[Treatment of Herbert B2-type scaphoid fracture with double screws prograde internal fixation through volar approach].

OBJECTIVE: To explore the clinical and function efficacy of the treatment of Herbert B2-type scaphoid fracture through volar approach with double screws prograde internal fixation. METHODS: From December 2005 to June 2010,18 patients with Herbert B2-type scaphoid fracture were treated with double screws prograde internal fixation through volar approach, including 14 males and 4 females with an average age of 33.11 years (ranged from 21 to 52 years). The fracture healing situation was evaluated by X-ray examination. Functional outcome was assessed by using Modified Mayo wrist score. RESULTS:

Pediatric Scaphoid Proximal Pole Nonunion With Avascular Necrosis.

A 13-year-old, right hand-dominant, otherwise healthy boy presented with left wrist pain 19 months after a nonmotorized scooter injury. Radiographs and magnetic resonance imaging at presentation demonstrated proximal pole scaphoid nonunion with avascular necrosis of the proximal fragment. Operative and nonsurgical treatment options were discussed and the family elected for an attempt at nonsurgical management. The patient was placed in a short-arm thumb spica cast, with a window for a bone stimulator, for 14 weeks. At the conclusion of the treatment, the pain had resolved and x-ray and compute

[Bipolar dislocation of the forearm: elbow and perilunate dislocation].

We report a case of bifocal dislocation of the forearm due to posterior dislocation of the elbow and transcaphocapital retrolunate dislocation of the carpus, Fenton's syndrome, associated with fracture of both bones of the forearm. The emergency procedure achieved reduction of the elbow, plate fixation of the radius and ulna, used a posterior and an anterior access to achieve reduction and osteosynthesis of the radial styloid, the scaphoid, and the capitatum and temporary schapholunate, triquetrolunate, scaphocapitate, and radiolunate pinning. At ten months, the patient only complained of mode

Evaluation of Herbert screw fixation for the treatment of displaced scaphoid nonunions.

Ten cases of unstable scaphoid nonunions treated with Herbert screw fixation were compared to nine cases treated with Herbert screw fixation supplemented with a single Kirschner wire (K-wire) across the fracture site. Corticocancellous bone graft from the distal radius was used in all cases to reconstruct the scaphoid. The age of the patients and duration of the nonunions were similar in both groups. Postoperative radiographs demonstrated that only 5 of 10 patients with Herbert screw fixation alone healed, while 8 of 9 patients with Herbert screw and K-wire fixation healed. The time to union a

[Partial intercarpal arthrodesis].

An intercarpal arthrodesis was performed in 21 patients with Kienböck's disease, scaphoid non-union, and instability of the wrist. In eight patients we used the method of Graner, in eight cases the procedure was an arthrodesis between scaphoid and lunate and in five cases an arthrodesis between other carpal bones. Eighteen of these patients were reviewed one to 14 years postoperatively. 30% were pain free, 40% had some residual pain, and 20% had moderate to severe pain. The range of motion in the wrist was restricted mainly in extension. In 40% we found arthrotic changes. Two cases resulted in

Structures at Risk During Volar Percutaneous Fixation of Scaphoid Fractures: A Cadaver Study.

BACKGROUND: Fracture of the scaphoid bone can be treated with cast immobilization or surgery. Historically, surgery was reserved for displaced fractures. However, because weeks of cast immobilization may result in stiffness, loss of strength, loss of bone density and an inability to work or participate in recreational activities for a prolonged period, operative treatment of non-displaced fractures has become increasingly common. Several surgical techniques for fixation have been described, but their risks and benefits have not yet been clearly elucidated. In a study in cadavers, we investigat…

High rate of failure after magnesium bioabsorbable compression screw fixation for scaphoid fractures.

PURPOSE: This retrospective study aimed to evaluate the clinical and radiological outcomes of magnesium (Mg) bioabsorbable compression screws in the management of scaphoid fractures and nonunion. Despite theoretical benefits, such as osteoinductive properties and gradual degradation facilitating bone remodeling of these novel implants, clinical evidence on their efficacy remains limited. MATERIALS AND METHODS: A retrospective analysis was conducted on 20 patients who underwent scaphoid fracture or nonunion surgery with Mg screws at our hospital between 2015 and 2024. Patients with a minimum o

Corrective osteotomy for symptomatic scaphoid malunion.

A prospective study aimed at assessment of the outcome of management of symptomatic scaphoid malunion (hump-back deformity). The work included 13 scaphoid malunions in 13 patients. All patients complained of weak painful hand grip and limitation of wrist function. All cases were subjected to a corrective opening wedge scaphoid osteotomy with insertion of a trapezoid-shaped tricortical iliac bone graft. At the final assessment, after a mean follow-up period of 42 months, the achieved results were rated excellent in seven cases, good in four and fair in two according to the scoring system used.

