Integrating Shared Decision-Making Into Breast Cancer Postmastectomy Reconstruction: A Systematic Review of Evidence, Barriers, and Real-World Implementation.

This systematic review synthesizes empirical evidence on the development, effectiveness, patient experience, and implementation of decision aids (DAs) for postmastectomy breast reconstruction (PMBR) and to evaluate the role of pre-consultation educational interventions in supporting shared decision-making (SDM). Following PRISMA guidelines and a preregistered protocol (OSF: https://osf.io/mu9hv), six electronic databases and gray literature sources were searched for studies on PMBR DAs through November 2025. Eligible studies were categorized using a five-domain framework aligned with IPDAS and

Designing inclusive newborn sequencing research: insights from parents in underrepresented communities.

BACKGROUND: It is essential that studies of genomic sequencing (GS) in newborns and children include individuals from under-represented racial and ethnic groups (URG) to ensure future applications are equitably implemented. We conducted interviews with parents from URG to better understand their perspectives on GS research, develop strategies to reduce barriers to enrollment, and facilitate research participation. METHODS: Semi-structured interviews with 50 parents from URG. RESULTS: Nearly all parents said they would be interested in participating in an infant GS study. Parents were interes

Digital Patient Decision Aids for Endometriosis Management: A Scoping Review.

BACKGROUND: Endometriosis treatment requires women to navigate complex, preference-sensitive decisions. Patient Decision Aids (PtDAs) help patients make value-aligned choices. However, the scope and quality of digital PtDAs for endometriosis, and the cap acity of conversational AI platforms to act as PtDAs, remain unclear. OBJECTIVES: Systematically map digital PtDAs for women of reproductive age with endometriosis, describe their content, development, and evaluation, and assess quality and replicability. SEARCH STRATEGY: Electronic databases, Google Scholar, grey literature, and web searche

Factors influencing engagement with patient-directed and facilitated advance care planning interventions for patients with advanced cancer.

BACKGROUND: Advance care planning (ACP) for patients with cancer is guideline recommended. However, there are significant challenges to engagement. Patient-facing ACP programs have been developed to improve access and engagement. This study aimed to identify factors impacting engagement for patients with cancer generally, and specifically for two patient-facing approaches. METHODS: As part of a randomized trial comparing the effectiveness of facilitated versus patient-directed ACP, interviews were conducted with patients with cancer who participated in the two interventions. A thematic analys

Identifying strategies for disseminating research to patients with chronic kidney disease and caregivers: a workshop report.

Health research results are primarily disseminated through scientific peer-reviewed journals and are not readily accessible to patients and caregivers, which can impede informed decision-making and limit the impact of research on patient outcomes. The aim of the workshop was to identify strategies to disseminate research in chronic kidney disease (CKD) to patients and caregivers. The workshop involved patients, caregivers (n = 27), and health professionals (n = 54) from Australia (10 breakout groups). The transcripts were thematically analyzed. Three themes (strategies) were identified. Genera

Recounting the untold stories of breast cancer patient experiences: lessons learned from a patient-public involvement and engagement storytelling event.

OBJECTIVES: Breast cancer remains a prevalent disease in women worldwide. Though advancements in breast cancer care have improved patient survival, a breast cancer diagnosis, and subsequent interventions have a lasting impact on patients' lived experiences during the pandemic. METHODS: We present the collaborative learning process from this patient engagement workshop series as a community-academic partnership. Narrative medicine tools were used to recount patients' lived experiences following diagnosis, where both patients and researchers shared their cancer research activities in each works

Fostering Shared Decision-Making Between Patients and Health Care Professionals in Clinical Practice Guidelines: Protocol for a Project to Develop and Test a Tool for Guideline Developers.

BACKGROUND: Clinical practice guidelines (CPGs) are designed to assist health care professionals in medical decision-making, but they often lack effective integration of shared decision-making (SDM) principles to reflect patient values and preferences, particularly in the context of preference-sensitive CPG recommendations. To address this shortcoming and foster SDM through CPGs, the integration of patient decision aids (PDAs) into CPGs has been proposed as an important strategy. However, methods for systematically identifying and prioritizing CPG recommendations relevant to SDM and related de

Influences of decision preferences and health literacy on temporomandibular disorder treatment outcome.

BACKGROUND: Shared decision-making is defined as the process by which physicians and informed patients make a shared medical decision, taking into account the preferences and values of the patients. It is well known that shared decision-making practices improve both clinicians' and patients' satisfaction and lead to better treatment outcomes. The aim of the study was to assess associations between patients' roles in decision-making, health literacy levels, and treatment outcomes of temporomandibular disorders (TMDs). METHODS: In total, 131 participants were enrolled. Participants underwent in

Physicians' perspectives on using a patient decision aid in female stress urinary incontinence.

