Understanding psychotic-like experiences in children in the context of dimensions of psychological problems.

INTRODUCTION: Although psychotic behaviors can be difficult to assess in children, early identification of children at high risk for the eme … with a wide range of psychological problems through the general psychopathology factor even before psychotic disorders typically manifest. This study supports the need for longitudinal analyses of future waves of the ABCD Study to determine if PLEs can detect children at high risk for serious psychological problems in adulthood.

Schizophrenia symptom severity and motivations for gambling in individuals with schizophrenia or schizoaffective disorder.

Individuals with schizophrenia are over three times more likely to have problem and pathological gambling (PPG) than the general population (Cunningham-Williams et al., 1998; Desai and Potenza, 2009), but little is known about this co-occurrence and how PPG relates to specific symptom dimensions of psychotic disorders. Although cognitive distortions in PPG have been linked to gambling motivations (e.g., distorted thoughts about odds of winning), how psychotic symptoms in schizophrenia or related disorders relate to gambling motivations have not been examined systematically to date. Individuals…

The uses and outcomes of quetiapine in depressive and bipolar mood disorders in clinical practice.

We conducted an electronic chart review of a sample of all people attending secondary mental health care, in the county of Lanarkshire, Scotland, who were commenced on quetiapine for the following mood disorders: non-psychotic depression (n = 171), psychotic depression (n = 39), bipolar mania (n = 24), bipolar depression (n = 38) and bipolar mixed states (n = 31), between 2002 and 2007. We retrospectively assigned severity and improvement Clinical Global Impression (CGI) scores to measure effectiveness. Quetiapine was co-prescribed with antidepressants in 75-97% of depressive disorders. Commen

Course of Cognitive Development From Infancy to Early Adulthood in the Psychosis Spectrum.

IMPORTANCE: Most patients with psychotic disorders experience severe cognitive impairment, but the onset and course of this impairment remain unclear. Moreover, the course of cognitive functions in other psychiatric conditions remains largely unexamined. OBJECTIVE: To chart the course of general and specific cognitive functions in individuals with psychotic disorders, psychotic experiences, and depression. DESIGN, SETTING, AND PARTICIPANTS: The Avon Longitudinal Study of Parents and Children (ALSPAC) is a prospective cohort study comprising all live births between April 1, 1991, and December…

Vitamin D and psychosis: mini meta-analysis.

Individuals with psychotic disorders are more likely to have vitamin D (VD) deficiency, while evidence suggests VD could have pathophysiological roles. We summarized meta-analytically the available evidence on VD levels in psychotic disorders in comparison with healthy controls and other psychiatric illnesses. We found seven studies, all reporting insufficient VD levels in patients with psychosis. Schizophrenia had a medium effect size for lower VD than healthy controls, and a trend for lower levels than other psychoses. There were non-significant differences between schizophrenia and major de…

Sleep-onset rapid eye movement periods in neuropsychiatric disorders: implications for the pathophysiology of psychosis.

This paper reviews the literature describing the occurrence of sleep-onset rapid eye movement periods in narcolepsy, schizophrenia, psychotic depression, and delirium tremens; the association of narcolepsy with psychotic disorders; the neuropathology of the brainstem in narcolepsy and schizophrenia; and other behavioral disorders resulting from probable brainstem pathology. These findings suggest that some forms of psychosis are a manifestation of pathophysiological changes in the brainstem. Some implications of this hypothesis for the treatment of psychoses are discussed. Future research shou…

[Imagery in psychosis: EMDR as a new intervention in the treatment of delusions and auditory hallucinations].

BACKGROUND: Historically, psychotherapy has focused on the treatment of patients' verbal representations (thoughts) and has proved particularly successful in the cognitive behavioural treatment of psychosis. However, there is mounting evidence that visual representations (imagery) play an important role in the onset and maintenance of psychiatric disorders, including psychotic symptoms. There are indications that heightened emotionality and vividness of visual representations are associated with severity of psychotic experiences. This may imply that a reduction in the vividness and emotionalit

Predictors of persistent psychotic symptoms in persons with methamphetamine abuse receiving psychiatric treatment.

The objective of this study was to identify predictors of sustained psychotic symptoms after methamphetamine (MA) abuse during the course of 6 months from patterns of MA and other substance use, depressive symptoms, family history of psychosis, antisocial personality disorder, and trauma history. A total of 295 individuals with MA abuse and psychotic symptoms seeking psychiatric services were assessed at baseline and then monthly on symptoms and substance use for 6 months. Trajectory analyses revealed two trajectories of the individuals with positive symptoms, with one group presenting with pe

Disordered eating: a defense against psychosis?

