Adverse cutaneous reactions are rarely seen with antipsychotics. Exanthematous eruptions, skin pigmentation changes, photosensitivity, urticaria, and pruritus have been previously reported. We report a bruise-like rash in a 4-year-old boy with risperidone use. The rash disappeared on discontinuation and then reappeared after re-initiating the medication. Clinicians should be aware of a bruise-like skin rash as a rare adverse effect of risperidone. This is especially important for pediatric cases where physical abuse may be suspected. The black and brown melanin pigments are synthesized in specialized lysosome-related organelles called melanosomes in melanocytes. Pigmented actinic keratosis and melanoma may exhibit overlapping clinical features, thus representing a diagnostic challenge for dermatologists. Although the differentiation between these two entities is traditionally done by histopathology, dermoscopy has been utilized as a useful additional aid for improving the clinical diagnostic accuracy of such pigmented skin lesions. We report the clinical and dermoscopic features of two pigmented actinic keratoses to discuss the difficulties in their preoperative differential diagnosis.

Black pigmentation of the thyroid gland has been observed in patients chronically taking minocycline antibiotics. Pigment changes of the skin, blood vessels, and bone have also been reported. Some of these changes may be caused by traumatic or toxic etiologies, therefore persons conducting forensic death investigations need be aware of minocycline-related changes. This article reviews pigment changes of the thyroid gland and discusses the potential forensic import of such findings.

The association between vitiligo and melanoma is complex. While the incidence of vitiligo in patients with melanoma is higher, the risk for the reverse, ie, the development of melanoma in a patient with vitiligo, is thought to be decreased.
