Retinal artery obstruction and atheromas associated with non-Hodgkin's large cell lymphoma (reticulum cell sarcoma).

A 71-year-old woman developed branch retinal artery obstruction as the presenting manifestation of a large cell non-Hodgkin's lymphoma. Multifocal chorioretinal scars were present in the same eye. She experienced progressive visual loss accompanied by development of multiple yellow retinal arterial wall plaques, extension of retinal opacification into other quadrants, and increasing vitreous cellular infiltration. Clinical diagnoses included branch retinal arterial obstruction caused by toxoplasmosis retinitis, multifocal choroiditis and panuveitis simulating the presumed ocular histoplasmosis…

Improved visual outcomes of central retinal artery occlusion with local intra-arterial fibrinolysis beyond the conventional time window.

Local intra-arterial fibrinolysis (LIF) is a promising therapeutic option for CRAO. However, the narrow time window of 6 h has greatly limited the application of LIF. In this study, we explored the efficacy of LIF beyond the conventional time windows and compared the result with conservative therapy. This prospective study included 179 CRAO patients with baseline visual acuity (VA) ≤ 20/400 treated at Renmin Hospital of Wuhan University. The mean time from vision loss to presentation was 5.5 days. 58 patients received conventional standard therapy (CST) alone.121 patients underwent LIF. Main o

Visual recovery and OCT evolution following central retinal artery occlusion treated with intra-arterial verapamil and alteplase infusion.

A 69-year-old woman presented with a "veil" over the left eye. Clinical examination demonstrated signs of central retinal artery occlusion. Visual acuity was compromised to 1/200 E in the left eye. Ocular massage and anterior chamber paracentesis failed to improve vision. An emergent intra-arterial catheterization with verapamil and alteplase infusion was performed less than 12 hours following symptom onset. Initial optical coherence tomography (OCT) showed inner retinal edema. One year later, OCT revealed relatively minor thinning, which could explain the patient's visual recovery to 20/40. T…

[Persistent value of intra-arterial fibrinolysis 8 hours or more following central retinal artery occlusion or of its branches].

PURPOSE: To establish that intra-arterial fibrinolysis of the ophthalmic artery can still be efficient 8 hours or more after a central retinal artery occlusion. MATERIAL AND METHODS: Nine to twenty hours (average 12.5 H) after a loss of vision due to a central retinal artery occlusion or the occlusion of its branches, 7 patients were treated by intra-arterial fibrinolysis. The vision of the affected eye was limited to light perception for four patients, to hand movement at 1 meter for two patients, and to 4/10 P2 for one patient. Under radiological guidance, 300.000 IU of urokinase was inject…

Homer1 Protects against Retinal Ganglion Cell Pyroptosis by Inhibiting Endoplasmic Reticulum Stress-Associated TXNIP/NLRP3 Inflammasome Activation after Middle Cerebral Artery Occlusion-Induced Retinal Ischemia.

Retinal ischemia, after cerebral ischemia, is an easily overlooked pathophysiological problem in which inflammation is considered to play an important role. Pyroptosis is a kind of cell death pattern accompanied by inflammation. Homer scaffold protein 1 (Homer1) has anti-inflammation properties and protects against ischemic injury. However, little is known about pyroptosis following middle cerebral artery occlusion (MCAO)-induced retinal ischemia and the regulatory mechanisms involved by Homer1 for the development of pyroptosis. In the present study, retinal ischemic injury was induced in mice

Association of Metabolic Syndrome and Its Components with Retinal Artery Occlusion: An Epidemiologic and Genetic Analysis.

PURPOSE: To examine the association between MetS, its five components (central obesity, hypertension, hyperglycemia, dyslipidemia, and hypertriglyceridemia) and RAO, and to assess whether genetic susceptibility to key components of the MetS (hypertension, hyperglycemia) interacts with these associations. METHODS: Based on the UK Biobank, Cox proportional hazards regression models were applied to assess the associations between MetS, its individual components, and the incidence of RAO. Restricted cubic spline analysis was applied to determine non-linear trends in their associations. Polygenic

Central retinal artery occlusion following transcatheter aortic valve implantation (TAVI) for aortic stenosis: a case report.

BACKGROUND: Transcatheter aortic valve implantation is the standard treatment for high-risk elderly patients suffering from symptomatic severe aortic stenosis. While this procedure is significantly less invasive compared to traditional methods, there are concerns regarding the potential risk of cerebrovascular accidents and ocular complications. We report a case of monocular central retinal artery occlusion that occurred following transcatheter aortic valve implantation due to aortic stenosis. The occurrence of central retinal artery occlusion post-transcatheter aortic valve implantation has n…

Therapeutic Efficacy of Hyperbaric Oxygen in Central Retinal Artery Occlusion: A Systematic Review and Meta-Analysis.

Background/Objectives: Central retinal artery occlusion (CRAO) is a vision-threatening condition with limited evidence-based treatment options. Hyperbaric oxygen therapy (HBOT) has emerged as a potential intervention, but its efficacy remains debated. This systematic review and meta-analysis evaluated the therapeutic efficacy and safety of HBOT in CRAO. Methods: Relevant studies were identified across seven databases using optimized Boolean and MeSH-based strategies. Eligible studies evaluated HBOT in CRAO and reported visual or safety outcomes. Extracted data included demographics, interventi…

Comparative effectiveness of varying hyperbaric oxygen protocols in the treatment of acute central retinal artery occlusion.

BACKGROUND: The comparative effectiveness of different hyperbaric oxygen (HBO) therapy doses in the acute treatment of central retinal artery occlusion (CRAO) has not been evaluated. METHODS: This retrospective cohort study aimed to compare the efficacy of an initial 2.8ATA HBO session (HBO18) to the more prevalent 2.0ATA protocol (HBO10). Excluded were patients with suspected inflammatory arteritis or branch retinal artery occlusion, and when HBO was medically contraindicated. Following the initial session, all patients completed two additional HBO10 sessions within 24 h. The primary outcome…

Investigation of retinal microcirculation alterations following carotid artery angioplasty and stenting using optical coherence tomography angiography.

BACKGROUND: Carotid artery stenosis (CAS) is a common vascular condition that can impair retinal and optic nerve blood supply, leading to ocular ischemic damage. Optical coherence tomography angiography (OCTA) provides a non-invasive method to evaluate retinal microcirculation and detect vascular changes in CAS patients. METHODS: This study utilized OCTA to evaluate changes in retinal microcirculation in CAS patients before and after carotid artery angioplasty and stenting. A 6 × 6 mm OCTA scan was performed to analyze deep retinal vascular complex (DVC) blood flow density, foveal avascular z

Ankle-Brachial Index as a Predictor of Acute Ischemic Cerebrovascular Event After Central Retinal Artery Occlusion.

BACKGROUND Identifying patients at higher risk of acute ischemic cerebrovascular events (AICE) following central retinal artery occlusion (CRAO) is crucial for secondary prevention of stroke. This study aimed to investigate whether a low ankle-brachial index value is associated with an increased risk of AICE after CRAO. MATERIAL AND METHODS We prospectively analyzed patients who were admitted to our hospital because of CRAO between February 2019 and March 2023 and whose ankle-brachial index was no greater than 1.40. We explored the potential association between the index and risk of AICE (defi…
