Bone-specific kinetic model to quantify periosteal and endosteal blood flow using indocyanine green in fluorescence guided orthopedic surgery.

This letter describes a hybrid plug/compartment (HyPC) kinetic model to fit dynamic indocyanine green fluorescence data acquired in a porcine model of long bone traumatic fracture. Parametric images of periosteal blood flow, endosteal blood flow, total bone blood flow and fraction of endosteal-to-periosteal flow were obtained by applying the HyPC model on a pixel-by-pixel basis. Intraoperative discrimination between healthy and damaged bone could facilitate debridement reducing post-operative complications from non-union and infection. The ability to quantify periosteal and endosteal blood flo

Short Lingual Osteotomy Using a Piezosurgery Ultrasonic Bone-Cutting Device During Sagittal Split Ramus Osteotomy.

Short lingual osteotomy is a useful method for the performance of sagittal split ramus osteotomy involving interference between the proximal and distal bone fragments when lateral differences exist in the setback distance. However, this procedure occasionally results in abnormal fracture and nerve injury; expert surgical skill is thus required. We herein describe a novel technique involving the use of an ultrasonic bone-cutting device (Piezosurgery; Mectron Medical Technology, Carasco, Italy) for vertical osteotomy posterior to the mandibular foramen. Successful short lingual osteotomy was per

A new skill for treating unclassified thumb polydactyly: ablation via a periosteal incision.

BACKGROUND: Thumb polydactyly is a common congenital abnormality of the hand. Surgical treatment includes reduction and reconstruction. A new type of thumb polydactyly and the achieved treatment are reported. METHODS: A 4-year-old boy with thumb polydactyly is reported. The duplicated two thumbs had one complete metacarpal and a separated triphalangeal phalanx. The ulna thumb was removed via periosteal incision, and a whole complex tissue sleeve was used to repair the ulna part of the joint capsule. RESULTS: The postoperative aesthetic and functional results were satisfactory. CONCLUSIONS:

[A case of acute osteomyelitis and subperiosteal abscess caused by Streptococcus pyogenes, presumably triggered by influenza A virus].

A feverish 5-year-old girl diagnosed with influenza A virus from a positive rapid influenza type A antigen test, and suffering pain and redness in her left knee had febrile convulsions and was admitted. Her knee pain and redness increased. She was diagnosed with acute osteomyelitis and subperiosteal abscess based on MRI. We started intravenous antibiotics and punctured and drained the abscess. S. pyogenes was isolated from blood and puncture pus culture and a rapid group A beta hemolytic Streptococcal antigen test was positive in a throat swab. After 10 days' drainage and 28 days' antibiotics

Periosteal reaction in systemic lupus erythematosus.

Musculoskeletal complaints are the most common presenting symptoms in most of the patients with systemic lupus erythematosus (SLE). However, periosteal new bone formation is an extraordinarily rare condition in SLE. We report a case of periosteal reaction in SLE. A 31-year-old woman with SLE presented with both knee pain. Radiographs revealed periosteal reactions in both femur and tibia and around the metaphysis of the right distal tibia. Periosteal reaction can be caused by benign or malignant lesions and infection. We cannot find any other cause of periosteal reaction in our case after thoro

Stress-induced spiculated periosteal reaction appearing as a malignant bone tumor: a case report.

OBJECTIVE: The aim of this study was to describe the appearance of a rare occurrence of a spiculated periosteal reaction caused by stress injury and the subsequent diagnostic assessments. A proposed mechanism for the etiology of stress-induced periosteal reactions in this case is offered. CLINICAL FEATURES: A 54-year-old female had ankle pain for 1 year. Radiographs revealed a spiculated periosteal reaction of the distal fibula. In light of the clinical history of prior breast carcinoma, the possibility of metastatic disease was entertained. INTERVENTION AND OUTCOME: Scintigraphy and magneti

Rest insertion combined with high-frequency loading enhances osteogenesis.

Mechanical loading can significantly affect skeletal adaptation. High-frequency loading can be a potent osteogenic stimulus. Additionally, insertion of rest periods between consecutive loading bouts can be a potent osteogenic stimulus. Thus we investigated whether the insertion of rest-periods between short-term high-frequency loading bouts would augment adaptation in the mature murine skeleton. Right tibiae of skeletally mature (16 wk) female C57BL/6 mice were loaded in cantilever bending at peak of 800 microepsilon, 30 Hz, 5 days/wk for 3 wk. Left tibiae were the contralateral control condit

Treatment of chondral lesions in advanced osteochondritis dissecans: a comparative study of the efficacy of chondrocytes, mesenchymal stem cells, periosteal graft, and mosaicplasty (osteochondral autograft) in animal models.

