Treatment of postparotidectomy salivary fistula with botulinum toxin.

We report on the successful treatment with botulinum toxin type A local injections of a salivary fistula that occurred after superficial parotidectomy. In a 58-year-old woman, transcutaneous discharge of saliva in the preauricular region had persisted in spite of 2 surgical revisions. Moreover, facial weakness and synkinesis had developed as a result of an iatrogenic lesion that had occurred at the time of primary surgery and required immediate reanastomosis of the main nerve trunk. Botulinum toxin A was injected into the deep lobe of the remaining parotid gland under ultrasonographic guidance

The Epstein-Barr virus in malignant non-Hodgkin's lymphoma of the upper aerodigestive tract.

Sixty malignant non-Hodgkin's lymphomas originating in the upper aerodigestive tract have been analyzed for their cytologic type, immunophenotype and association with the Epstein-Barr virus (EBV). The majority of these tumors were B-cell lymphomas of blastic cytology (78%) with the exception of lymphomas in the parotid gland. Large B-cell lymphomas were the most frequent encountered in the sinonasal region and Waldeyer's ring. Twelve lymphomas were of T- or T/NK (natural killer)-cell lineage. They were in the nasal cavity and the paranasal sinuses (4), the tonsil (5), and the oral cavity (3).

Metastatic malignant melanoma of the oral cavity. A retrospective study.

Metastatic malignant melanoma of the oral cavity is rarely reported in the dental literature. This retrospective study identified metastatic oral lesions in 3.0% of 809 patients with melanoma treated at the National Institutes of Health between 1953 and 1989. Fifteen cases met established rigorous criteria for metastatic tumors and were reviewed for disease course and outcome. Nine white men and six white women, with an average age of 40.6 years, had cutaneous primary tumors predominantly of the trunk and head and neck region that commonly presented as moles that were enlarging, bleeding, or s

Cryosectional observations of functional anatomy of the temporomandibular joint.

The anatomy and function of the temporomandibular joint are complex, and our understanding about how the retrocondylar tissues change when the condyle translates anteriorly has been incomplete. We therefore examined the sectional anatomy of 24 fresh human temporomandibular joints at different gradations of jaw opening, protrusion, and laterotrusion. We found that the tissues of the posterior disk attachment expanded in volume and occupied the space behind the condyle when the mouth was opened, protruded, or laterotruded to the contralateral side. Volumetric expansion of the tissue seemed to be

Cytological diagnosis of sialadenosis, sialadenitis, and parotid cysts by fine-needle aspiration biopsy.

This analysis permits the following conclusions: The value of punctate cytology consists in the possibility to verify the clinical diagnosis of a cyst by aspiration of fluid, and to make a preoperative assessment of the necessity of an operation and its type and extent, if proof of a benign or malignant tumor was obtained by microscopic examination. The diagnostic value of punctate cytology can be increased if after aspiration of cyst fluid also solid tissue components from the region of the cyst are included. Patients whose biopsy aspirates did not give evidence for the presence of a tumor sh

Expression of podoplanin in Warthin tumours.

PURPOSE: Warthin tumour is the second most common benign tumour of the parotid gland. This study was designed to investigate the lymphatic vessels in Warthin tumours in an effort to understand better its pathogenesis. MATERIALS AND METHODS: Tissue specimens of 31 patients (19 men and 11 women; mean age 57 years, median size of the tumours 2.86 cm) were analysed by means of immunohistochemistry applying the monoclonal antibody D2-40. RESULTS: We found numerous D2-40-positive sinus-like vessels particularly at the inner layer of the capsule. CONCLUSIONS: Since subcapsular sinuses are a major

Anatomical knowledge of zygomatico-buccal plexus in a cadaveric study.

