OBJECTIVES: To show the pathologic characteristics of upper urinary tract tumors with special emphasis on the peculiarities due to anatomical site and secondary to endoscopic treatment.

The subepithelial hematoma of the renal pelvis and ureter was described by Antopol and Goldman in 1948. Hematuria and filling defect in the renal pelvis are the most consistently recurring clinical and radiological findings. Only 21 cases were found in the literature on the subject and all of them were treated surgically, usually with partial or total nephrectomy. Here we present a case of Antopol-Goldman lesion which was treated conservatively.

We report a 2-year-5-month-old girl with malformed lower limbs. The radiographic skeletal survey revealed agenesis of the ilio-pubic rami with pubic dehiscence, right hip dislocation, bilateral coxa vara, short femurs, femoro-tibial synchondrosis, bilateral hypoplastic tibiae more severe on the left side, and hypoplastic left calcaneus and talus. To the best of our knowledge, this combination of multiple congenital skeletal abnormalities has not been reported before.

AIMS: To ascertain the outcome associated with antenatal renal pelvis dilatation; to recommend guidelines for postnatal investigation and determine an upper limit of normal for the anterioposterior dimensions of the fetal renal pelvis.

To determine the incidence of urinary tract abnormalities detected in the presence of moderate fetal renal pelvis dilatation, we followed up pre- and postnatally 116 fetuses and children between 1985 and 1995. At prenatal ultrasound, 50 (43%) fetuses showed regressive dilatations, 57 (49%) a stable pattern, and 9 (8%) an evolutive pattern. Mucinous adenocarcinoma of the renal pelvis is a rare tumor. We report the case of a fifty-three-year-old man with this unusual cancer and review the literature. Usual features of this tumor include long duration of symptoms, an association with calculi and hydronephrosis, and a preoperative appearance of an inflammatory condition.

A variable pressure volumetric pump was used for 10% hemiacidrin irrigation of the renal pelvis in 7 renal units (6 patients) with struvite calculi after initial percutaneous debulking and extracorporeal shock-wave lithotripsy (ESWL). We describe an unusual and rare case of humoral hypercalcemia due to Stage D squamous cell carcinoma of the renal pelvis in a patient with no evidence of bony metastases. The literature on humorally mediated hypercalcemia associated with epithelial tumors of the renal pelvis is reviewed.

We report the fourth case of an osteoclastoma-like giant cell tumour of the renal pelvis. A special feature was that although thorough sampling of the tumour showed an osteoclastoma-like pattern throughout, it was intimately associated with carcinoma in situ change of the adjacent transitional epithelium and this provides further support for the view that these tumours are of epithelial derivation. However, immunohistological and ultrastructural studies failed to reveal epithelial features within the tumour cells and the possible significance of this finding is discussed.

Two cases of peripelvic hemorrhage simulating tumors (Antopol-Goldman lesion) are reported. In the 2 cases, nephrectomies were performed. With more knowledge of this lesion, and of its rapid disappearance, a waiting period and repeated intravenous pyelogram might prevent some unnecessary surgery.
