Pulpal and periradicular response to caries: current management and regenerative options.

The pulp-dentin complex is a strategic and dynamic barrier to various insults that plague the dentition. Researchers have yet to understand the complete potential of this shifting junction and its components. The most common cause of injury to the pulp-dentin complex is carious breakdown of enamel and dentin. In recent years, there has been a change in restorative management of caries. The emphasis is on strategies to preserve dentin and protect the pulp. This article provides a brief review of the effect of caries on the pulp, of subsequent events on the periradicular tissues, and of current

Periapical radiolucency not requiring endodontic therapy: an unusual case.

The article is a case report of an unusual manifestation of primary hyperparathyroidism seen in the mandible. Primary hyperparathyroidism is a rare disorder that can present its first symptoms in the jaws. The pre- and post-treatment radiographic features of such cases have only rarely been reported in dental literature. This case report highlights the importance of careful clinical and radiographic examination before commencing root canal treatment.

Paraesthesia of the lip and chin area resolved by endodontic treatment: A case report and review of literature.

Lower lip numbness is a symptom that may be due to entirely benign causes, or it may be the first sign of a more sinister problem. It has been reported as being the sole symptom of pathological lesions and metastatic tumours in the mandible.

Multiple myeloma presenting as a periapical lesion in the mandible.

The case reported here demonstrates an asymptomatic radiolucent lesion of plasma cell myeloma mimicking as a periapical cyst. The tooth involved was not carious, vital and did not show any root resorption.

Detection of epidermal growth factor receptor in inflammatory periapical lesions.

Epithelial cell proliferation is often observed in periapical lesions of endodontic origin. The mechanisms which stimulate the epithelial cell rests of Malassez to proliferate are not understood fully. Fifteen inflammatory periapical lesions (10 granulomas and five cysts) obtained from periapical surgery and six additional periapical lesions (four granulomas and two cysts) collected from extracted teeth were examined using immunohistochemical staining and 125I-EGF (epidermal growth factor) binding assay to detect the presence of epidermal growth factor receptor. The results indicated that the

Production of interleukin-1 by polymorphonuclear leukocytes resident in periradicular tissue.

Twenty-one patients undergoing endodontic surgery were identified. Periradicular tissue samples were recovered, and those showing significant numbers of polymorphonuclear leukocyte (PMN) infiltration were prepared for immunoperoxidase identification of interleukin (IL)-1 alpha and IL-1 beta-producing cells using specific polyclonal antibodies. In selected tissue specimens, 90% or more of the PMN's were found to stain positively for IL-1 alpha and IL-1 beta. In addition, significant numbers of plasma cells and tissue histiocytes stained positively for these IL's. Cell suspensions from selected

Analysis of maxillary sinusitis using computed tomography.

Odontogenic maxillary sinusitis can be defined as sinusitis induced by a dental lesion. We examined the CT findings of 68 patients with maxillary sinusitis in order to differentiate between inflammation of sinus origin and inflammation of dental origin. Maxillary sinusitis was classified into four types according to clinical symptoms, history and conventional radiographic findings: type 1, simple sinusitis; type 2, odontogenic sinusitis; type 3, mixed sinusitis; type 4, slight sinus abnormality with a dental lesion. The relationship between the type of maxillary sinusitis and CT findings was a

[The causes of post-endodontic failure: a descriptive analysis of a population of 198 cases of surgical retreatment].

In this article, a statistical analysis describes 198 cases of post-endodontic failures referred from private practitioners for surgical retreatments. Biopsies of these lesions showed that 68.18 per cent of them were identified as apical granulomas, 3.5 per cent as cysts, 16.16 per cent as epitheliated granulomas and 12.12 per cent as "other" cases. The two main groups of causes associated with these failures are: technical errors (55.05 per cent of the cases) and procedural mistakes (17.68 per cent).

Biomedical engineering for the conservation of teeth--the use of a Nd-YAG laser for a treatment of apical focus.

When the apical focus (root cyst) is treated, if the tooth concerned can be preserved, the root canal should first be enlarged and cleaned sufficiently, and then sealed with filling materials. However, if an expensive prothesis is placed on the crown of the tooth concerned, or if the root canal is blocked with a post core of calcification, root canal treatment becomes difficult from a practical point of view. We therefore tried irradiation using the Nd-YAG laser, which is known for its high transmissibility into teeth, to the root canal, and the apical area. In its histological images it is co

A radiographic survey of periapical jawbone lesions.

A radiographic survey of the jawbone adjacent to the teeth revealed a high incidence of bone pathosis in 889 randomly chosen patients. Jaw lesions, nearly all inflammatory, were present in every second person. Bone-destructive inflammatory processes (rarefying osteitis) were the most frequently encountered lesions, occurring in both the maxilla and the mandible. Bone-appositioning inflammatory processes (condensing osteitis), on the other hand, appeared mostly in the mandible, very often involving the first molar, thus indicating the differing biologic behavior of the two jawbones. Other bony

Differential diagnosis of periapical radiolucent lesions.

When a clinician is confronted with an apical radiolucent lesion he must follow an organized thought process and diagnostic technique prior to therapy. This technique employs the collection of data, called the diagnostic data base, and radiographic data. The diagnostic data base and radiographic data combined with the results of the vitality tests will provide the clinician with an appropriate working diagnosis. If the data support a diagnosis of radiolucent lesion associated with a nonvital tooth then endodontic therapy should be provided and the patient should be seen on a routine follow-up

Ectopic meningioma of the mandible in a 20-year-old woman: a case report and literature review.

Ectopic meningiomas are a very rare tumor entity. We present a case of a meningioma arising in the mandible of a young woman and initially supposed to be a radicular cyst. Histopathological and immunhistochemical evaluation showed typical cell characteristics of a meningioma. Only six cases of ectopic meningiomas in the mandible have been described in the literature until now, mainly in women at an advanced age and with surgical removal of all tumors. For the first time, no surgical excision has been performed in this case and follow-up control after 12 months showed no significant progression…

In this case, the six-month follow-up OPG following surgery revealed no recurrence, although a close follow-up is recommended because of the high recurrence rate.