Four corner arthrodesis limited to the centre using a scaphoid one piece graft and a dorsal circular plate.

We present 20 patients, who had a four corner arthrodesis, from July 2006 to March 2008, using a dorsal circular plate, to treat scaphoid nonunion and scapholunate dissociation with advanced collapse (SNAC, SLAC). The surgical technique was a fusion restricted to the central area filled with one piece of cancellous bone graft taken from the excised scaphoid. Wrist motion, grip strength, and Disabilities of Arm Shoulder and Hand (DASH) score improved after surgery at a mean follow-up of 20.2 months. Fusion occurred in 19/20 patients. Two patients (10%) had persistent pain. The rest had a good c

[CT study of fractures and pseudarthroses of the carpal scaphoid].

The authors analyze a group of 6 patients suffering from fractures and pseudoarthrosis of the carpal scaphoid; some of them were treated with Herbert bone screw. The authors emphasize the utility of CT, as proposed by Sanders (1988), in both pre- and postoperative evaluation of such conditions, since conventional X-rays can provide questionable findings as to both the state of the bone in the proximal fragment and the healing process at the fracture line.

Trispiral tomography for the evaluation of wrist problems.

Trispiral tomography is an x-ray technique that is essential in the diagnosis and management of a wide variety of problems involving the carpal bones and their joints. It facilitates the visualization of acute fractures, particularly those of the scaphoid and hook of the hamate, as well as cysts, sclerosis, and erosive lesions. It is useful in differentiating between acute and chronic changes, and it is often indispensable in evaluating healing after acute fractures or bone grafts to treat nonunions.

Use of Biophysical Stimulation Therapy in Surgically Treated Acute Scaphoid Fractures.

BACKGROUND: Scaphoid fractures remain challenging because of the bone's limited vascularity, particularly at the proximal pole. Although surgical fixation represents the standard of care for displaced and high-risk fractures, pulsed electromagnetic field (PEMF) stimulation has been proposed as a noninvasive adjunctive therapy, with osteogenic, anti-inflammatory, bone-healing, and regenerative effects. METHODS: This multicenter retrospective cohort study, conducted in accordance with the strengthening the reporting of observational studies in epidemiology (STROBE) guidelines, evaluated PEMF st

Comparing Open and Arthroscopic Grafting for Scaphoid Nonunion: Is There Truly a Noticeable Difference?

BACKGROUND: Scaphoid nonunion remains a challenging injury with no clear consensus on treatment. Surgical options, such as bone grafting procedures, are available for the treatment of scaphoid nonunions. While open grafting provides direct visualization, it is theoretically believed to lead to several problems due to the complex ligamentous structure responsible for wrist stability and challenges in the vascular supply of the scaphoid. On the other hand, despite its technical challenges, arthroscopic grafting is thought to avoid complications by preserving surrounding tissues. QUESTIONS/PURPO

Prognostic factors in the treatment of carpal scaphoid non-unions.

In this literature review, the authors analyse the prognostic factors in the curative treatment of scaphoid non-unions. The main negative prognostic factors are smoking, the time elapsed since the fracture, and avascular necrosis of the proximal fragment. If the latter is present, the revascularization by a pedicle or microsurgical bone autograft is probably the treatment of choice. In non-unions without evidence of osteonecrosis, vascularized bone grafts are probably not superior to conventional bone grafts, which can presently be performed under arthroscopic control, with minimal morbidity.

Avascular necrosis of carpal bones.

MR imaging is extremely well suited to detection of early phases of avascular necrosis, permitting diagnosis before collapse of the carpal bones has occurred. The sensitivity of this imaging modality allows differentiation of subtle changes in the bone marrow signal. This is used as criteria for a new stage classification of Kienböck's disease based on MR imaging appearance. The prognosis of scaphoid fractures and estimation of likelihood of avascular necrosis of the proximal fragment can be inferred by using gadolinium enhancement to evaluate the bone marrow vascularity.

Non-union of the scaphoid: Russe graft vs Herbert screw.

The results of treatment in 50 consecutive patients with established non-union of the scaphoid are presented. All patients were treated under the care of a single consultant, for the same indications. Russe bone graft is compared with wedge bone graft plus Herbert screw fixation, in terms of union and function. Overall, a higher success rate was obtained by wedge graft plus Herbert screw, and a significantly better range of movement. However, Russe bone graft appears equally satisfactory for fibrous non-union.
