Abdominal CT findings to distinguish between tuberculous peritonitis and peritoneal carcinomatosis.

OBJECTIVE: Determine useful CT findings for differentiation between tuberculous peritonitis and peritoneal carcinomatosis. MATERIAL AND METHOD: Abdominal CT scans in 27 clinically or pathologically proven cases of tuberculous peritonitis and 53 pathologically proven cases of peritoneal carcinomatosis were retrospectively reviewed. CT findings were assessed for ascites, abnormalities of peritoneum, omentum, and mesentery, abdominal lymphadenopathy, and other associated findings in abdomen, bone, and lung bases. Statistical differences of CT findings between two diseases were analyzed using Chi…

Severe peritonitis in patients treated with peritoneal dialysis: a case series study.

Severe peritonitis causing death and/or technique termination (catheter explanted) is one of the most devastating complications of peritoneal dialysis (PD). The aim of this case series study was to reveal the predictors of risk and clinical characteristics of these cases. We included 38 patients with either peritonitis causing death (18 patients, 47%) or catheter removal (20 patients, 53%) in the period 1996-2006. Their last clinical, laboratory and peritoneal equilibration test data before the peritonitis episode and hospitalization data after the start of peritonitis were reviewed. Their med…

A case of fulminant peritonitis caused by Streptococcus mitis in a patient on peritoneal dialysis.

A 54-year-old woman on peritoneal dialysis (PD) was hospitalized with peritonitis with a high body temperature, abdominal pain and cloudy peritoneal fluid. She progressively fell into septic-like shock within only 6 hours after onset. The causative bacteria were Streptococcus mitis (S. mitis), part of the normal flora of oral cavity, intestine, female genial tract and upper respiratory tract. S. mitis shows pathogenicity for diseases such as endocarditis, brain abscesses and sepsis in children with malignancy or transplantation. However, S. mitis rarely shows severe pathogenic responses in adu…

Atrial natriuretic peptide ameliorates peritoneal fibrosis in rat peritonitis model.

BACKGROUND: Atrial natriuretic peptide (ANP) was recently reported to ameliorate fibrosis in the heart and experimental renal diseases and vascular thickening after balloon injury. Peritoneal fibrosis is an important complication of long-term peritoneal dialysis, and peritonitis is a factor in its onset. In the present study, we investigated the effects of ANP in a rat peritonitis-induced peritoneal fibrosis model. METHODS: As pretreatment, an osmotic pump containing vehicle (saline) or ANP (0.15 or 0.3 μg/min) was inserted through the carotid vein in male Sprague-Dawley rats. ANP or saline w…

Pleiotrophin triggers inflammation and increased peritoneal permeability leading to peritoneal fibrosis.

Long-term peritoneal dialysis induces peritoneal fibrosis with submesothelial fibrotic tissue. Although angiogenesis and inflammatory mediators are involved in peritoneal fibrosis, precise molecular mechanisms are undefined. To study this, we used microarray analysis and compared gene expression profiles of the peritoneum in control and chlorhexidine gluconate (CG)-induced peritoneal fibrosis mice. One of the 43 highly upregulated genes was pleiotrophin, a midkine family member, the expression of which was also upregulated by the solution used to treat mice by peritoneal dialysis. This growth

Mast cells in peritoneal fluid in rats with experimentally induced peritoneal adhesions.

Mast cells (MC) produce, store and release many biologically active substances, especially inflammatory factors, chemotactic substances for neutrophiles, cytokines and prostaglandins. They play very important role in fibrinosis and they are an important factor in peritoneal adhesions formation and lysis. In this study we tried to evaluate role of mast cells in peritoneal adhesions formation. We estimated number of mast cells in peritoneal fluid in rats with experimentally developed peritoneal adhesions. The number of mast cells per ml was counted in flow cytometry in specimens of peritoneal fl

Colonic diverticulosis as a risk factor for peritonitis in Chinese peritoneal dialysis patients.

OBJECTIVES: The risk of development of enteric peritonitis in Chinese peritoneal dialysis (PD) patients with colonic diverticulosis is not known. There have been no reports on whether colonic diverticulosis may affect peritonitis outcomes. The objectives of this study were to examine whether colonic diverticulosis is a risk factor for the development of enteric peritonitis and to study its influence on the outcome of enteric peritonitis. PATIENTS AND METHODS: All continuous ambulatory PD patients that had barium enema and colonoscopic examinations performed between January 1994 and January 20…

Determinants of catheter loss following continuous ambulatory peritoneal dialysis peritonitis.

BACKGROUND: Few patients are able to resume peritoneal dialysis (PD) therapy after an episode of peritonitis that requires catheter removal. PD catheter loss is therefore regarded as an important index of patient morbidity. The aim of the present study was to evaluate factors influencing catheter loss in patients suffering from continuous ambulatory PD (CAPD) peritonitis. PATIENTS AND METHODS: We retrospectively reviewed 579 episodes of CAPD peritonitis from 1999 to 2006 in a tertiary-care referral hospital. Demographic, biochemical, and microbiological characteristics were recorded. Episodes…

Sodium removal in peritoneal dialysis: the role of icodextrin and peritoneal dialysis modalities.

One of the main goals of dialysis is the control of extracellular volume, because inadequate sodium and fluid removal result in fluid overload and increased mortality. In the present study, we evaluated the roles of continuous ambulatory peritoneal dialysis (CAPD), continuous cycling peritoneal dialysis (CCPD), and the use of icodextrin on sodium removal in 29 patients (n = 18 on CAPD, n = 11 on CCPD). Daily removal of sodium by each modality and dialysis adequacy by Kt/V and creatinine clearance were evaluated. A significantly higher amount of sodium was removed in CAPD patients than in CCPD

Role of aldose reductase in the peritoneal changes of patients undergoing peritoneal dialysis.

