BACKGROUND: Faecal incontinence commonly affects women, principally because of childbirth. Our aims were to determine the functional effect of childbirth on the pressures generated by the anal sphincter and to determine the patterns of injury to the sphincter.

A prospective study of postoperative mobilization of flexor tendon repairs in zone 5 was conducted over a 2-year period between 1994 and 1996 using a controlled active motion (active extension-active flexion) regimen of mobilization. Fifty-two patients, who had a total of 151 flexor digitorum superficialis (FDS) and 103 flexor digitorum profundus (FDP) divisions, were available for review at a mean follow-up of 10 months. Of the 161 fingers with division of one or both flexor tendons, 66% exhibited independent FDS function and 90% achieved good or excellent results of digital range of motion.

BACKGROUND: The acquired immunodeficiency syndrome (AIDS) has transformed in a illness pandemic and it is thought that 120 million of people will be infected by the year 2000. About thirty five percent of HIV positive patients will have rectal manifestations during their illness, and from these the most important anorectal manifestation are perianal sepsis, anal condyloma, hemorrhoidal disease, chronic diarrhea syndrome, anal ulcer and anorectal tumors.
PURPOSE: To determinate in our medical environment which anorectal manifestation have the most frequency in HIV positive patients.

The authors examined the associations between dietary factors and the risk of duodenal ulcer in a prospective cohort of 47,806 men, aged 40-75 years, who were free of diagnosed gastric or duodenal ulcer or cancer. Fifty consecutive patients with fever and cirrhosis were prospectively studied to assess if cirrhotic fever was a true clinical entity and to determine its characteristics and outcome. The etiology of non-Hodgkin's lymphomas (NHL), including chronic lymphocytic leukemia (CLL), is likely to be related to immune function. Anger, aggression, or hostility, or an emotional state consistent with these terms, has been accepted as being of fundamental importance to our understanding of suicidal behavior. However, the presumed association has not been investigated thoroughly in prospective studies. The results of the present 8-year longitudinal examination of anger with oneself and with society and its relationship to emerging suicidal ideation and attempted suicide in adolescents entering adulthood provide, at best, only equivocal support for such an association.

Topiramate is a new antiepileptic drug, which is not yet marketed in Germany. The anticonvulsive activity is probably mediated by sodium-channel blockade combined with gabaergic and weak antiglutamatergic properties. We investigated 23 patients with partial seizures in this open prospective study. The efficacy was analysed under stable concurrent antiepileptic drugs. Two of the patients became seizure free. In this prospective study, the parachuting injuries which occurred during 2031 jumps in basic courses of free fall were compared with the injuries occurring during 2468 jumps for reserve paratroopers on training exercises. Fifty-eight injuries were recorded in 51 paratroopers. The ankle was most commonly affected, and 80 per cent of the injuries involved the lower extremity. Only 14 per cent of the injured troopers suffered severe injuries (fractures, knee ligament ruptures). The injury rate for paratroopers on basic courses (19.7 injuries per 1000 jumps) was significantly higher (P < 0.0001)

During a prospective study of cerebrovascular accidents we evaluated 64 cases of cerebral venous thrombosis (CVT) in pregnancy and puerperium. CVT made up approximately half of the young strokes and 40% of strokes occurring in females. Majority of them were below 25 years of age, multipara women from rural areas who were delivered at home by untrained dais. A total of 700 patients who had carotid endarterectomy (CEA) in the UK and Ireland during a 6-month interval between March and August 1994 were studied prospectively.