The authors present four cases suffering with either bulimia nervosa or anorexia nervosa in conjunction with a psychotic illness. In all cases there appeared to be a reciprocal relationship between the eating disorder and psychosis such that improvement in eating precipitated or exacerbated the psychotic symptoms. We suggest that disordered eating serves as a defense against psychosis. Difficulties in treating such patients are discussed.

Genetic architecture of 11 major psychiatric disorders at biobehavioral, functional genomic and molecular genetic levels of analysis.

We interrogate the joint genetic architecture of 11 major psychiatric disorders at biobehavioral, functional genomic and molecular genetic levels of analysis. We identify four broad factors (neurodevelopmental, compulsive, psychotic and internalizing) that underlie genetic correlations among the disorders and test whether these factors adequately explain their genetic correlations with biobehavioral traits. We introduce stratified genomic structural equation modeling, which we use to identify gene sets that disproportionately contribute to genetic risk sharing. This includes protein-truncating

Stress and psychosis: towards the development of new models of investigation.

The experience of stress is commonly implicated in the onset and maintenance of psychotic disorders such as schizophrenia. Previous studies that have addressed this relationship have had mixed results and serious methodological flaws associated with study design are common. One central limitation is the over-reliance on the experience of life events as a measure of the experience of stress. Research in the general stress literature suggest that attention also needs to be paid to the experience of other types of stressful events (such as 'hassles') as well as qualitative appraisals of events to…

Psychotic symptoms in the elderly: a prospective study in a population sample.

BACKGROUND: Most of what is known about psychotic symptoms in older persons is based on hospital series of severe disorders. METHOD: In the course of a longitudinal community-based survey of older persons, the presence of psychotic symptoms was assessed on two occasions 3-4 years apart. RESULTS: The point prevalence was 5.7%. Risk factors were cognitive impairment, living alone, being male, having had limited education, social isolation, poor health and depressive symptoms. Of the 65 persons who were symptomatic at the start, only six of those still alive had one or more psychotic symptoms a

[THE ROLE OF STRESS IN MANIC-PSYCHOTIC EPISODE DURING THE TERROR ATTACK OF OCTOBER 7 - CASE REPORT].

INTRODUCTION: The bio-psycho-social model is mostly used to understand the etiology and pathogenesis of psychiatric disorders. Even though in our era, the biological factors became more dominant in the field, the stress-diathesis model is quite acceptable to explain and understand the evolution of psychotic as well as affective disorders. BACKGROUND: In this case report we present a patient, in her late 40's, admitted to our department with a manic-psychotic episode for the first time in her life, after the massive terror attack of October 7, and in which we suggest that the signs and symptom

Pathological laughter as an obsessive-compulsive phenomenon.

METHOD: Pathological laughter has been known to occur in organic and functional psychotic disorders and is sometimes seen in hysteria. Though it is an important phenomenon, its significance in other psychiatric disorders has been negligible. This case report highlights the phenomenology of pathological laughter as an obsessive-compulsive phenomenon in an adolescent girl. RESULTS: The girl was brought by her parents with a history of laughing without reason, but on detail evaluation, it was found that her laughing was an irrational and ego-dystonic phenomenon. She also showed resistance to her…

Psychosis risk syndrome is not prodromal psychosis.

One of the most exciting trends in schizophrenia research is the shift in focus from treatment studies to studies about the early identification and prevention of schizophrenia. These studies have primarily focused on adolescents or young adults with prodromal symptoms or on clinically high-risk individuals who show similar impairments in cognitive and social functioning to those seen in individuals with schizophrenia and, thus, are considered at high risk of developing schizophrenia or other psychotic disorders. Some researchers have labeled this condition as psychosis risk syndrome (PRS). Th…

First-onset functional brief psychoses in the elderly.

BACKGROUND AND PURPOSE: The origin and nosological status of psychotic states first arising in late life remain uncertain. We aimed to evaluate the diagnostic stability of brief psychoses with late-life onset. METHODS: A 10-year retrospective analysis of all records of elderly patients with a first-ever episode of psychosis was undertaken. RESULTS: Of 2,072 admissions of elderly patients, 604 had their first brief psychotic disorder (International Classification of Diseases diagnoses). All "organic" psychoses were excluded. The study sample comprised 83 individuals (36 male, 47 female) with

Shared Obsessive-Compulsive Disorder: An Italian Case Report.

Shared obsessive-compulsive disorder (S-OCD) has seldom been reported in the literature, whereas shared psychotic disorder has been described since the end of 19th century. The aim of this report was to describe a case of S-OCD in a married couple. A 38-year-old OCD patient, after several pharmacological trials, eventually improved when a combination of escitalopram and risperidone was prescribed. During his symptoms' remission, his wife started developing the same OCD symptoms. She was prescribed escitalopram, but, given her inconsistent compliance, it was difficult to obtain full effectivene