The details of the facial nerve pattern were clearly explained in the parotid gland (PG), lateral area of the face, and periorbital areas to prevent the unexpected outcome of medical intervention. However, it remains unclear whether information about the zygomatico-buccal plexus (ZBP) in the masseteric and buccal regions. Therefore, this study aimed to help clinicians avoid this ZBP injury by predicting their common location. This study was conducted in forty-two hemifaces of twenty-nine embalmed cadavers by conventional dissection. The characteristics of the buccal branch (BB) and the ZBP wer

Combined radio-immunotherapy leads to complete clinical regression of stage IV Merkel cell carcinoma.

Merkel cell carcinoma (MCC) is a rare, aggressive neuroendocrine tumour of the skin. While localised disease carries an overall favourable prognosis, metastatic disease is associated with poor clincal outcomes. Most cases of metastatic MCC are managed with systemic chemotherapy or immunotherapy, though 5-year survival for these patients remains a dismal 17%. Here, we present the case of a 79-year-old man with MCC of the right ear with metastases to regional lymph nodes, ipsilateral parotid gland and thoracic spine. He was treated with a combination of first-line radiotherapy and concurrent imm

Synchronous Parotid and Homolateral Submandibular Gland Pleomorphic Adenoma.

INTRODUCTION: Pleomorphic adenomas are benign neoplasms of salivary glands. The simultaneous homolateral occurrence of these tumors in salivary glands is exceedingly rare. CASE REPORT: An adult female presenting to our OPD with the swelling of right-sided preauricular and submandibular regions was diagnosed with the pleomorphic adenoma based on fine needle aspiration cytology. The patient was subjected to the excision of both swellings under general anesthesia. Postoperative facial nerve functions were within normal limits and final histopathology confirmed pleomorphic adenoma involving both

Submandibular gland-sparing intensity-modulated radiotherapy.

Xerostomia is one of the most debilitating late effects of head and neck radiotherapy (RT) and significantly impacts quality of life. The submandibular gland (SMG) provides approximately 70% of the unstimulated saliva that accounts for about 95% of the salivary flow during a 24-hour period. Intensity-modulated RT (IMRT) has been used in recent years to lower the RT dose to the parotid gland(s) to reduce long-term xerostomia. There is little experience with SMG-sparing IMRT. Mean RT doses to the SMG exceeding 39 Gy cause permanent ablation of both stimulated and unstimulated salivary flow. Limi

Superficial musculoaponeurotic system flap for reconstruction of the intraoral defects.

Intraoral tumors are the main causes of the intraoral and maxillofacial defects. Skin grafts and several soft tissue flaps can be used to reconstruct the intraoral defects including local, regional, and free flaps. Here we present a case of intraoral adenocystic carcinoma treated with segmental maxillectomy, resection of the parotid gland, and the buccal mucosa leaving a full-thickness intraoral defect. The defect was covered with a superficial musculoaponeurotic system (SMAS) flap elevated using the preauricular incision. Reconstruction with SMAS flap for temporal defects and parotidectomy de

Intermediate lymphocytic lymphoma of the salivary gland.

Three patients had a painless mass in the parotid (two patients) or submandibular regions (one patient). These patients were 70, 66, and 65 years of age, respectively. One patient was male and the other two were female. No symptoms suggesting the presence of autoimmune sialadenitis had been noticed, although one patient suffered from rheumatoid arthritis. Histologic study showed that small atypical lymphoid cells with slightly irregular and indented nuclear contours proliferated diffusely and secondary lymph follicles were distributed at random among them. These findings are similar to those t

Kimura's disease sequentially involving multiple sites in the head and neck: A case report with a 13-year follow-up and literature review.

Kimura's disease is a rare, chronic inflammatory condition with a high recurrence rate, primarily affecting young to middle-aged Asian males. It typically manifests as masses in the head and neck, accompanied with regional lymphadenopathy. This report describes the case of a 50-year-old man initially diagnosed with Kimura's disease 13 years ago, following surgical removal of a left submandibular mass. Subsequent recurrences involved the parotid and contralateral submandibular regions, requiring radiotherapy and intermittent oral prednisone therapy. After 6 years, he developed progressive propt

Clinical features in salivary gland lymphoepithelial carcinoma in 10 patients: Case series and literature review.

