Comparison of imaging methods in POEMS syndrome.

BACKGROUND: POEMS syndrome is a clinical condition with a very heterogeneous clinical manifestation. Its presentation as well as monitoring is complex and dependent on the clinician's experience. One of the leading presenting symptoms is based on evaluation of skeletal damage with typical osteosclerotic or mixed lesions. AIMS AND METHODS: Our aim was to compare the usefulness of different imaging methods in the diagnostics of POEMS syndrome, such as conventional radiography, densitometry, technetium scintigraphy, PET/CT scan, MRI and angiography on a series of three patients with POEMS syndro

Sclerostin: current knowledge and future perspectives.

In recent years study of rare human bone disorders has led to the identification of important signaling pathways that regulate bone formation. Such diseases include the bone sclerosing dysplasias sclerosteosis and van Buchem disease, which are due to deficiency of sclerostin, a protein secreted by osteocytes that inhibits bone formation by osteoblasts. The restricted expression pattern of sclerostin in the skeleton and the exclusive bone phenotype of good quality of patients with sclerosteosis and van Buchem disease provide the basis for the design of therapeutics that stimulate bone formation

Findings from panoramic radiographs of the edentulous population and review of the literature.

OBJECTIVE: The purpose of this study was to evaluate the frequency of positive radiographic findings in panoramic radiographs of edentulous arches. METHOD: A sample of 327 panoramic radiographs was selected from the patient pool at the University of Oklahoma College of Dentistry. Radiographs were evaluated for presence of retained root tips, impacted teeth, radiolucent and radiopaque areas, soft tissue calcifications, and foreign bodies. RESULTS: Approximately 57.5% of panoramic radiographs revealed no positive radio-graphic findings, while the remaining 42.5% contained 1 or more positive ra

Craniometaphyseal dysplasia: case report.

Craniometaphyseal dysplasia is a rare genetic bone remodeling disorder characterized by undertubulation of the long bones, especially in the lower extremities, causing deformities of the metaphyses of the long bones, and sclerosis of the skull base or cranial bone hyperostosis. The authors report a case of craniometaphyseal dysplasia in an 8-year-old Brazilian child, emphasizing the importance of precocious diagnosis of this rare genetic disorder.

Avoiding problems in patients with craniotubular bone disorders.

Since osteomyelitis can occur concurrently with osteopetrosis, all craniotubular bone disorder patients should be treated carefully. We recommend preoperative antibiotic coverage and minimizing any excessive force to avoid the risk of osteomyelitis and jaw fractures when surgery is necessary.

Plasma-cell dyscrasia with polyneuropathy. The spectrum of POEMS syndrome.

BACKGROUND: The POEMS (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes) syndrome and osteosclerotic myeloma (polyneuropathy and sclerotic bone lesions) may both be manifestations of plasma-cell dyscrasia, but the interrelation of these diseases is not clear. We therefore set out to define the clinical spectrum of disease in patients with plasma-cell dyscrasia and polyneuropathy who have the complete or incomplete form of the POEMS syndrome or osteosclerotic myeloma. METHODS: Among 2714 patients with plasma-cell dyscrasia who were identified between 1973 a

Sclerosteosis involving the temporal bone: histopathologic aspects.

Sclerosteosis is a rare, potentially lethal, autosomal recessive, progressive craniotubular sclerosing bone dysplasia with characteristic facial and skeletal features. The temporal bone changes include a marked increase in overall size, extensive sclerosis, narrowing of the external auditory canal, and severe constriction of the internal auditory meatus, fallopian canal, eustachian tube, and middle ear cleft. Attenuation of the bony canals of the 9th, 10th, and 11th cranial nerves, reduction in size of the internal carotid artery, and severe obliteration of the sigmoid sinus and jugular bulb a

[Enthetic reactions of the hand in progressive scleroderma].

Frequent bone changes in progressive systemic sclerosis are the marginal osteolysis, the arthritic erosions and also the reactions at the numerous entheses of the hand, where the resorptive reactions predominate the osteoproliferative ones. The most frequent resorptions occur at the proximal corners of the tufts. At the margin of the tufts there are also enthesitic reactions, but they do not mark the beginning of an acroosteolysis.

