Use of blood products in pediatric cardiac surgery.

In pediatric cardiac surgery, there is a substantial gap between published recommendations or guidelines for blood product use and clinical practice. The drawbacks of blood transfusion are well acknowledged though. The aim of this paper is to present the rationale for packed red blood cells, fresh frozen plasma (FFP), and platelets used in pediatric patients. Blood hemoglobin level is the current trigger used for packed red blood cells transfusion, though commonly admitted to be suboptimal. An increase in hemoglobin level is likely to be associated with an increase in blood oxygen content and

Successful Vaginal Delivery in a Pregnant Woman With Bernard-Soulier Syndrome: A Case Report.

Bernard-Soulier syndrome (BSS) is a rare inherited platelet disorder that poses major risks during pregnancy because of potential bleeding complications. Managing such cases in low-resource settings is especially challenging. We report the case of a 23-year-old Afghan woman with BSS who achieved a successful full-term vaginal delivery at a tertiary center. Early referral, coordinated management by hematology and obstetric teams, prophylactic platelet transfusions, and prompt treatment of postpartum hemorrhage led to a favorable maternal and neonatal outcome. This experience shows that, even wh…

Microbiological Screening of Bacterial contamination of Platelet Concentrates units: a single center observational study.

BACKGROUND: Bacterial contamination of donated platelets is a major issue in transfusion medicine. Platelet concentrates (PCs) are highly susceptible to bacterial contamination due to their storage conditions at room temperature, which offers a suitable environment for bacterial proliferation. Therefore, the aim of the study is to determine the prevalence of bacterial contamination in the donated platelets units and to identify the isolated microorganisms in samples collected in blood bank services at King Abdullah Medical City (KAMC) in Makkah, Saudi Arabia. MATERIAL AND METHODS: A total of

How do we maintain a dual inventory of room temperature stored platelets with cold-stored platelets?

BACKGROUND: There has been renewed interest in cold-stored platelets (CSPs) for the management of bleeding patients. CSPs might offer advantages over conventional room temperature stored platelet units (RTP), such as a lower risk of bacterial growth during storage, a longer shelf lf life, and possibly improved hemostatic effectiveness. However, CSPs have reduced in vivo survival compared to RTP. In 2023, the FDA issued guidance allowing 14-day storage of CSPs for the treatment of active bleeding when conventional platelets are not available or their use is not practical. This How Do We describ

Massive transfusion: complications and their management.

Massive transfusion, or the rapid administration of a quantity of blood products that approximates an individual's blood volume, is associated with many potentially lethal complications. If the need for transfusion is immediate, ie, before adequate typing and crossmatching procedures can be completed, O negative RBCs can be given safely in the interim. Hypothermia caused by cold banked blood is aggravated by multiple environmental factors and should be aggressively avoided through the use of heat lamps, warming coils, blankets, and other warming devices. The coagulopathy seen in massive transf

Compensatory effect of fibrinogen in a patient with bone marrow aplasia, septic shock, and severe thrombocytopenia guided by thromboelastometry: a case report.

Platelet transfusion is a common practice to prevent spontaneous bleeding or bleeding due to invasive procedures. Transfusion of allogeneic blood components is associated with increased mortality and a worse clinical outcome. The clot strength is assessed by thromboelastometry and determined by the interaction between platelets and fibrinogen. The compensatory effect of high levels of fibrinogen on clot strength in patients with thrombocytopenia has been demonstrated in different clinical settings including sepsis. We report the case of a patient with severe thrombocytopenia whose thromboelast…

Platelet concentrates in macular hole surgery. A journey through the labyrinth of terminology, preparation, and application: a comprehensive review.

The surgical management of macular holes is undergoing continuous evolution, with recent focus on the utilization of platelet concentrates as a promising adjunctive intervention. Currently, they present a valid surgical approach for achieving anatomical and functional success with a non-inferiority comparably to the alternative surgical techniques. Nonetheless, the utilization of varied platelet concentrates terminologies, coupled with the lack of standardization in their preparation methodologies, engenders both lexical confusion and challenges in comparing scientific studies published up unt

Recurrent menorrhagia in an adolescent with a platelet secretion defect.

BACKGROUND: Although von Willebrand disease is the most common inherited bleeding disorder, platelet function disorders are less well recognized as a cause of bleeding. We report a case of menorrhagia caused by an unsuspected platelet secretion defect. CASE: A 13-year-old Asian female, with unknown family history, presented with menorrhagia not responsive to intravenous conjugated estrogens, requiring transfusion of 7 units of packed red blood cells. Initial screening tests for bleeding disorders were normal; however, due to high clinical suspicion, further specific testing with platelet aggr

Thrombocytopenia and its management in the surgical patient needing multiple reconstructive procedures.

