The POLPSA lesion: MR imaging findings with arthroscopic correlation in patients with posterior instability.

OBJECTIVE: To evaluate the features of the posterior labrocapsular periosteal sleeve avulsion (POLPSA) lesion on MR imaging in athletes with posterior shoulder instability. DESIGN AND PATIENTS: Six male athletes (age range 19-43 years) with avulsion of the posterior glenoid periosteum were identified on MR imaging. There were four football players, one wrestler, and one competitive weightlifter. The weightlifter had a bilateral condition so that seven shoulders were evaluated. MR imaging was performed with a 1.5 T magnet utilizing conventional and fat-saturated fast spin-echo coronal oblique

The clinical presentation of shoulder instability including on field management.

In the athlete, shoulder instability may result from external high-energy trauma or repetitive overuse. Our understanding of how shoulder instability may contribute to and result from shoulder injury has improved considerably during the past decade. In addition, the delicate balance between mobility and stability and the interrelationship between instability and rotator cuff disease make the clinical management of the unstable shoulder and its field treatment a challenge for physicians, therapists, trainers, and others responsible for the care of injured athletes.

Pitfalls in the radiologic evaluation of extremity trauma: Part I. The upper extremity.

Family physicians often are required to evaluate patients who present with acute skeletal trauma. The first of this two-part series discusses the features and evaluation of some commonly missed fractures and dislocations of the upper limb, excluding the hand. Dislocations of the sternoclavicular joint are infrequent and often missed. Clavicular fractures in adults usually are not hard to diagnose. Acromioclavicular joint dislocations represent about 10 percent of all dislocation injuries to the shoulder girdle. Forty percent of all dislocations occur at the glenohumeral joint. Scapular fractur

Overuse injuries in youth sports.

Although youth sports participation is beneficial on many levels, it is also associated with an increased risk of injury. Risk factors for injury in children and adolescents include the presence of growth cartilage, existence of muscle imbalance, and pressure to compete despite pain and fatigue. Overuse injuries, such as patellofemoral pain, Osgood-Schlatter disease, calcaneal apophysitis, Little League elbow, Little League shoulder, spondylolysis, and osteochondritis dissecans, are common injuries in organized sports. However, proper education, supervision, and training can help reduce the ri

Nerve Transfers for Treatment of Isolated Axillary Nerve Injuries.

BACKGROUND: The most common neurological defect in traumatic anterior glenohumeral dislocation is isolated axillary nerve palsy. Most recover spontaneously; however, some have persistent axillary neuropathy. An intact rotator cuff may compensate for an isolated axillary nerve injury; however, given the high rate of rotator cuff pathology with advancing age, patients with an axillary nerve injury are at risk for complete shoulder disability. OBJECTIVE: To review reconstruction of the axillary nerve to alleviate shoulder pain, augment shoulder stability, abduction and external rotation to allev

An Arthroscopic Modification of Coracoclavicular Ligament Reconstruction and Distal Clavicle Fracture Fixation in the Lateral Position.

Distal clavicle fractures and acromioclavicular injuries are common shoulder injuries. Despite the frequency of these injuries, there is no gold standard for treatment. Recent suspensory fixation techniques have minimized the amount of hardware required for surgical treatment of both acromioclavicular separations and distal clavicle fractures. Advantages include reconstruction of ligamentous anatomy, minimal hardware irritation, earlier return to activity, and decreased need for hardware removal. These reconstructions are classically done in the beach chair position in an open fashion. Our pre…

The thrower's shoulder.

Throwers, or athletes who engage in repetitive overhead motions, are a unique subset of athletes that experience distinct shoulder injuries. Athletes engaged in baseball comprise the majority of patients seeking orthopedic care for throwing related injuries. Injuries specific to throwers most commonly involve the labrum and the undersurface of the rotator cuff. In addition, tissue changes in both the anterior and posterior glenohumeral capsule are common with repetitive overhead motions. These capsular changes alter. This article will examine the pathomechanics of injuries to throwers, elabora…

Delays to initial reduction attempt are associated with higher failure rates in anterior shoulder dislocation: a retrospective analysis of factors affecting reduction failure.

