Vacuum extraction or caesarean section in the second stage of labour: A systematic review.

BACKGROUND: Prolonged second stage of labour is an important cause of maternal and perinatal morbidity and mortality. Vacuum extraction (VE) and second-stage caesarean section (SSCS) are the most commonly performed obstetric interventions, but the procedure chosen varies widely globally. OBJECTIVES: To compare maternal and perinatal morbidity, mortality and other adverse outcomes after VE versus SSCS. SEARCH STRATEGY: A systematic search was conducted in PubMed, Cochrane and EMBASE. Studies were critically appraised using the Newcastle-Ottawa scale. SELECTION CRITERIA: All artictles includi

A proportional hazards model with time-dependent covariates and time-varying effects for analysis of fetal and infant death.

Birth-weight- and gestational-age-specific perinatal mortality curves intersect when compared by race and maternal smoking. The authors propose a new measure to replace fetal and infant mortality and an analytic strategy to assess the effects of risk factors on this outcome. They used 1998 data for US Blacks and Whites. Age-specific post-last menstrual period (LMP) mortality rate was defined as the proportion of deaths (stillbirth, perinatal death, or infant death) at a given age post-LMP. The authors used extended Cox regression with time-varying covariates and hazard ratios to model the effe…

Perinatal outcome of dichorionic and monochorionic-diamniotic Finnish twins: a historical cohort study.

INTRODUCTION: Although the perinatal mortality of monochorionic twins has been reported to be higher, the role of chorionicity is debated and data from Finland are still lacking. To examine the effect of chorionicity on the main outcome measures, perinatal and neonatal mortality and neonatal morbidity of Finnish twins, a comprehensive population-based historical cohort study was performed at Helsinki University Hospitals. MATERIAL AND METHODS: All 1034 dichorionic and monochorionic-diamniotic twin pregnancies managed at Helsinki University Hospital area during 2006, 2010, 2014 and 2018 were c…

Impact of the COVID-19 pandemic on perinatal care and outcomes: A retrospective study in a tertiary hospital in Northern Ghana.

BACKGROUND: Perinatal mortality remains a global challenge. This challenge may be worsened by the negative effects of the COVID-19 pandemic on maternal and child health. OBJECTIVES: Examine the impact of the COVID-19 pandemic on perinatal care and outcomes in the Tamale Teaching Hospital in northern Ghana. METHODS: A hospital-based retrospective study was conducted in the Tamale Teaching Hospital. We compared antenatal care attendance, total deliveries, cesarean sections, and perinatal mortality before the COVID-19 pandemic (March 1, 2019 to February 28, 2020) and during the COVID-19 pandemi…

Mode of delivery and perinatal results in breech presentation.

OBJECTIVES: Our purpose was to evaluate the outcome of deliveries with fetuses in breech presentation at labor and to compare the results by route of delivery. Specially reviewed were fetuses weighing > or = 1500 gm. STUDY DESIGN: An observational study of consecutive cases of all singleton pregnancies and twin pregnancies with the first fetus presenting in breech delivered at Chicago Lying-In Hospital from July 1980 to December 1987 was performed. Crude perinatal mortality and effect of mode of delivery (cesarean vs vaginal) by weight were compared after correction for nonpreventable causes.…

A case-control study of 1253 twin pregnancies from a 1982-1987 perinatal data base.

In one regional perinatal network between 1982-1987, 101,506 women delivered infants greater than 500 g, of which 1253 were twin pregnancies … anomalies. Fetal and neonatal mortality rates were significantly increased in the twin infants; the perinatal mortality rates for twin A and twin B were 48.8 and 64.1, respectively, compared with 10.4 per 1000 births for the singleton controls. When the twin infants A and B were of similar weight, they had a similar perinatal mortality (odds ratio 1.0; 95% confidence …

[From resuscitation space in the delivery room of the Women's hospital to the perinatal center of the Leipzig University].

