PURPOSE: To compare the results of early and delayed extension orthosis fabrication in closed tendinous mallet injuries.
METHODS: Between March 1992 and May 2011, 45 patients with isolated closed tendinous mallet finger injuries were treated with orthosis fabrication. The patients were classified into 2 different groups based on their date of presentation. Group 1 consisted of 28 patients who presented within 2 weeks of sustaining the trauma, and group 2 consisted of 17 patients who received treatment beginning between 2 and 4 weeks after sustaining the trauma. During the final assessments, t

The purpose of this study was to evaluate a new method showing how custom foot orthoses (CFO) improve dynamics of plantar loading. The method is based on the probability distribution of peak pressure time series and is quantified using the Regression Factor (RF). RF is a least square regression slope between the experimentally observed plantar pressure magnitude probability distribution and a modeled Gaussian shape. Plantar pressure data from a randomized controlled trial of 154 participants with painful Pes Cavus were retrospectively re-analyzed. The participants were randomized to an active

BACKGROUND: Changes in impulse during the first rocker (braking force) and third rocker (propulsion force) may affect changes in gait speed after orthotic intervention. The purpose of this investigation was to objectively measure changes in impulse during double support and correlate those findings to changes in gait speed with and without ankle foot orthosis in individuals with hemiplegia.
METHODS: Fifteen adults with stroke-related hemiplegia walked with and without ankle foot orthosis while foot pressure data was collected bilaterally. Outcome measures included: gait cycle time (s), mean f

A system to restore walking in the vicinity of a wheelchair for people with paraplegia resulting from spinal cord injury is under development. The approach combines single channel surface electrical stimulation with an orthosis. The orthosis is spring loaded and contains a pneumatic system that stores energy during knee extension caused by quadriceps stimulation and transfers it to hip joint for hip extension. A laboratory version of the prototype of the gait system has been fabricated and engineering bench tests were performed. The paper presents the design of the wearable prototype and resul

BACKGROUND: Femoral nerve palsy has been described as a result of hip hyperflexion during orthotic treatment for developmental dysplasia of the hip, but femoral nerve palsy in newborns who have not had brace treatment has not been reported.
METHODS: Two cases of femoral nerve palsy after breech lie in utero are reviewed. Neither of these infants had undergone treatment of any sort for the hip.
RESULTS: In both children, Ortolani-positive hip dislocation was encountered. Both were treated with abduction orthoses with lesser hip flexion than the Pavlik-type brace. Nerve recovery was complete,

We report a 92-year-old female with ankylosing spondylitis who suffered a non-displaced cervical fracture of the C4 vertebral body as the result of a mechanical fall. A rigid cervical collar was used to maintain stability. The patient later went on to develop upper airway obstruction as a result of fracture displacement. She was emergently intubated for airway protection. To our knowledge, this is the first report of interval, acute onset upper airway obstruction caused by cervical instability following trauma and fracture in a patient with ankylosing spondylitis. This example highlights one o

BACKGROUND: Although foot orthotics are widely prescribed for the treatment of flatfoot, the biomechanical effects of such devices are not yet fully clear. Accordingly, this study conducted an experimental investigation to evaluate the effects of orthoses on the gait patterns of patients with flatfoot during level walking.
METHODS: Eleven adults with flatfoot deformities were recruited. For each participant, kinematic and kinetic data were measured under three test conditions, i.e. walking barefoot, walking with shoes, and walking with shoes and insoles. During each test, the participants' ga

A key challenge in rehabilitation robotics is the development of a lightweight, large force, high degrees-of-freedom device that can assist in functional rehabilitation of the arm. Pneumatic actuators can potentially help meet this challenge because of their high power-to-weight ratio. They are currently not widely used for rehabilitation robotics because they are difficult to control. This paper describes the control development of a pneumatically actuated, upper extremity orthosis for rehabilitation after stroke. To provide the sensing needed for good pneumatic control, position and velocity

This study examined the outcomes of ultrasound-monitored Pavlik harness treatment in 25 infants (2 boys and 23 girls) representing a total of 31 cases of developmental dysplasia of the hip of Graf type IIc or more severe. For all infants, Pavlik harness treatment started after ultrasonographic diagnosis in our clinic. If there was no improvement by the third week of follow-up, the harness treatment was discontinued. Of the 25 patients (31 hips), 16 patients (18 hips) were successfully treated with the Pavlik harness. The effects of age at start of treatment, gender, side of pathology, first cl

We performed a paired study of mature patients with Perthes' disease to compare the radiological results after treatment between conservatively- and surgically-treated groups. One patient was selected from each group to create the pairs for this study. Each pair was strictly matched for gender, body mass index, age at onset, stage at the first visit, necrotic area and radiological at-risk signs and each was assessed by comparing the values of six radiological measurements. Eighteen pairs (36 hips) fitted the criteria. The radiological measurements which showed a statistically better result in

OBJECTIVE: The purpose of this case study was to improve stability, posture, and speed of gait in an individual with paraplegia through the application of a hybrid system including trunk-hip-knee-ankle-foot orthosis (THKAFO) with lockable joints and multichannel functional electrical stimulation (FES) with implanted electrodes.
METHODS: Two hybrid orthoses were implemented and evaluated on a person with complete absence of motor function and sensation below the T-9 level spinal cord injury. The first hybrid was a modified isocentric reciprocal gait orthosis (IRGO) with the knees controlled by

The purpose of this study was to determine the effect clinically prescribed ankle-foot orthoses (AFOs) have on the temporal-spatial parameters of gait, as compared with barefoot walking in children with cerebral palsy. A retrospective chart review of data collected between 1995 and 1999 in our motion analysis laboratory was performed. A retrospective chart review of 700 patients revealed 115 patients (mean age 9 years) who had a primary diagnosis of CP (diplegia n=97, hemiplegia n=18). All were wearing clinically prescribed hinged or solid AFOs at the time of undergoing a three dimensional gai

Walking function may be enhanced by correcting equinus and equinovarus deformities in CP. Nonoperative management should be used in young children. Muscle balancing procedures, such as gastrocnemius aponeurosis lengthening, Achilles' tendon lengthening, and SPLATT, are particularly useful in correcting these deformities.

Injuries of the forefoot are a challenge for orthopaedic surgeons. After healing of bone and soft tissue damage, posttraumatic metatarsalgia can persist. The paper presents a therapy algorithm with newly designed insoles, which consist of the common orthopaedic adaptations like a rocker bottom and a stiffening of the sole. The therapy is shown on two cases. With these orthotics adequate is possible and pain relief realistic.

UNLABELLED: By retrospective analysis we investigated the treatment of "congenital" dysplasia of the hip (D/IIIa-type according to Graf) in newborn infants with the Graf-Mittelmeier-Aktivspreizhose (abduction harness).
METHOD: Between 1990 and 1997 twenty of twenty-three newborn infants with the IIIa-type were treated with the abduction harness by short-term ultrasound monitoring. We started the therapy in the first days of life, if we could identify a IIIa-type with an alpha-angle close to type D and with only minor tendency to decentration.
RESULTS: Eighteen infants (90%) treated by abduct

We report on two patients who developed bilateral peroneal nerve palsy after using a knee board behind a water ski boat. This device causes the rider's knees to be in a hyperflexed position secured with a strap across the thighs. Treatment for this compressive neuropathy is conservative. Recreational users may wish to limit the duration and frequency of participation in this sport, thus decreasing the predisposition to prolonged nerve compression. In addition, manufacturers may consider making fundamental design changes such as padding the nylon straps or outrigger devices that contact the pro
