Association between serum vitamin D levels and pediatric distal radius fractures: a case-control study.

This study evaluated whether serum vitamin D levels are associated with distal radius fractures in children. In this case-control study, pediatric patients with confirmed distal radius fractures (cases) were compared to children visiting an orthopedic clinic for routine checks or minor muscular pain (controls). Serum vitamin D, phosphorus, and calcium levels were analyzed. We enrolled 98 children (mean age: 8.2 ± 2.6 years). Overall, 87.8% had serum 25-hydroxyvitamin D [25(OH)D] concentrations below 30 ng/ml, with a cohort mean of 22.04 ± 7.35 ng/ml. No statistically significant differences we…

A Step-by-Step Protocol for Manual Reduction and Splint Immobilization of Distal Radius Fractures.

Distal radius fractures (DRFs) are among the most common upper extremity fractures encountered in clinical practice. Closed reduction followed by external immobilization remains a fundamental treatment approach; however, the procedure is highly operator-dependent and lacks standardization. This article presents a step-by-step protocol for manual reduction and four-splint immobilization using volar, dorsal, radial, and ulnar splints for acute DRFs. The protocol encompasses patient selection, anesthesia administration, patient positioning, sustained longitudinal traction with countertraction, re…

Second Metacarpal Index as a Predictor of Secondary Displacement in Conservatively Treated Distal Radius Fractures.

Background and Objectives: Distal radius fractures (DRFs) represent the most common fracture in the elderly population and are typically the first fractures to occur in the sequence of fragility fractures. Although fracture instability is an important criteria for guiding treatment, there is no universal consensus on how to define an unstable DRF. Given the demonstrated influence of bone quality on fracture stability, it has been hypothesized that second metacarpal index (2MCI) may also serve as a predictor of instability in DRFs. This study aimed to evaluate the predictive value of 2MCI for f…

FPL Reconstruction After Rupture Following Volar Plate Fixation of Distal Radius Fractures: A Case Series.

PURPOSE: Volar locking plate (VLP) of distal radius fractures (DRF) carries a number of surgical risks including flexor pollicis longus (FPL) rupture secondary to irritation from prominent hardware. This study reviews FPL reconstruction (with interposition graft or tendon transfer) after VLP-related rupture. METHODS: This retrospective case series reports the experience at one tertiary center with FPL reconstruction after DRF. Patients treated between January 1, 2013, and June 12, 2023, were eligible for inclusion. Preoperative radiographs were analyzed for Soong grading and fracture reductio…

Machine learning-based prediction of the necessity for the surgical treatment of distal radius fractures.

BACKGROUND: Treatments for distal radius fractures (DRFs) are determined by various factors. Therefore, quantitative or qualitative tools have been introduced to assist in deciding the treatment approach. This study aimed to develop a machine learning (ML) model that determines the need for surgical treatment in patients with DRFs using a ML model that incorporates various clinical data concatenated with plain radiographs in the anteroposterior and lateral views. METHODS: Radiographic and clinical data from 1,139 patients were collected and used to train the ML models. To analyze and integrat…

Recommendations for Early Surgical Intervention in Adolescents With Salter-Harris II (SH2) Distal Radius Fractures.

INTRODUCTION: Nonsurgical management is the standard of care for most distal radius fractures involving the epiphyseal plate. However, because of the lack of remodeling potential in adolescents, nonanatomic reduction can result in a symptomatic malunion necessitating late complex surgical correction. Guidelines outlining optimal management strategies for SH2 distal radius fractures in the adolescent population are needed. METHODS: This study is a retrospective review of isolated SH2 distal radius fractures from two, large, regional, referral pediatric ACS trauma level 1 hospitals between 2017…

Variation in Treatment of Young Adult Distal Radius Fractures by Pediatric and Adult Orthopaedic Surgeons.

INTRODUCTION: There remains a lack of consensus on the optimal treatment of isolated distal radius fractures in young adults. The primary aim of this study was to identify differences in treatment of isolated distal radius fractures in patients aged 17 to 21 years treated by adult versus pediatric orthopaedic surgeons. The secondary aim was to identify whether there is a variation in utilization of open reduction and internal fixation (ORIF) versus closed reduction and percutaneous pinning when treated surgically by adult versus pediatric orthopaedic surgeons. METHODS: Patients aged 17 to 21 …

Risk Factors for the Incidence of the Volar Lunate Facet Fragments in Distal Radius Fractures.

Background  The volar lip of the distal radius is the key structure for wrist joint stability. Rigid fixation of the volar lunate facet (VLF) fragment is difficult because of its unique anatomy, and a high rate of postoperative displacement was demonstrated. Purposes  The aim of the study is to identify risk factors for VLF in distal radius fractures (DRFs) and to reconsider the important point for primary fixation. Patients and Methods  One hundred fifty-five patients who underwent open reduction and internal fixation for an DRF were included and classified into one of the following two group…

Supplementary Fixation Improves Stability of Intra-Articular Distal Radius Fractures Managed With a Spanning Plate.

PURPOSE: Previous studies evaluating weight bearing of distal radius fractures treated through dorsal spanning bridge plates used extra-articular fracture models, and have not evaluated the role of supplementary fixation. We hypothesized that supplementary fixation with a spanning dorsal bridge plate for an intra-articular wrist fracture would decrease the displacement of individual articular pieces with cyclic axial loading and allow for walker or crutch weight bearing. METHODS: Thirty cadaveric forearms were matched into 3 cohorts, controlling for age, sex, and bone mineral density. An intr…

Effect of uncertain clinical conditions on the early healing and stability of distal radius fractures.

BACKGROUND AND OBJECTIVES: The healing outcomes of distal radius fracture (DRF) treated with the volar locking plate (VLP) depend on surgical strategies and postoperative rehabilitation. However, the accurate prediction of healing outcomes is challenging due to a range of certainties related to the clinical conditions of DRF patients, including fracture geometry, fixation configuration, and physiological loading. The purpose of this study is to investigate the influence of uncertainty and variability in fracture/fixation parameters on the mechano-biology and biomechanical stability of DRF, usi

[Treatment for non-union scaphoid fractures with headless cannulated screws and distal radius cancellous autograft].

INTRODUCTION: scaphoid fractures comprise 50-80% of the fractures of the carpal bones. Ten percent of the scaphoid fractures evolve to non-union and show degenerative changes in the carpus in 75-97% at five years and in 100% at 10 years. The objective of this work was to evaluate the rate and time to union in patients with diagnosis of scaphoid non-union without fragmentation of the proximal pole after treatment with two cannulated headless screws and distal radius cancellous autograft. MATERIAL AND METHODS: case series with short-term follow-up of four patients with scaphoid non-union withou