OBJECTIVE: Lymphoepithelial carcinoma (LEC) accounts for 0.4% of malignant tumors of the salivary gland and 0.8% of parotid gland malignancies. Over the past 50 years, less than 300 cases have been reported in the literature. The purpose of this study was to investigate the characteristics of salivary gland LEC. METHODS: We retrospectively reviewed the medical records and analyzed clinical data obtained from 10 patients seen at our hospital between 2005 and 2020 with salivary gland LEC. RESULTS: All patients presented with a self-palpable, non-tender, hard swelling, or lump near the jaw or i

Efficacy of sclerotherapy with radio-opaque foam guided by digital subtraction angiography for the treatment of complex venous malformations of the head and neck.

Our aim was to evaluate the efficacy of sclerotherapy using radio-opaque foam and guided by digital subtraction angiography (DSA) for complex venous malformations in the head and neck in 11 selected patients between 2011 and 2013. The sclerosing foam was manufactured by the classic Tessari method and consisted of air, 1% polidocanol, and radio-opaque media iopromide (Ultravist(®)300) in a ratio of 7:2:1. We recorded the site and size of the lesion, time and duration of treatment, and therapeutic response. The lesions were on the face, cheek, temporal region, parotid region, neck, tongue, floor…

[Basal cell adenoma of Weber's glands].

INTRODUCTION: In 1967, Kleinsasser first reported about basal cell adenomas in salivary glands, which represent approximately 1-2% of all salivary gland tumors. About 70% of these tumors are found in the parotid gland. Less often the minor salivary glands are involved, particularly in the upper lip region. We would like to report on a case of occurrence in the upper pole of the palatine tonsil. CASE REPORT: A 68-year-old woman was seen, who complained about a growth in her left tonsil. Other than a slow increase size, she had no symptoms. Examination revealed a solid tumor approximately 3 cm

A modified retromaxillary approach to the infratemporal fossa: three case studies.

The infratemporal fossa (ITF) is an anatomically complex region with multiple neural and vascular structures entering and exiting through foramina in the skull base. The main obstacles to approaching the ITF are the zygomatic arch, the parotid gland, the facial nerve, and the ascending ramus AND condylar head of the mandible. Different surgical approaches to the ITF exist and the best approach should provide optimal visibility, minimal impairment of temporomandibular joint function, and preservation of motor and sensory nerve integrity. This report describes a modified Obwegeser retromaxillary

Salivary proteins in rheumatoid arthritis and Sjögren's syndrome: one-dimensional and two-dimensional electrophoretic studies.

Parotid saliva from patients with rheumatoid arthritis and Sjögren's syndrome contains elevated levels of multiple anionic proteins of pI approximately 3.75-4.75 and Mr approximately 32,000. Further studies on these components involving narrow range pH 3.5-5.0 immobilized pH gradients (IPGs) and two-dimensional (2D) electrophoresis with narrow- or broad-range IPGs in the first dimension have confirmed their association with these disorders. Immunoblotting showed that these proteins include multiple forms of tissue kallikrein. Treatment with neuraminidase results in removal of these anionic sub

Patterns of regional lymph node metastases from cutaneous melanomas of the head and neck.

A consecutive series of 111 patients treated between 1964 and 1989 with primary cutaneous malignant melanoma of the head and neck and histologically proven regional metastases was reviewed to determine the patterns of nodal metastases. The primary sites were grouped in the following categories: face (34), anterior scalp (25), anterior neck (16), posterior scalp (15), ear (11), and posterior neck (10). All patients underwent radical neck dissection: 80 of the procedures were therapeutic and 31 elective. A total of 106 specimens were positive for metastases. Thirty-three of the 57 patients under
