Early pregnancy maternal hemoglobin and the risk of neonatal congenital heart disease: insights from a case-control study, Guangdong, China.

INTRODUCTION: The association between maternal hemoglobin in the first trimester (1st TRI) and the risk of neonatal congenital heart disease (CHD) remains unclear. METHODS: A case-control study was conducted involving 102 CHD neonates and 408 controls. Iron-deficiency anemia in the first trimester (1st TRI) was characterized by a hemoglobin level below 110 mg/L, in the absence of hemoglobinopathies such as thalassemias and sickle cell anemia like thalassemia or sickle cell anemia. We utilized logistic regression models to assess the association between maternal hemoglobin/iron-deficiency anem

Early treatment versus expectant management of hemodynamically significant patent ductus arteriosus for preterm infants.

RATIONALE: Patent ductus arteriosus (PDA) is associated with significant morbidity and mortality in preterm infants. Cyclo-oxygenase (COX) inhibitor drugs are used to prevent or treat a PDA. There are concerns regarding adverse effects of COX inhibitors in preterm infants. Controversy exists about whether early targeted treatment of a hemodynamically significant (hs)-PDA improves clinical outcomes. This review updates our previous Cochrane review (2020). OBJECTIVES: To assess the effectiveness and safety of early treatment strategies versus expectant management for hemodynamically significant

Sirtuin modulators: an updated patent review (2012 - 2014).

INTRODUCTION: Since 2000 sirtuins (SIRT1-7) have gained growing attention for their connections with many biological processes such as cellular metabolism regulation, neuroprotection, apoptosis, inflammation, and cancer progression. In particular, SIRT1 has been the most studied isoform, not only for its role during caloric restriction but also as target in prevention of aging-related diseases. SIRT inhibition can be useful for treating cancer, HIV infection or muscular diseases, SIRT activation can exert positive effects in aging-related disorders such as metabolism, cardiovascular, and neuro

To close or not to close: the very small patent ductus arteriosus.

Patent ductus arteriosus (PDA) accounts for approximately 10% of all congenital heart diseases, with an incidence of at least 2-4 per 1000 term births. Closure of the large, hemodynamically significant PDA is established as the standard of care, and can be performed safely and effectively using either surgical or transcatheter methods. The appropriate management of the very small, hemodynamically insignificant PDA is less clear. Routine closure of such defects has been advocated to eliminate or reduce the risk of infective endocarditis (IE). However, the risk of IE in patients with a small PDA

N-terminal-pro-B-type natriuretic peptide in premature patent ductus arteriosus: a physiologic biomarker, but is it a clinical tool?

This study investigated whether N-terminal-pro-B-type natriuretic peptide (NT-proBNP) levels could serve as prognostic indicators of the therapeutic responsiveness of the patent ductus arteriosus to pharmacologic treatment. The levels of NT-proBNP in premature neonates with hemodynamically significant patency of the ductus arteriosus (hsPDA) were assessed before and after treatment using ibuprofen, indomethacin, or both. The baseline NT-proBNP levels were similar in both the infants who responded and those who did not respond to medical treatment. The combined data for all the subjects showed

Indomethacin treatment for symptomatic patent ductus arteriosus: a double-blind control study.

A double-blind control study was designed to determine the efficacy and safety of indomethacin treatment of patients with symptomatic patent ductus arteriosus. Infants with severe respiratory distress syndrome and symptomatic patent ductus arteriosus were eligible for the prospective study if the ratio of left atrial/aortic root diameter remained greater than or equal to 1.3:1 following a 24-hour period of medical management. Thirty-nine eligible infants were randomly assigned to the control or indomethacin group and given 0.2 mg/kg of enteral indomethacin or placebo in a double-blind manner.

Improving RBC K transport and hemoglobin-O2 binding by amiloride: A novel therapeutic approach for reversion of angina and myocardial ischemia in coronary heart diseases.

UNLABELLED: Coronary heart disease (CHD) is the leading cause of morbidity and mortality across the entire world, in which reversion of angina or improvement of ECG remains an unrealistic therapeutic option for most patients, suggesting that microvascular dysfunction or impaired oxygen delivery might be critical factors in CHD. This research article, thus presents the rationale basis, clinical and experimental, for the first therapeutic innovation addressing the role of red blood cell (RBC) H/K and O2/CO2 exchanges in CHD. It is followed by a randomized single-blind trial of Amiloride and Opti
