Persistent orodental pain, atypical odontalgia, and phantom tooth pain: when are they neuropathic disorders?

Patients with unrelenting pain in the teeth, gingival, palatal or alveolar tissues often see multiple dentists and have multiple irreversible procedures performed and still have their pain. Up to one-third of patients attending a chronic facial pain clinic have undergone prior irreversible dental procedures for their pain without success. In these cases, if no local source of infectious, inflammatory, or other pathology can be found, then the differential diagnosis must include a focal neuropathic pain disorder. The common diagnoses given include the terms atypical odontalgia, persistent orode

A rare case of mandibular third molar displaced into submandibular space: A complication of mandibular third molar extraction.

KEY CLINICAL MESSAGE: Accidental displacement of mandibular third molars or their roots into neighboring anatomical spaces, notably the submandibular region, during extraction, is a rare but significant complication. Comprehensive preoperative assessment, meticulous surgical techniques, and precise radiographic imaging play pivotal roles in successful management. Adhering to established protocols is imperative to mitigate potential complications and ensure optimal outcomes. ABSTRACT: In this report, we emphasize the critical importance of comprehensive preoperative preparation to tailor surgi

Management of complex multi-space odontogenic infections.

The successful management of multi-space orofacial odontogenic infections involves identification of the source of the infection, the anatomical spaces encountered, the predominant microorganisms that are found during the various stages of odontogenic fascial space infection, the impact of the infectious process on defense systems, the ability to use and interpret laboratory data and imaging studies, and a thorough understanding of contemporary antibiotic and supportive care. The therapeutic goals, when managing multi-space odontogenic infections, are to restore form and/or function while limi

Comparative evaluation of local infiltration of articaine, articaine plus ketorolac, and dexamethasone on anesthetic efficacy of inferior alveolar nerve block with lidocaine in patients with irreversible pulpitis.

INTRODUCTION: The inferior alveolar nerve block (IANB) has a poor success rate in patients with irreversible pulpitis. The purpose of this study was to evaluate the effect of ketorolac and dexamethasone infiltration along with standard IANB on the success rate. METHODS: Ninety-four adult volunteers who were actively experiencing pain participated in this prospective, randomized, double-blind study. All patients received standard IANB of 2% lidocaine with 1:200,000 epinephrine. Twenty-four patients did not receive any supplemental infiltrations (control). Twenty-four patients received suppleme

Morbidity after chin bone harvesting--a retrospective long-term follow-up study.

BACKGROUND: Resorption of the alveolar bone after tooth extraction may result in insufficient bone volume for implant placement. Augmentation of the resorbed site using autogenous bone grafts harvested from the maxillofacial region, for example, the chin, is a common method; however, it also involves donor site morbidity. Chin graft morbidity involves impaired sensibility in the frontal teeth, the gingival, and skin postoperatively. MATERIALS AND METHODS: A group of 60 patients with partial edentulism in the maxilla and insufficient bone volume for implant therapy were augmented with bone gra

Response of periradicular tissues to growth factors introduced into the surgical site in the root-end filling material.

AIM: The objective of this study was to evaluate the healing of the periradicular tissues when exogenous growth factors were delivered to the respected root-end. The healing response was compared with that when Diaket was used as a control. METHODOLOGY: Non-surgical root canal treatment was performed on mandibular teeth in mongrel dogs. Surgical treatment followed and included root-end resection and root-end cavity preparation. Insulin-like growth factor in combination with platelet-derived growth factor, or fibroblast growth factor alone, were then placed in the root-end preparations on a po

Digitally Guided Aspiration Technique for Maxillary Sinus Floor Elevation in the Presence of Cysts: A Case Series.