FAM20C and FAM20A in normal and ectopic mineralization: A focus on oro-renal syndromes.

FAM20C is part of the FAM20 family and is crucial for phosphorylating secreted proteins. It plays roles in various biological processes, including cellular calcium regulation and cardiovascular function. Pathogenic variants of FAM20C cause Raine syndrome, resulting in osteosclerotic dysplasia or hypophosphatemic rickets. Its paralog, FAM20A, is a secreted pseudokinase needed for optimal FAM20C activity. Mutations in FAM20A cause Enamel-Renal Syndrome. Common features in both syndromes include Amelogenesis Imperfecta, gingival fibromatosis and ectopic mineralization. We summarize current knowle

Finite-element analysis of the proximal tibial sclerotic bone and different alignment in total knee arthroplasty.

BACKGROUND: Despite potential for improving patient outcomes, studies using three-dimensional measurements to quantify proximal tibial sclerotic bone and its effects on prosthesis stability after total knee arthroplasty (TKA) are lacking. Therefore, this study aimed to determine: (1) the distribution range of tibial sclerotic bone in patients with severe genu varum using three-dimensional measurements, (2) the effect of the proximal tibial sclerotic bone thickness on prosthesis stability according to finite-element modelling of TKA with kinematic alignment (KA), mechanical alignment (MA), and

Hypophosphatemic osteomalacia and bone sclerosis caused by a novel homozygous mutation of the FAM20C gene in an elderly man with a mild variant of Raine syndrome.

BACKGROUND: Hypophosphatemia and increased serum fibroblast growth factor 23 (FGF23) levels have been reported in young brothers with compound heterozygous mutations for the FAM20C gene; however, rickets was not observed in these cases. We report an adult case of Raine syndrome accompanying hypophosphatemic osteomalacia with a homozygous FAM20C mutation (R408W) associated with increased periosteal bone formation in the long bones and an increase in bone mineral density in the femoral neck. CASE: The patient, a 61-year-old man, was born from a cousin-to-cousin marriage. A short stature and sev

Severe osteopathia striata with cranial sclerosis in a female case with whole WTX gene deletion.

Osteopathia striata with cranial sclerosis (OSCS) is caused by truncating mutations or deletions in the X linked gene, WTX, and is characterized by sclerotic striations of the metaphyses and diaphyses of long bones, pelvis, and scapula, along with craniofacial hyperostosis. Females typically manifest with craniofacial dysmorphisms including macrocephaly, hypertelorism, depressed nasal bridge, and hypoplastic maxilla, often have cleft palate, and less often extra skeletal anomalies. Here we report on a sporadic female patient with OSCS born at 33 weeks, with coarse facies, an abnormal head shap

The male phenotype in osteopathia striata congenita with cranial sclerosis.

Osteopathia striata with cranial sclerosis (OSCS) is an X-linked disease caused by truncating mutations in WTX. Females exhibit sclerotic striations on the long bones, cranial sclerosis, and craniofacial dysmorphism. Males with OSCS have significant skeletal sclerosis, do not have striations but do display a more severe phenotype commonly associated with gross structural malformations, patterning defects, and significant pre- and postnatal lethality. The recent description of mutations in WTX underlying OSCS has led to the identification of a milder, survivable phenotype in males. Individuals

Hypocalcemia as a presentation for multifocal osteosarcoma.

We report the case of a 16-year-old male with multifocal osteosarcoma presented with symptomatic hypocalcemia. The patient presented signs of hypocalcemia accompanied by generalized bone pain and a femoral mass. Further evaluation revealed secondary hyperparathyroidism, highly elevated alkaline phosphatase levels, and generalized sclerotic and lytic lesions. Biopsy confirmed osteosarcomatosis. Despite aggressive chemotherapy, he died from progressive disease 2 months after diagnosis. The course of disease in this patient suggests that hypocalcemia reflects a poor prognosis.
