Previous investigations of a treated model of hemorrhagic shock in the rat indicated the frequent occurrence of bacteremia that appeared to derive from the gut. This paper determines the incidence of bacteremia and endotoxemia during the acute shock period and compares this with similar observations in humans in varying degrees of shock. Studies in 26 rats indicated that bacteremia and endotoxemia was present in 50% and 87%, respectively, by the end of 2 hours at a mean arterial pressure of 30 mmHg. Observations in 50 patients admitted to the trauma unit showed that positive bacterial blood cu

Adequate prehospital care of the severely traumatised patient is important to prevent or attenuate early as well as late life threatening complications, such as tissue hypoxia, ischemia/reperfusion injury and finally multiple organ failure. A mismatch of oxygen supply and oxygen demand is a hallmark in the pathophysiology of multiple trauma. Oxygen supply may be diminished by the following factors: shock-related decrease of cardiac output, anemia and hypoxia. On the other hand, oxygen demand may be increased by pain, panic, and agitation. Hence, it is a central point in prehospital care to red

Tests conducted with 48 dogs and 100 rabbits demonstrated that intravenous administration of novocain has a well-marked positive influence in extreme conditions, such as massive blood losses and traumatic shocks. It is shown that in the mechanism of the drug's action particularly important is its capability to increase the cardiac performance, to eliminate spasms of resistive vessels and to contract the blood-carrying vessels of the microcirculatory stream. The final effect, however, is in a great measure dependent upon the initial functional state of the organism (the period of hemorrhagic or

Natural disease processes can predispose individuals to develop external body lesions that mimic traumatic injury. This can be particularly problematic in pediatric cases in which clinicians are alerted to the problems of child abuse. Streptococcal toxic shock syndrome is a systemic infectious illness that can manifest itself with erythematous and necrotizing skin lesions. We report a fatal case of streptococcal toxic shock syndrome in a 4-year-old girl who presented with skin lesions that were initially thought to be traumatic in origin. A report of possible child abuse was filed with the sta

In acute experiment on 86 adult rabbits with traumatic shock modelled according to Cannon, it was established that intravenous administration of Nalorphine at the doses of 0.5 and 2 mg/kg of weight at the early period of shock contributed to prolongation of lifetime of the majority of animals. In some of them, the small doses accelerated the occurrence of lethal outcome. In administration of 0.1 mg/kg of weight of Naloxone, almost all the animals survived. At the late period of shock, the effectiveness of these preparations is insufficient.

PURPOSE: Traumatic hemorrhagic shock (THS) is a complex, dynamic process and, no consensual definition of THS is available. This study aims (1) to explore existing definitions of traumatic hemorrhagic shock (THS), (2) to identify essential components of these definitions and (3) to illustrate in a pragmatic way the consequences of applying five of these definitions to a trauma registry.
METHODS: We conducted (1) a scoping review to identify the definitions used for traumatic hemorrhagic shock (THS); (2) an international experts survey to rank by relevance a selection of components extracted f

We report a case of hypoglycaemic shock which occurred in a patient 18 h after injury. The patient was involved in a road traffic accident and sustained multiple rib fractures and bruising in the (R) lumbar region. Eleven hours after his last meal he developed sudden weakness, profuse sweating, air hunger, thirst, disorientation and suddenly lapsed into coma 18 h after admission. Administration of 50 ml of 50% dextrose resulted in immediate restoration of consciousness. This case shows that this complication can occur in trauma patients even though the metabolic response to trauma is usually a

BACKGROUND: Traumatic shock activates the hypothalamic-pituitary-adrenal axis (HPA) to mediate a cascade of defensive mechanisms that often include overwhelming inflammatory response and immunosuppression, which may lead to multiple organ failure. Androstenetriol (5 androstene, 3beta, 7beta, 17beta triol-AET) is a metabolite of dehydroepiandrosterone that markedly up regulates host immune response, prevents immune suppression, modulates inflammation and improves survival after lethal infections by pathogens and lethal radiation.
HYPOTHESIS: AET-induced immune modulation will improve survival

Patients with traumatic brain injury (TBI) or head trauma present challenges for emergency physicians and neurosurgeons. Traumatic brain injury is currently a community health issue. For the best possible care, it is crucial to understand the various helpful therapy techniques in the pre-operative and pre-hospital phases. The initial rapid infusion of large volumes of mannitol and a hypertonic crystalloid solution to restore blood pressure and blood volume is the current standard of care for people with combined hemorrhagic shock (HS) and traumatic brain injury. The selection and administratio
