Open reduction and posterior capsular shift for cases of neglected unreduced posterior shoulder dislocation.

BACKGROUND: Neglected unreduced posterior shoulder dislocation is a challenging orthopaedic problem. PURPOSE: To evaluate the results of open reduction with posterior cruciate capsular repair for such cases. This corrects the wide posterior capsule, which is the main pathologic entity. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Over a period of 5 years, 17 patients with unreduced neglected posterior shoulder dislocation with an impression fracture involving less than 25% of the humeral head were treated by open reduction together with posterior cruciate capsular repair as des

Long-term functional results after manipulation of the frozen shoulder.

The use of shoulder manipulation in the treatment of frozen shoulder syndrome remains controversial. Opponents cite the risk of dislocation, fracture, nerve palsy, and rotator cuff tearing as limiting the usefulness of manipulation. A retrospective study of 38 shoulder manipulations in 32 patients was performed. These patients were followed for an average time of 58 months. The patients were examined in follow up for combined shoulder range of motion, external and internal rotation strength, and status of the long head of the biceps. Manipulation was performed in all patients by the senior aut

The prevalence of the complications and their associated factors in humeral lengthening for achondroplasia: retrospective study of 54 cases.

The aim of this study is to evaluate the complications of humeral lengthening and their associated factors. Fifty-four achondroplastic patients were treated by bilateral humeral lengthening. Our original shoulder sling was sufficient to prevent shoulder dislocation. Pre-existing radial head dislocation was observed in 18 patients. Lengthening was accomplished in all cases without a decrease in the elbow function. Seven humeri fractured after the fixator removal. The risk factors for postoperative fracture were a waiting period of less than 5 days, a healing index less than 25, and the concave

Total elbow arthroplasty for distal humeral fractures in the elderly population: good functional outcomes and a high implant survival rate can be expected after surgery.

INTRODUCTION: Distal humeral fractures in elderly patients are difficult to treat due to poor bone quality and high complication rates. Total elbow arthroplasty (TEA) is an alternative for non-reconstructable fractures. This study aimed to evaluate long-term outcomes and implant survival of elderly subjects treated with linked TEA for acute distal humeral fractures. METHODS: A retrospective study was conducted on data from patients who underwent TEA between January 2017 and December 2024 with fractures unsuitable for open reduction and internal fixation (ORIF). Clinical (range of motion, visu

Conservative Treatment of an Isolated Greater Tuberosity Fracture With Dislocation: Management of Post-Traumatic Subacromial Impingement Syndrome and a Mini-Review of the Literature.

This case report evaluates the effectiveness of conservative treatment for an isolated greater tuberosity fracture-dislocation, detailing the treatment process and addressing post-traumatic subacromial impingement syndrome with a mini-review of the literature. A 26-year-old male fell from a height, resulting in a self-reduced dislocated shoulder. Examination revealed extensive ecchymosis, pain, and limited motion in the right shoulder. Radiological assessments showed an isolated greater tuberosity fracture, partial tears of the supraspinatus and subscapularis muscles, a suspected ALPSA lesion,

Inadvertent, intraoperative, non- to minimally displaced periprosthetic humeral shaft fractures in RTSA do not affect the clinical and radiographic short-term outcome.

INTRODUCTION: Little information is available on the clinical and radiographic outcome of intraoperative, non- to minimally displaced humeral fractures that occur during implantation of a stemmed, reverse shoulder prosthesis but are only recognized on routine postoperative radiographs. The goal of this study is to report the clinical and radiographic outcome for this rarely reported fracture type. MATERIALS AND METHODS: 39 conservatively treated non- to minimally displaced intraoperative periprosthetic humeral fractures after stemmed RTSA were detected from our radiographic database between 1

Bilateral posterior shoulder dislocations with reverse Hill-Sachs lesions: a report of two cases and a literature review.

BACKGROUND: Posterior shoulder dislocation accompanied by reverse Hill-Sachs lesions is relatively rare, whereas bilateral posterior shoulder dislocation is exceedingly rare. CASE PRESENTATION: We report two cases of bilateral posterior shoulder dislocations associated with reverse Hill-Sachs lesions. In Case 1, the lesions were managed using the McLaughlin procedure and the modified McLaughlin procedure. In Case 2, open reduction and internal fixation of the left shoulder was unsuccessful; consequently, reconstruction of the right humeral head was performed using bone cement. CONCLUSIONS: Bil

[Clinical picture of anterior instability of the shoulder joint].

The authors have studied the clinical picture of 360 patients with primary traumatic dislocations and the long-term results have been observed in 286 patients. The analysis of 50 arthrotomies of the shoulder performed in cases of anterior instability has been made. A direct relationship between the clinical course of instability and the severity of the intra-articular lesions appearing during the first traumatic dislocation has been established. Such lesions include ruptures or avulsions of the anterior portion of the cartilage wall, ruptures of the joint capsule, avulsion of the capsule from

The biomechanical effect of fibular strut grafts on humeral surgical neck fractures with lateral wall comminution.

No studies have evaluated the effect of fibular strut augmentation on the stability of locking plate fixation for osteoporotic proximal humeral fractures with lateral wall comminution. The purpose of this study was to evaluate the stability of locking plate fixation with a fibular strut graft compared with locking plate alone in an osteoporotic two-part surgical neck fracture model with lateral cortex comminution. Ten paired fresh-frozen cadaveric humeri were randomly allocated into two groups, either the locking plate alone (LP group) or locking plate with fibular strut graft augmentation (LP

Intramedullary Fixation for Proximal Humeral Fractures.

Proximal humeral fractures are a commonly encountered injury; however, no consensus has been reached for the ideal treatment. Current surgical fixation options include plate, plate with fibular strut allograft, intramedullary fixation, pinning, suture constructs, and external fixation. Each of these options possesses distinct advantages and disadvantages. With the evolution of implant design, a greater understanding of the mechanisms of failure of fixation, and the ability to preserve fracture biology, the management of proximal humeral fractures with intramedullary fixation has become an acce