Management of chondral lesions in osteochondritis dissecans remains a challenge. This study investigated the efficacy of periosteal graft, osteochondroidal autograft, autologous chondrocyte and mesenchymal stem cell transplants in the treatment of chondral lesions in animal models. Full-thickness articular cartilage defects were created in the weight-bearing surface of the medial femoral condyle in 20-week-old NZW rabbits. A total of 56 knees were randomly divided into four groups as follows: group 1, transfer of cultured chondrocytes; group 2, transfer of cultured mesenchymal stem cells; grou

Long-term effects of plate osteosynthesis: comparison of four different plates.

Various phenomena have been observed subsequent to plate osteosynthesis, for example, refracture after plate removal. Experimental research … osteal stripping and subperiosteal plate insertion. It was assumed that applying the plate onto the periosteum would be associated with effects on periosteal blood supply directly related to the plate and consequently cortical porosis. Plate related stress shielding and the implant material had no significant effect on the extent of cortical porosis.

Subperiosteal implantation of octacalcium phosphate (OCP) stimulates both chondrogenesis and osteogenesis in the tibia, but only osteogenesis in the parietal bone of a rat.

BACKGROUND: It is not known whether long bones and calvaria have distinct biological characteristics. Octacalcium phosphate (OCP), which is a precursor phase of the hydroxyapatite, has been reported to stimulate bone formation if implanted in the subperiosteal region of mouse calvaria. The present study was designed to investigate how the long bone and the calvarium respond to OCP implantation and to compare their biological characteristics. METHODS: The synthetic OCP was implanted into the subperiosteal region of rat tibiae and parietal bones being mixed with bovine type I collagen treated b

Anterior cranial base reconstruction: role of galeal and pericranial flaps.

Reconstruction of surgical defects in 30 patients undergoing surgery of the anterior cranial base was performed using pericranial, galeopericranial, and galeal scalp flaps. Twenty-seven patients had resection of neoplasms, the majority of which were malignant. Fifty-seven percent of patients received prior therapy consisting of surgery and/or radiotherapy. Adequate healing of the cranial base was noted in all cases without persistent cerebrospinal fluid leaks, meningitis, or brain herniation. Mucosalization of the intranasal surface was noted. No skin grafts were used. At a median follow-up of

Histopathology of the facial nerve after longitudinal temporal bone fracture.

The temporal bone histopathology in a patient who suffered repeated head trauma and a longitudinal temporal bone fracture shows unusual features associated with the facial nerve. In the distal meatal segment, a type of traumatic neuroma was found with disorganized nerve bundles and distinct areas of Schwann's cell proliferation but lacking fibrosis. Periosteal new bone formation in the labyrinthine segment narrows the fallopian canal and protrudes into the nerve, which completely fills the canal. The tympanic and mastoid segments of the nerve show severe degeneration of nerve fibers and an inc

Extended palatal island mucoperiosteal flap.

The extended palatal island mucoperiosteal flap was used in 53 cases over a ten-year period. Its many uses include reconstruction of the soft palate and resurfacing of retromolar and cheek defects. Its reliability with a success rate of 96% and spontaneous epithelialization of the donor site at three months makes it an attractive method of reconstruction.

Scanning electron microscopy findings in osteogenesis imperfecta fetalis.

A case of osteogenesis imperfecta fetalis in a stillborn black male infant is described. Scanning electron microscopic studies of the skeleton showed normal intercartilaginous septa in the metaphyses of long bones, covered by uniform, calcifying globules 1 mum in diameter. In the diaphyses, however, the orientation of bone trabeculae was abnormal. The trabeculae were thin, with wide and densely packed osteocytic lacunae, and with more resorbing surfaces than in the control specimens. It is concluded that in osteogenesis imperfecta there is abnormal periosteal ossification, together with increa

Cystic fibrosis and hypertrophic osteoarthropathy in children. Report of three cases.

Three patients with cystic fibrosis were noted to have swelling of knee and ankle joints during exacerbation of their lung disease. Synovial fluid was analyzed in one patient and the synovium underwent biopsy in another. These studies excluded other causes of arthritis but did not contribute any new information on the nature of secondary hypertrophic osteoarthropathy. Radiological examination of long bones confirmed the diagnosis of hypertrophic osteoarthropathy in all three patients. Since many patients with cystic fibrosis survive longer, more instances of hypertrophic osteoarthropathy are e