Historical review on the development of neonatal intensive care and new-born nursing at the women hospital of Leipzig University from 1969 up to 1995 and the foundation of a Perinatal Center. The successes of perinatal care are demonstrated best by the decline of neonatal mortality of extremely low birth weight infants from more than 90% up to 20% in 1995. It is attributed to the progress in perinatal medicine as well as to a perfect functioning regionalization of high risk pregnancies.

Hepatitis B carrier and perinatal outcome in singleton pregnancy.

The objective of this article is to evaluate the effect of hepatitis B antigenemia on perinatal outcome. Perinatal outcome of 824 women with … fetal distress, perinatal asphyxia, congenital abnormality, gastrointestinal tract abnormality, and perinatal mortality were similar among the two groups. We conclude that the presence of hepatitis B surface antigen in pregnant women does not pose additional risk for the pregnancy.

Perinatal complications associated with preterm deliveries at 24 to 33 weeks and 6 days gestation (2011- 2012): A hospital-based retrospective study.

BACKGROUND: Morbidity and mortality of preterm babies are important issues in perinatal medicine. In developed countries, preterm delivery is the cause of about 70% of mortality and 75% of morbidity in the neonatal period, respectively. OBJECTIVE: The aim of this study was to determine the risk factors for preterm labor and the outcomes, in terms of perinatal mortality and morbidity at the time of discharge home, among preterm infants at less than 34 weeks gestation. MATERIALS AND METHODS: A retrospective study was conducted and all infants with a gestational age of 24 to 33 weeks and 6 days…

Impact of improved perinatal care on the causes of death.

A total of 440 perinatal deaths occurring in a maternity hospital over a 6 year period have been analysed clinically and pathologically. The decline in mortality could be attributed to a reduction in asphyxial deaths, lethal malformations, and macerated stillbirths. The establishment of a neonatal intensive care unit seemed to have been more successful in combating birth asphyxia than respiratory distress syndrome. Although the greatest reduction in perinatal mortality was in babies with birthweights between 1 and 1.5 kg, there was no decline in the deaths from hyaline membrane disease or intr…

[Breech presentations in 1957-1961 and 1967-1975 at the Landesfrauenklinik Magdeburg].

Breech presentation is classed as an abnormality because of the high associated perinatal mortality and morbidity rates and the slightly increased maternal morbidity. A review of 14 years' experience with breech delivery was undertaken to ascertain patterns of care. During this period there were 33 183 births and there were 1271 cases of breech presentation. The method of delivery in the 1271 cases included 91,5 per cent vaginal deliveries and 8,5 per cent cesarean sections. There was an increase to 15,3 per cent during the last 2 years of the review. This review indicates a decline in perinat…

Fetal intestinal perforation and meconium peritonitis associated with maternal autoimmune hepatitis.

Autoimmune hepatitis (AIH) in pregnancy can affect both fetal and maternal outcomes. Little is known regarding the fetal outcomes of AIH in pregnancy. The major risks include spontaneous abortions, fetal mortality, perinatal mortality and prematurity. Two common drugs used in the management of AIH, azathioprine and prednisone, may also be associated with adverse fetal outcomes. We present the case of perinatal focal intestinal perforation with a meconium pseudocyst in a preterm infant of a mother with autoimmune hepatitis on azathioprine and methylprednisone.

Management of patients with poor symphysis pubis-fundus growth by Doppler flow velocimetry of the umbilical artery--an effective method to detect the fetus at risk.

This study evaluated Doppler flow velocimetry of the umbilical artery in patients with suspected intrauterine growth retardation as detected by poor symphysis-fundus growth (SFG). The sensitivity of Doppler in detecting light for gestational age babies and intrauterine growth retardation was poor. Antenatal complications, neonatal morbidity and perinatal mortality occurred significantly more frequently in patients with abnormal flow. Doppler flow velocimetry is an excellent test to identify babies at high risk for perinatal morbidity and mortality, when poor SFG is present.
