Do clerkship directors think medical students are prepared for the clerkship years?

PURPOSE: Educators have begun to question whether medical students are adequately prepared for the core clerkships. Inadequate preclerkship preparation may hinder learning and may be predictive of future achievement. This study assessed and compared the views of clerkship directors regarding student preparation for the core clinical clerkships in six key competencies. METHOD: In 2002, a national survey was conducted of 190 clerkship directors in internal medicine, family medicine, pediatrics, surgery, obstetrics/gynecology, and psychiatry from 32 U.S. medical schools. Clerkship directors were

Patient references in the 2005 and 2015 CanMEDS frameworks.

BACKGROUND: Patient involvement in postgraduate medical education (PGME) can help residents improve their communication, professionalism, and collaboration. The CanMEDS Framework defines such competencies for physicians and informs teaching and assessment activities in PGME. However, it is unclear how patients are referenced in the CanMEDS Framework and if these references encourage the active involvement of patients in PGME. To inform how patients are referenced in the revisions of the CanMEDS Framework, scheduled for publication in 2025, our aim was to determine how patients are referenced i

A boy and his AAC team: building instructional competence across team members.

Families and education professionals face unique challenges when working together to support augmentative and alternative communication (AAC) for a child who does not use speech and has only prelinguistic forms of communication. For this study, a family-school team was provided with a specially designed support package comprising training and one-on-one coaching in evidence-based instructional strategies. As part of a larger mixed methods investigation, a single-case experimental design was conducted to explore the effectiveness of the support package on facilitating team members' competence i

Pediatric pain management - what are we able to do in Polish emergency medical services?

Pain management in emergency medical care remains underestimated, especially in pediatric patients. This is due to neglecting the issue, lack of knowledge in the appropriate use of analgesics and finally- the fear of being criticized by physicians in hospital departments. Moreover, it is difficult to objectively assess a child with acute pain. Even experienced "adult" anesthesiologists wonder how to recognize if a child suffers strong pain or is just anxious. Many different pain scales are available and some of them may be used in emergency medical care.. The administration of analgesia used i

Dyslexic doctors: a resource in need of discovery.

Dyslexic students are applying for admission to medical school. In most cases they have worked hard in college, and their credentials are good (with the likely exception of a low score on Skills Analysis--Reading on the Medical College Admissions Test). In the past a number of well known dyslexics have made substantial contributions in various disciplines, including a few in medicine. Without appropriate assistance, many dyslexic students will be unable to compete successfully and to complete medical school. With appropriate assistance, however, they can complete medical school and become comp

Web-based Discussion Forums on Pregnancy Complaints and Maternal Health Literacy in Norway: A Qualitative Study.

BACKGROUND: The Internet is one of the fastest growing information sources for pregnant women and seems to be used across social and economic strata. However, we still lack knowledge on how interaction in Web-based discussion forums influence maternal health literacy, in terms of how pregnant women access, appraise, and apply information to promote and maintain good health. OBJECTIVE: The aim of this study was to understand how Web-based discussion forums influence maternal health literacy; hence, we explored the role of interactions in Web-based discussion forums among women who experienced

Development of Entrustable Professional Activities for Advanced Inflammatory Bowel Disease Fellowship Training in the United States.

BACKGROUND: The level of inflammatory bowel disease (IBD) training in general gastroenterology fellowship is often insufficient to prepare trainees to deliver advanced IBD care in practice. Advanced IBD fellowships have been developed to fill this training gap, but there is no established curriculum, and significant variability exists across programs. Entrustable professional activities (EPAs) are practical and realistic objectives that define essential tasks of a specialty that physicians should master to be competent during independent practice. The American College of Gastroenterology (ACG)

INTRODUCTION: Global trends in occupational therapy practice such as telehealth, community-based practice, and culturally safe care have become integral in occupational therapy. It highlights the need for curricula that integrate emerging practices alongside core competencies of practitioners. Thus, understanding occupational therapy graduates' perspectives concerning their educational curriculum is important as it provides insights into their experiences, identifies gaps in education, and ensures alignment with evolving professional demands. These perspectives further assist in refining curri

Physician-Led Teams: A Model Curriculum for Emergency Medicine Residency.

BACKGROUND: Physician-led teams in emergency medicine (EM) are associated with higher quality and value than independent non-physician practice, yet most residents receive little formal preparation in leading inter-professional teams that include nurse practitioners (NPs) and physician assistants (PAs). Clear, scalable training guidance for residency programs is lacking. METHODS: We conducted a narrative synthesis of national policy positions, scope-of-practice trends, and published literature on team leadership training in EM and related disciplines, including other specialties, nursing, and

Australian speech pathologists' views of what professional practices lead to successful outcomes of therapy.

The relative importance attributed to factors perceived by speech pathologists as contributing to successful levels of therapeutic outcomes were addressed in a survey of 372 practicing therapists in Australia. The survey consisted of 42 items derived from interviews with a sample of ten speech pathologists in Sydney. Therapists were asked to rate the importance of each item on a seven-point scale ranging from "not at all important" to "critically important." The findings indicate that the most important ingredients of successful therapy incorporate a mix of support and therapeutic activity out

An Authentic Learning Experience for Medical Students on Conducting a Family Meeting.

BackgroundMedical schools often lack training for serious illness conversations with patients and caregivers. We developed a curriculum in our elective Transitioning to Residency medical student course, focused on end-of-life discussions. This paper provides an overview of the curriculum and outcomes from an advanced preparation assignment and student evaluations.MethodsThe curriculum included a "hands-on" skills session delivered via Zoom. Small groups of students (3-4) assumed roles on an interprofessional team (Intensivist, cardiologist, nurse, social worker). They met with two adult childr

The role of therapist MI skill and client change talk class membership predicting dual alcohol and sex risk outcomes.

OBJECTIVE: We investigated the technical model of motivational interviewing (MI) in a dual-outcome intervention (i.e., alcohol, sexual risk; N = 164; 57% female). METHOD: We identified latent classes of client change statements, based on the proportion of change talk (CT) over the session. We then examined whether outcomes were related to CT class, and whether the relations between MI skill and outcomes varied by CT class. RESULTS: We found three classes of alcohol-CT and two classes of sexual risk-CT. While CT class membership did not predict outcomes directly, greater therapist MI-consiste

Finding peace of mind when there still is so much left undone-A diary study on how job stress, competence need satisfaction, and proactive work behavior contribute to work-related rumination during the weekend.

Unfinished work tasks have been identified as a significant job-related stressor in recent occupational stress research. Extending this research, we examine how and when not finishing one's tasks by the end of the work week affects work-related rumination at the weekend. Drawing on control theory, we examined competence need satisfaction as a mediating mechanism that links unfinished tasks at the end of the work week to work-related rumination at the weekend. Furthermore, we scrutinized whether proactive work behavior within the work week may neutralize the detrimental effects of unfinished ta

The Effect of Cerebellar Transcranial Direct Current Stimulation on A Throwing Task Depends on Individual Level of Task Performance.

The effect of cerebellar transcranial direct current stimulation (tDCS) on motor performance remains controversial. Some studies suggest that the effect of tDCS depends upon task-difficulty and individual level of task performance. Here, we investigated whether the effect of cerebellar tDCS on the motor performance depends upon the individual's level of performance. Twenty-four naïve participants practiced dart throwing while receiving a 2-mA cerebellar tDCS for 20 min under three stimulus conditions (anodal-, cathodal-, and sham-tDCS) on separate days with a double-blind, counter-balanced cro