Thrombocytopenic patients with surgically correctable lesions such as radial clubhands are too often repeatedly transfused with randomly pooled platelets. This is followed by a clinical rejection of the foreign protein and rapid destruction of the platelets. There may be excessive bleeding episodes. Tissue typing technics now available can provide antigenically compatible platelet concentrates which elicit few of these reactions. Therefore, multiple reconstructive procedures can be achieved with a greater safety margin than previously possible.

H12-(ADP)-liposomes for hemorrhagic shock in thrombocytopenia: Mesenteric artery injury model in rabbits.

BACKGROUND: Damage control resuscitation improves patient outcomes after severe hemorrhage and coagulopathy. However, effective hemostasis methods for these critical situations are lacking. OBJECTIVE: We evaluated the hemostatic efficacy of fibrinogen γ-chain (HHLGGAKQAGDV, H12)-coated, adenosine-diphosphate (ADP)-encapsulated liposomes (H12-[ADP]-liposomes) in thrombocytopenic rabbits with hemorrhagic shock. METHODS: Acute thrombocytopenia (80%) was induced in rabbits that also received mesenteric vessel injury, leading to hemorrhagic shock. Five minutes after injury, subjects received intr

[Indications of the different components of blood and outcome of transfusion practices in postpartum hemorrhage].

Despite a high incidence of mortality associated with post-partum haemorrhage, indications for transfusion of blood and its components in obstetrics remain ill defined. Transfusion is often massive if needed. Transfusion management depends upon appreciation of blood loss, with systematic research of associated disseminated intravascular coagulopathy (DIC) or other preexisting coagulation disorders. If DIC is ruled out, the classic rules of volemic substitution apply, besides certain qualitative characteristics intrinsic to the blood components or products used. Treatment of DIC calls for fresh

Genetics by genetic algorithm: Defining an ideal platelet donor population to support patients with HLA-mediated alloimmune refractoriness.

BACKGROUND: Immune factors underlie approximately 20% of patients exhibiting platelet transfusion refractoriness (PTR). PTR can be mitigated by supplying HLA-A and B compatible platelets to these patients. Since 2019, Canadian Blood Services has managed a program to provide compatible platelets to alloimmunized patients. This study evaluates donor pool size and ethnic diversity options to serve Canada's population. STUDY DESIGN AND METHODS: We adapted simulation-based stem cell matching methods by generating simulated patients and matching them to existing and simulated donors using a Python …

The treatment of idiopathic thrombocytopenia with vinblastine-loaded platelets.

We devised a method to enhance delivery of vinblastine to macrophages, the cells believed to be responsible for platelet destruction in idiopathic thrombocytopenic purpura. Our strategy was based on the ability of platelets to bind vinca alkaloids such as vinblastine. Platelets were incubated with an excess of vinblastine, concentrated and then, after excess alkaloid had been removed, given to 11 patients with idiopathic thrombocytopenia refractory to other treatment (including intravenous injections of vinca alkaloids). Platelet antibodies in the patients' plasma led to ingestion of the vinca

Correction of Severe Myelofibrosis, Impaired Platelet Functions and Abnormalities in a Patient with Gray Platelet Syndrome Successfully Treated by Stem Cell Transplantation.

Gray platelet syndrome (GPS) is an inherited disorder. Patients harboring GPS have thrombocytopenia with large platelets lacking α-granules. A long-term complication is myelofibrosis with pancytopenia. Hematopoietic stem cell transplant (HSCT) could be a curative treatment. We report a male GPS patient with severe pancytopenia, splenomegaly and a secondary myelofibrosis needing red blood cells transfusion. He received an HSCT from a 10/10 matched HLA-unrelated donor after a myeloablative conditioning regimen. Transfusion independence occurred at day+21, with a documented neutrophil engraftment

[Anesthetic management of cesarean section for a patient with idiopathic thrombocytopenic purpura].

A 39-year-old parturient with idiopathic thrombocytopenic purpura (ITP) was scheduled for cesarean section at 37 weeks gestation. ITP, diagnosed during the first pregnancy, recurred during the second pregnancy, and she was treated with high dose gamma-globulin and platelets transfusion to increase her platelets count over 5.0 x 10(4) x microliter-1 before cesarean section. During the operation under general anesthesia with propofol and pentazocin, atonic hemorrhage occurred gradually with increasing blood loss after the parturition. Since administration of oxytocin, ergometrine maleate, and pr

[Alloimmune thrombocytopenia in the newborn infant caused by maternal PlA1 antibodies].

The case of pregnant woman, whose first child was born with neonatal alloimmune thrombocytopenia (NATP) and PlA2/PlA1 constellation, is used as an example to draw up a programme for the care of pregnant women whose children are expected to be born with NATP. In one of 2,000 to one of 3,000 cases, the PlA1-positive foetal platelets cause a sensitization of the PlA1-negative mother. Transplacental passage of maternal IgG alloantibodies leads to accelerated destruction of foetal platelets. The pathophysiologic process is similar to Rh-erythroblastosis foetalis. The infants show symptoms ranging f