BACKGROUND/AIMS: The mesothelium of patients undergoing peritoneal dialysis (PD) is exposed to glucose in dialysate. Glucose metabolites 3-deoxyglucosone and advanced glycation endproducts (AGEs) in the PD fluid induce peritoneal damage. Circulating factors also affect the peritoneum in the uremic model and predialysis patients. Aldose reductase (AR) generates precursors of 3-deoxyglucosone. We have reported AR acceleration in uremic patients. Therefore, AR acceleration might affect the peritoneum. The purpose of this study was to evaluate the AR level in PD patients and to determine the facto

[Fungal and tuberculous peritonitis in patients on peritoneal dialysis: characteristics, therapy and outcome].

Peritonitis is a serious clinical complication in patients with terminal chronic renal failure (CRF) on peritoneal dialysis (PD). The incidence of peritonitis varies from center to center, and during the last decade it occurs approximately in one patient during 24-60 therapeutical months, which is the result of good education of patients, but also of employing the new systems for PD. Fungi as well as Mycobacterium tuberculosis are rare causes of peritonitis in patients on PD therapy. The incidence of peritonitis with these two causes varies: 1-15% and 0.7-3%, respectively. The most frequent ca…

Hyaluronan modifies inflammatory response and peritoneal permeability during peritonitis in rats.

The effect of high-molecular-weight hyaluronan (HA) on peritoneal and systemic inflammation and peritoneal permeability to water and solutes was studied during endotoxin-induced peritonitis in rats. Acute peritonitis was induced by adding lipopolysaccharide (LPS) to the dialysis fluid (Dianeal 3.86; Baxter Healthcare, Ireland, Castlebar). HA was added to the dialysis solution in a concentration of 10 mg/dL. During 4- and 8-hour dwells of the dialysis fluid, we studied the intensity of peritoneal (dialysate) and systemic (blood) inflammation (dialysate cell count and differential, cytokine and …

Malignant peritoneal mesothelioma treated by continuous hyperthermic peritoneal perfusion chemotherapy.

Malignant mesothelioma of the peritoneum is a rare tumour for which the therapeutic approach has not yet been standardized. The efficacy of the current regimes is limited. Effective locoregional therapy is crucial, since this tumour is most often confined to the peritoneal cavity at the time of the initial diagnosis and remains there for much of its clinical course. If and when haematogenous metastases occur, they rarely contribute to the death of the patient, which is often caused by the overgrowth of the primary tumour and its local complications. A case of diffuse malignant peritoneal mesot

The standard peritoneal permeability analysis in the rabbit: a longitudinal model for peritoneal dialysis.

OBJECTIVE: The development of an experimental peritoneal dialysis (PD) model in rabbits to investigate peritoneal transport characteristics during a longitudinal follow-up and to assess normal values of these peritoneal transport parameters. DESIGN: Peritoneal transport parameters were determined in conscious, unrestrained rabbits by standard peritoneal permeability analysis adjusted for rabbits (SPAR). In this test a 1-hour dwell with 3.86% glucose dialysate is used. Dextran 70 (1g/L) was added to the dialysate to allow calculation of fluid kinetics. Dialysate samples were taken before, 10,

A scanning electron microscopy study of peritoneal stomata in different peritoneal regions.

Peritoneal stomata constitute the principal pathways for the drainage of intraperitoneal contents from the peritoneal cavity to the lymphatic system and have been claimed to be exclusively restricted to the peritoneal surface of the diaphragm. This concept has been revised by the demonstration of peritoneal stomata in the omental, mesenteric, ovaric and pelvic peritoneum. Therefore, the aim of this study was to further assess peritoneal surfaces of several other abdominal organs and of the abdominal wall with special reference to the occurrence of peritoneal stomata. The peritoneum covering th

The epidermo-peritoneal potential in patients treated with continuous ambulatory peritoneal dialysis.

At physiologic pH, S. epidermidis moves along an electrical potential gradient. We measured the epidermo-peritoneal electrical potential (EPP) in 23 end-stage renal failure patients treated with CAPD. There was a negative correlation between the mean EPP and the patient's age (r = 0.47, p = 0.016), but no correlation between the mean EPP and the duration of treatment (r = 0.003, p = 0.5). The EPP was greater in those patients with a history of recurrent bacterial peritonitis due to S. epidermidis [median EPP 23 mv (95% confidence limits 16-51)] compared to those with only one or no episodes of…

Transfer of autologous haemoglobin from the peritoneal cavity during peritoneal dialysis.

Transfer of autologous haemoglobin from the peritoneal cavity was evaluated retrospectively in 14 patients who received this marker intraperitoneally (group 1) during routine continuous ambulatory peritoneal dialysis (CAPD). Five additional patients were studied during acute peritonitis (group 2). A model for balance of both dialysate volume and amount of haemoglobin is developed to assess movement of the compound into lymph or adjacent tissues. Under the conditions of the study the transfer was slow (8 +/- 10 ml/h) in patients without peritonitis (group 1), and significantly faster (25 +/- 22…

Diverticular disease of the colon: a risk factor for peritonitis in continuous peritoneal dialysis.

To study the possible influence of colonic diverticula on the risk of peritonitis of enteral (intestinal) origin in patients undergoing continuous peritoneal dialysis, a barium enema was carried out as a pretreatment investigation in 129 consecutive patients starting CAPD over a 9-year period. In this prospective study comprising 3103 patient months, a total of 44 (15% of all) episodes of peritonitis with micro-organisms of enteral origin occurred in 30 of the 129 patients. The estimated probability of developing such an episode was 16% and 24%, respectively, within 1 or 2 years of treatment. …