INTRODUCTION AND HYPOTHESIS: A treatment choice for female stress urinary incontinence (SUI) is preference sensitive for both patients and physicians. Multiple treatment options are available, with none being superior to any other. The decision-making process can be supported by a patient decision aid (PDA). We aimed to assess physicians' perceptions concerning the use of a PDA. METHODS: In a mixed methods study, urologists, gynecologists and general practitioners in the Netherlands were asked to fill out a web-based questionnaire. Questions were based on the Tailored Implementation for Chron

Improving acute pain management of trauma patients on medication-assisted therapy.

BACKGROUND: Approximately 9,500,000 people in the United States misused opioids in 2020. Many people manage their opioid use disorder (OUD) with medication-assisted treatment (MAT). Using MAT to address OUD adds to the complexities and challenges of adequate acute pain control. LOCAL PROBLEM: Chart review indicated only 20% of trauma patients on MAT achieved adequate pain control on the trauma service at the University of Louisville Hospital. This quality initiative aimed to increase patient pain control to 50% in 90 days. METHODS: A rapid cycle quality improvement project with four plan-do-

Effect of a discordant opinion offered by a second opinion physician on the patient's decision for management of spinal disc disease.

BACKGROUND: Factors that influence a patient's decision for spinal surgery and selection of the spine surgeon have not been studied in the context of a Second Opinion (SO). Providing insight into these factors will guide surgeons in their discussion of treatment options with patients. OBJECTIVE: This study aims to assess the impact of a discordant SO on the final decision of patients as compared to their initial preference regarding spinal disc disease treatment for chronic neck and low back pain. PATIENT INVOLVEMENT: Patients in this study engage in clinical vignettes designed to induce dec

Would you like to participate in this trial? The practice of informed consent in intrapartum research in the last 30 years.

BACKGROUND: Informed consent is the cornerstone of the ethical conduct and protection of the rights and wellbeing of participants in clinical research. Therefore, it is important to identify the most appropriate moments for the participants to be informed and to give consent, so that they are able to make a responsible and autonomous decision. However, the optimal timing of consent in clinical research during the intrapartum period remains controversial, and currently, there is no clear guidance. OBJECTIVE: We aimed to describe practices of informed consent in intrapartum care clinical resear

Patients' and clinicians' perspectives on shared decision-making regarding treatment decisions for depression, anxiety disorders, and obsessive-compulsive disorder in specialized psychiatric care.

RATIONALE, AIMS, AND OBJECTIVES: People worldwide are affected by psychiatric disorders that lack a "best" treatment option. The role of shared decision-making (SDM) in psychiatric care seems evident, yet remains limited. Research on SDM in specialized mental health is scarce, concentrating on patients with depressive disorder or psychiatric disorders in general and less on patients with anxiety and obsessive-compulsive disorder (OCD). Furthermore, recent research concentrates on the evaluation of interventions to promote and measure SDM rather than on the feasibility of SDM in routine practic

PlayBionic: Game-Based Interventions to Encourage Patient Engagement and Performance in Prosthetic Motor Rehabilitation.

BACKGROUND: Prosthetic motor rehabilitation usually relies on the highly repetitive training of movements. Patients might drop out of training because the rehabilitation process is long and often discouraging. Game-based interventions provide a potentially useful alternative to standard myoelectric (electromyographic [EMG]) training and can increase engagement with training. OBJECTIVE: To assess the short-term impact of a game-based rehabilitation protocol on parameters for EMG control, evaluate how game-based interventions affect patient motivation, performance, and effort, and compare the g

Management of neuromuscular diseases and spinal muscular atrophy in Latin America.

Latin America (LA) has a population of ~645 million people distributed over 33 countries with marked political, cultural and economic differences. In LA, patients with inherited neuromuscular diseases (NMDs) often do not have access to specialized medical centers and many of them go undiagnosed. General management and care of spinal muscular dystrophy (SMA) patients in the region varies due to heterogeneous health care. An active generation of young clinical neurologists is being trained for the specialized care of SMA and other neuromuscular (NM) patients, both in the private and public secto

Improving Hypertension Control and Patient Engagement Using Digital Tools.

Hypertension is present in 30% of the adult US population and is a major contributor to cardiovascular disease. The established office-based approach yields only 50% blood pressure control rates and low levels of patient engagement. Available home technology now provides accurate, reliable data that can be transmitted directly to the electronic medical record. We evaluated blood pressure control in 156 patients with uncontrolled hypertension enrolled into a home-based digital-medicine blood pressure program and compared them with 400 patients (matched to age, sex, body mass index, and blood pr