OBJECTIVES: Sinus floor elevation (SFE) is a widely established surgical procedure for dental implant placement in the atrophic posterior maxilla. However, the presence of maxillary sinus cysts (MSCs) can significantly complicate this intervention. This study presents and evaluates the efficacy and safety of the Digitally Guided Aspiration Technique (DGAT), a novel approach for managing MSCs during SFE procedures. MATERIALS AND METHODS: Implant survival and success rates were evaluated according to established criteria, and all complications were systematically documented. Three-dimensional m

Reduced need for alveolar bone grafting by presurgical orthopedics and primary gingivoperiosteoplasty.

OBJECTIVE: The purpose of this study was to evaluate if narrowing and approximation of the alveolar cleft through presurgical alveolar molding followed by gingivoperiosteoplasty (GPP) at the time of lip repair reduces the need for a bone-grafting procedure. DESIGN: This was a retrospective blind study of patients with unilateral or bilateral alveolar clefts who underwent presurgical infant alveolar molding and GPP by a single surgeon. Alveolar bone formation was assessed prior to the eruption of the maxillary lateral incisor or canine by clinical examination, panoramic and periapical radiogra

Treatment of deep caries lesions in adults: randomized clinical trials comparing stepwise vs. direct complete excavation, and direct pulp capping vs. partial pulpotomy.

Less invasive excavation methods have been suggested for deep caries lesions. We tested the effects of stepwise vs. direct complete excavation, 1 yr after the procedure had been carried out, in 314 adults (from six centres) who had received treatment of a tooth with deep caries. The teeth had caries lesions involving 75% or more of the dentin and were centrally randomized to stepwise or direct complete excavation. Stepwise excavation resulted in fewer pulp exposures compared with direct complete excavation [difference: 11.4%, 95% confidence interval (CI) (1.2; 21.3)]. At 1 yr of follow-up, the

Long-term effect on tooth vitality of regenerative therapy in deep periodontal bony defects: a retrospective study.

Over the last few decades, many authors have investigated the effect of periodontal disease and treatment on pulpal status with controversial results. This study was conducted to verify whether periodontal disease in a deep intrabony defect and complex therapy, including aggressive root planing such as in periodontal regeneration, have an influence on tooth vitality. One hundred thirty-seven patients who fulfilled the requirements were included. The collected data did not support the need for "preventive" root canal treatment in severely compromised teeth that are planned to undergo periodonta

Long-term maintenance of an apparently hopeless tooth: a case report.

Recognizing the treatment factors that will increase the predictability of regenerative surgical procedures may encourage periodontal treatment of apparently hopeless teeth. This case report describes the long-term clinical outcome of a periodontal (15 mm of clinical attachment loss) and bone defect associated with a periapical lesion affecting a maxillary lateral incisor in an esthetically demanding, systemically healthy young patient. Treatment procedures comprised endodontic treatment of the periapical lesion and cause-related therapy aimed to eliminate bleeding on probing and control the i

Retrograde peri-implantitis: a case report introducing an approach to its management.

BACKGROUND: Although several potential etiologic factors associated with retrograde peri-implantitis (RPI) and potential treatment options have been discussed in the literature, the etiology has not been fully investigated and the definitive management methods remain undefined. We propose a decision-making protocol for the treatment of RPI and provide new insight into the etiology of this process based on the findings from two clinical cases. METHODS: The medical and dental histories of two patients who developed RPI were thoroughly reviewed. Both patients were treated according to the treatm

Five-year evaluation of implants placed at extraction and with dehiscences and fenestration defects augmented with ePTFE membranes: results from a prospective multicenter study.

BACKGROUND: Barrier membranes have been used to promote bone ingrowth on implants with dehiscences and fenestrations. Membranes also have been used to protect defects adjacent to implants placed at the time of extraction. The concept of guided bone regeneration relates to preferentially allowing cells from bone to migrate into various defects while excluding fibrous tissue and epithelium. The purpose of these procedures is to enhance bone-to-implant contact at the treated sites and to prevent mucosal complications. PURPOSE: The purpose of this article is to report clinical outcomes for implan
