The major mechanism whereby positive end-expiratory pressure (PEEP) decreases cardiac output is believed to be a decrease in the pressure gradient for venous return. However, although PEEP increases right atrial pressure (PRA), It may also elevate mean systemic pressure (PMS), the static circulatory filling pressure that is the upstream pressure for venous return. In an intact canine preparation, we studied the effects of 15 cm H2O PEEP on cardiac output, PRA, and PMS (the equilibrium PRA during ventricular fibrillation). To examine the role of neurovascular reflexes, PEEP was applied before a

The changes in urine flow and renal sympathetic nerve activity (RSNA) due to pulmonary lymphatic obstruction (PLO) were examined in anesthetized, artificially ventilated New Zealand white rabbits. PLO was produced by pressurizing an isolated pouch created in the right external jugular vein at the points of entry of the right lymphatic ducts. During this maneuver, urine flow increased from 8.5 ± 0.3 mL/10 min to 12 ± 0.5 mL/10 min (P < 0.0001) and RSNA increased from 24.0 ± 4 to 40.0 ± 5 μV·s (P < 0.0001). Bilateral lesioning of the paraventricular nucleus (PVN) of the hypothalamus or cervical

We report about a patient (66 years) who was referred to our psychiatric hospital because of a progressive confusional state with acute onset. The colleagues of the referring psychiatric hospital considered a first manic episode as the cause of the symptoms and under therapy with haloperidol the confusional state had shown a progression. The clinical examination's findings were a mild central facial paresis on the right side and a mild hemiparesis on the right side with elevated reflex levels. The patient was disoriented, he had cognitive and mnestic deficits. His reasoning was slowed, incoheren

OBJECTIVE: To emphasize the importance of the mechanism and surgical approach to trigeminocardiac reflex (TCR) developing 48 h after orbital trauma due to a foreign body.
CASE REPORT: After gunshot injury of a 17-year-old male patient, computerized tomography evaluation revealed a right globe perforation and an intraorbital metallic foreign body in the right orbita adjacent to the lateral wall. The ocular perforation was repaired, but the foreign body was not removed. Constant bradycardia (45/min) developed 48 h after the operation. Since there were no cardiological findings, a temporary card

We investigated the effect of single-pulse transcranial magnetic stimulation (TMS) over the occipital pole on a forced-choice visual letter-identification task. Magnetic stimuli were applied on the midline but with the initial current directed pseudorandomly toward either left or right hemisphere; visual stimuli were presented randomly in either left or right hemifield; magnetic-visual stimulus onset asynchrony varied randomly between 12 values: -500 ms and from -50 ms to +50 ms in 10 ms steps. The data revealed the existence of a hitherto unknown fourth task-interfering TMS effect that was ma

A 36-year-old female had a giant congenital pigmented nevus on her anterior trunk. She complained of muscle weakness of the right arm and leg in August 1988, and she was admitted to our hospital to take medical examinations in September 1988. Neurological examination revealed right hemiparesis and exaggerated deep tendon reflexes. Three small falx meningiomas were shown on CT scans and MR images, but they did not appear to be causes of her symptoms. CT-myelography and MR images revealed an intradural extramedullary tumor spreading from the foramen magnum to C2. Oncotomy and C1 laminectomy were

A 76-year-old man presented with a 2-week history of intermittent episodic rotational vertigo accompanied by blurred vision, vomiting, and imbalance. Neurologic examination revealed no sensory, motor, or cranial nerve deficits. However, the head impulse, nystagmus, and test of skew (HINTS) exam demonstrated right-beating nystagmus that intensified with visual suppression, a vertical skew deviation with right hyperphoria, and a normal vestibulo-ocular reflex (VOR) on video head impulse testing (vHIT), except for clustered refixation saccades for rotations in the plane of the left horizontal sem

Hyperglycemia-induced involuntary movements (HIIM) include tremors, hemichorea-hemiballismus (HCHB), and more rarely, dystonia. Presentations may vary, but hyperintensity involving the basal ganglia area on the T1 sequence of MRI brain remains a commonality. We report the occurrence of focal dystonia with uncontrolled hyperglycemia but no focal abnormalities on MRI. On admission, the patient's blood glucose was 861, and she claimed to have never missed insulin dosage. A physical exam revealed no cranial nerve abnormalities and weakness in the right upper extremity with no sensory involvement.

We report the first case of perioperative visual loss due to cortical blindness after supine cervical spine surgery. A 46-year-old female presented with severe right-sided brachialgia of 1½ years' duration. Her magnetic resonance imaging (MRI) (cervical spine) showed severe right foraminal stenosis at C5-6. She underwent C5-6 anterior cervical discectomy and fusion. Nine hours after surgery, during a routine postoperative round, the patient complained of complete bilateral visual loss. The fundus examination and pupillary light reflex were normal. MRI of the brain showed the posterior cerebral

We report an unusual case of electric burns suffered by a 15-yr-old boy. The patient's neck had come in contact with a high voltage broken electric wire and by reflex he had pulled it away with his right hand. He presented with a tracheo-cutaneous fistula with a right-sided pneumothorax. Emergency airway management included insertion of a tracheostomy tube through the traumatic opening in the neck and insertion of an intercostal tube drain. When the diagnostic endoscopy revealed an externally communicating tracheo-oesophageal fistula, protecting the lower airways from gastrointestinal contamin

OBJECTIVE: The appearance of trigeminal neuralgia in infancy is exceptional. Trigeminal neuralgia as the only manifestation of a Chiari malformation is also infrequent. We present the case of a patient with a Chiari type I malformation in which the only symptom was the presence of trigeminal neuralgia since infancy. We have found no similar cases in the literature.
CLINICAL CASE: A 39 year old man with no significant clinical history complained of brief, recurrent episodes of pain in the right maxillary region, compatible with trigeminal neuralgia since the age of eight. The frequency, durati
