(®) (Takiron co., ltd, Japan) is an absorption device, which is called Super FIXSORB MX(®) in Japan. This absorbable osteosynthetic device constitutes unsintered hydroxyapatite particles/poly l-lactide (u-HA/PLLA) composites. This study focuses on reporting clinical cases of using OSTEOTRANS MX(®). MATERIALS AND METHODS: Seventeen patients (16 men and 1 woman) aged 10-80 years (mean: 39.9 years, SD: ±20.7) with 86 fracture sites were treated. In all cases we used 1.0 mm plates and 5 mm or 7 mm screw
