A 23-year-old woman with recurrent uterine endometrial stromal sarcoma underwent FDG PET/CT, which showed pelvic recurrence and multiple pulmonary metastases. In addition, increased activity in a vertical soft tissue mass along the left ureter was noted, which appears similar to retroperitoneal fibrosis on PET/CT images. However, pathological examination demonstrated that the elevated FDG activity in this mass was a result of tumor thrombus from endometrial stromal sarcoma in the left ovarian arteries and veins.

A 69-year-old man was referred to our hospital for a right renal pelvic tumor noted on a computed tomography (CT) scan at another hospital. Urine cytology was negative. Dynamic CT revealed a right renal pelvic tumor and, accidentally, a small left renal tumor enhanced in the arterial phase and washed out in the venous phase. In this case report, we share our experience of a case with inferior vena cava (IVC) syndrome caused by secondary retroperitoneal fibrosis related to prior pelvic irradiation. A 60 year-old-female who has history of pelvic irradiation developed severe leg edema. Radiological examination revealed obstruction of IVC. Soon after recanalization of IVC with metallic stent placement, her symptom relieved.

We report two cases of iliocaval occlusion caused by retroperitoneal fibrosis; one presented acute symptoms and one chronic. Both were treated by use of transluminal angioplasty and stenting with excellent clinical, hemodynamic, and imaging results at 36 and 51 months. These cases confirm the benefit of endovascular techniques in the treatment of nonmalignant obstructive disease of large veins, and specifically in the case of retroperitoneal fibrosis.