INTRODUCTION: Little is understood about the relationship between delay to treatment and initial reduction success for anterior shoulder dislocation. Our study examines whether delays to initial treatment, from injury and hospital presentation, are associated with higher reduction failure rates for anterior shoulder dislocation. METHODS: A retrospective database and chart review was performed for patients undergoing intravenous sedation for attempted reduction of anterior shoulder dislocation in the emergency department (ED). Stepwise regression analysis was performed to identify predictors o

The dangers of snowboarding: a 9-year prospective comparison of snowboarding and skiing injuries.

We studied 2,552 snowboarding injuries and 5048 skiing injuries sustained during 1988-97. The number of snowboarding injuries had been increasing year by year and was 6 times as many as skiing injuries (2.0 versus 0.35 per 1,000 visits). The types of snowboarding injuries included fractures (39%), lacerations (21%), dislocations (17%), and contusions (15%). Upper extremity injuries were more frequent than those in the lower extremity in snowboarders. The commonest fractures involved the radius (48%), clavicle (11%), humerus (11%), and ulna (7-8%). The shoulder joint was most commonly dislocate

Current Concepts in Arm Care Exercise Programs and Injury Risk Reduction in Adolescent Baseball Players: A Clinical Review.

CONTEXT: Awareness of important injury risk factors associated with excessive pitching volume has been highlighted in the literature, but injury rates remain high. Shoulder pain in baseball players is associated with various changes in musculoskeletal movements, which presents as measurable impairments throughout the kinetic chain. Baseball coaches and rehabilitation professionals have utilized exercise programs targeting strength and flexibility of the throwing arm to prevent injuries. The purpose of this review is to summarize the current evidence regarding the effectiveness of arm care exer

Two-stage glenohumeral fusion for the paralytic shoulder.

BACKGROUND: Shoulder fusion after nerve injury can improve overall arm function; however, high revision and low patient satisfaction rates have been described. The purpose of this study is to describe a 2-stage shoulder fusion, first pinning in a position of function and then converting to a shoulder fusion. Our hypothesis is that temporary pinning improves overall satisfaction after shoulder fusion in comparison to satisfaction reported in the literature. METHODS: A retrospective review was performed for patients receiving 2-stage shoulder fusion between 2020 and 2023. Patient demographics a

Needle to scalpel: Navigating the path from SIRVA to surgical intervention.

Shoulder injury related to vaccine administration (SIRVA) is a rare but potentially debilitating condition characterized by persistent shoulder pain and restricted movement that begins within 48 hours of vaccine administration into the deltoid region. Improper vaccine injection into the subacromial/subdeltoid region of the shoulder or glenohumeral joint can cause immune-mediated inflammatory response, adhesive capsulitis, rotator cuff tendonitis or tears, and septic subacromial bursitis or lytic lesions, among other injuries and reactions. The literature to date focuses on SIRVA prevention and

Understanding the Disabled Throwing Shoulder Requires Updated Review of Anatomy, Mechanics, Pathomechanics, and Treatment.

Understanding both shoulder health and shoulder pathology in throwers is relevant to overhead athletes, and also relevant to nonoverhead athletes and even nonathletes. The thrower's shoulder is a clinically relevant topic of medical research scholarship today. Some of the greatest advances in sports medicine result from our ability to visualize normal and pathologic anatomy of joints, including the shoulder, using a small camera and a fiberoptic light source, through small portals, and without disruptive surgical dissection, or, in a word, "arthroscopy." Updates on the thrower's shoulder, incl

Hemodynamic Instability After Axillary Artery Rupture Following Inferior Glenohumeral Joint Dislocation: A Case Report.

BACKGROUND Inferior shoulder dislocation is a rare type of glenohumeral joint dislocation. A serious complication to shoulder dislocation is axillary artery injury, which should be taken into consideration early to avoid potentially permanent damage. Literature on artery injury following inferior shoulder dislocation is sparse. CASE REPORT We report the case of a 71-year-old man with a traumatic inferior shoulder dislocation due to a fall. The patient had a medical history of stroke, and thus had a daily intake of 10 mg Warfarin. Previously, he had reported 2 anterior shoulder dislocations. Th
